ROLE RESPONSIBILITIES AND ACCOUNTABILITIES BUT NOT LIMITED TO THE FOLLOWING:
- Responsible for initiating and carrying out a variety of administrative and technical duties in support of community care coordination and consult/referral management.
- Provides administrative coordination of patient care, appointments, and logistical factors associated with patient care, inpatient and outpatient, related to the coordination of care between the VA Medical Center and outside facilities, both VA and non-VA.
- Uses a broad working knowledge of all medical center programs and services and of the varying priorities and complexities of outside care for specialty services.
- The complexity of duties may encompass numerous clerical responsibilities, including but not limited to typing, filing, data entry, scheduling appointments and tests, ensuring transfer of medical records from and to the Medical Center and other facilities, ordering office supplies and necessary forms, scheduling patients' appointments and/or consults, tracking patients' appointment status in the community, data tracking, making regular follow-up calls, completing electronic forms.
- Research and maintain an updated database of community health care services to assist Veterans with locating care in the community.
- Assist with processing emergent and non-emergent consults for health care and services in the community.
- Assists the VA Community Care Supervisory MSA with developing contingency plans for data/input/transmission.
- Provide on-the-job training to new employees and assist Advanced MSAs with difficult and more complex situations, providing accurate instructions and workable solutions.
- Maintain VA Office of Community Care-approved electronic systems to track consult/care coordination management for Community Care services.
Work Schedule: Monday - Friday. 8:00AM-4:30PM.

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)
