The post holder will be expected to provide high quality care to patients in the Paediatric Intensive Care, and to provide outreach services to children at risk of deterioration in the inpatient paediatric areas at St Marys Hospital. They will also contribute to the research, teaching and training activities on the Intensive Care unit, including providing educational and clinical supervision. The post holder will work with medical, nursing and managerial colleagues to ensure the continued high performance of the intensive care unit and will share responsibility for junior staff management. They will be expected to participate in governance activities and strategy and business planning activities of the department. The post holder will need to demonstrate excellent leadership skills. They will be expected to undertake the administrative duties associated with the care of their patients. The post holder must be available by telephone and be able to attend the hospital within approximately 30 minutes when on call. Clinical Care The post holder will: Put the patient at the centre of everything they do. Assess and plan the delivery of patient care and take responsibility for implementing and evaluating that care, using an appropriate nursing model Caring for level 2 and level 3. Team Working and Communication The post holder will: Communicate daily with patients, relatives and carers to ensure that they are informed of nursing goals and medical objectives and are involved in decision-making. This requires a high level of interpersonal skill. The post holder will arrange for patients, relatives and carers to see senior nursing or medical staff, as required. Provide support for relatives and patients in a sensitive and respectful manner. Exhibit a thorough awareness of the patient activity in the entire unit regardless of the patient allocation system employed on a shift basis. Additionally, this includes liaison with both community pharmacists (for example to ensure the continuity of supply of medication) and other pharmacists (including the medicines management pharmacists from GSTT). Provide polypharmacy/drug reviews of medications. To prescribe medication safely and in line with local CCG/national guidance including evidence based and cost-effective prescribing. Provide safe, evidence-based, cost-effective, individualised patient care. This significantly includes documentation about resuscitation and advanced care planning (onto recognised registers such as Co-ordinate My Care) of all patients newly admitted to the Nursing Home as well as a retrospective review of already admitted patients where such resuscitation and advanced care planning discussions have not yet been performed. Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills Refer patients directly to other services/agencies as appropriate including social services and allied agencies/services. What you'll do:
- Lead and motivate your team to ensure the championing of and maintenance of a positive local culture within your service

Ensure recovery focused support plans are created, reviewed, evaluated and amended in accordance with changing needs, presenting risks and individual preferences in order to promote recovery
Undertake key working responsibilities for a caseload assigned by the Team Leader/Service Manager
Be flexible and responsive to the needs of service-users as directed by their personal support plan
Provide recovery focused support to achieve personal goals/aspirations in partnership with service-users, family members and statutory agencies. To work with service-users on a one to one basis to achieve positive outcomes within the given service timeframe
Ensure ongoing assessment and management of risks within an attitude of 'positive risk taking'
