Evaluate outcomes of care delivery with the registered practitioner To coach patients and carers to understand the signs, symptoms or situations to be aware of that would indicate concerns to raise with a healthcare professional Inform patients and carers on how to make contact with the relevant services when they feel their condition or needs have changes, including what to do out of hours Effectively utilise a computer and various software applications in order to enter and maintain patient data, communicate with the multidisciplinary team and complete other IT tasks required by the post Discuss support options with patients and carers sensitively and ensure that the haematology team are kept informed of outcomes Liaise with ward teams prior to a patients admission regarding any support needs the patient may require Promote patient involvement relating to the provision of cancer care To participate and display a willingness to learn and support cancer patients and develop relevant technical competency and to keep up to date with current national and local issues and development To participate in departmental audits as required under the supervision of the healthcare team and input data as requested To be responsible for own learning and knowledge relevant to cancer and maintain compliance with all mandatory training as per Trust policy. To participate in regular formal supervision and develop a personal development plan related to the departmental competency frameworks to promote learning and enhancing skills To be responsible for managing own daily timetable of delegated tasks including time for supervision and training. To access and input confidential and sensitive patient information through integrated databases in accordance with the agreed policy and procedure Carry out administrative duties as required by the role. Demonstrate an awareness of the limits of own practice and knowledge and when to seek appropriate support and advice Assist the healthcare team to develop education and information resources for the general public and other health professionals regarding all aspects of cancer and cancer prevention To maintain accurate records/documentation of enquiries while respecting the right to patient confidentiality utilising written and electronic methods i.e. iPortal, MedOnc and Somerset Cancer Registry. Any matters of a confidential nature including particular information relating to patients, their treatment and diagnosis, individual staff records must under no circumstances be divulged or made available to any unauthorised person(s).

To work with the Quality Improvement Team (QIT) to identify, implement and review the systems within the Practice, to maximise income and patient outcomes. To utilise Practice and patient data to improve performance and assist the Practice in meeting targets and improving the provision of health to the Practice population.
