As an NDIS-registered provider, we are dedicated to delivering high-quality, person-centered services that help participants achieve their goals. We are seeking a dedicated and compassionate Disability Services Officer (Support Coordinator) to become integral members of our Sydney Support team. We are committed to providing exceptional services for individuals living with a wide range of disabilities, empowering them to live their best lives. Our team supports adults and young people with various disabilities, enabling them to live in the community through a variety of housing options.
This role involves providing personal and domestic assistance to clients at home or in the neighborhood. This includes accompanying clients to appointments, shopping, and more. The goal is to assist our clients in enjoying social or recreational activities, building essential life skills, and actively participating in their local community.
Responsibilities
Needs Assessment: Conduct thorough assessments to identify participant goals, strengths, and challenges in line with their NDIS plans.
Support Planning: Develop and implement personalized support plans focusing on independence, skill development, and community engagement.
Service Coordination: Liaise with participants, families, and service providers to coordinate effective support services.
Progress Monitoring: Regularly review and report on participants' progress, making adjustments to support plans as necessary.
Advocacy: Advocate for participants' rights and ensure access to essential resources and services.
Community Engagement: Encourage participants to engage in social, educational, and recreational activities.
Documentation: Maintain accurate records and prepare reports in accordance with NDIS requirements.
Candidates must be available to work in the Western Sydney Region.

Candidate Preferred Requirements
•A Diploma or higher qualification in Community Services, Disability, or a related field.
•Valid NSW Driver’s License & their own vehicle
•Minimum 2 years of experience in disability support or a similar role.
•Strong understanding of the NDIS framework and person-centered approaches.
•Excellent interpersonal, communication, and organizational skills.
•Current NDIS Worker Screening Check (or Willing to Obtain)
•First Aid and CPR certification (preferred).
