Collaborating with multidisciplinary teams, healthcare professionals, and community partners to provide a holistic approach to meet individual needs.
Supporting individuals in maximizing their independence and enhancing their quality of life.
Working closely with health colleagues and community services to ensure that every person receives the appropriate level of support.

Our Ideal Candidate
This position offers an excellent opportunity for individuals with outstanding communication skills and a compassionate manner, who are capable of effectively managing fast paced work. Candidates should have the experience and ability to work under-pressure in a fast paced environment, in order to manage competing demands. We encourage applications from candidates with experience in adult social care and those passionate about delivering person-centred care and prevention strategies to manage risk.
In addition, we would expect the following from all suitable candidates:
Qualified Social Worker with current registration with Social Work England and eligibility to work in the UK.
2 years post qualifying experience working in adult services, assessing and supporting individuals with diverse needs and successful completion of their ASYE.
Sound knowledge of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005 as well as social work theories
Experience in Adult Safeguarding work
Strong assessment, outcome focussed care planning, and risk management skills
Ability to travel across the borough to visit individuals, occasionally being required to undertake visits out of borough
