The post holder will be expected to provide high quality care to patients in the Paediatric Intensive Care, and to provide outreach services to children at risk of deterioration in the inpatient paediatric areas at St Marys Hospital. They will also contribute to the research, teaching and training activities on the Intensive Care unit, including providing educational and clinical supervision. The post holder will work with medical, nursing and managerial colleagues to ensure the continued high performance of the intensive care unit and will share responsibility for junior staff management. Clinical Care The post holder will: Put the patient at the centre of everything they do. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required. Key Responsibilities Clinical role The post-holder will Make professionally autonomous decisions for which he/she is accountable Provide face to face visits via a clinical ward round during the working week. Provide a first point of contact for Nursing Homes for advice regarding medical/nursing conditions of patients, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports To prescribe safe, effective and appropriate medication as defined by current legislative framework. This would include the prescription of monthly repeat medication, acute medication and the additional/cessation of medication as indicated (e.g. following discharge from hospital or from the advice of other healthcare professions including but not limited to specialist consultants). Additionally, this includes liaison with both community pharmacists (for example to ensure the continuity of supply of medication) and other pharmacists (including the medicines management pharmacists from GSTT). Provide polypharmacy/drug reviews of medications. To prescribe medication safely and in line with local CCG/national guidance including evidence based and cost-effective prescribing. Provide safe, evidence-based, cost-effective, individualised patient care. This significantly includes documentation about resuscitation and advanced care planning (onto recognised registers such as Co-ordinate My Care) of all patients newly admitted to the Nursing Home as well as a retrospective review of already admitted patients where such resuscitation and advanced care planning discussions have not yet been performed. This is likely to be performed with the patient and/or their representatives (dependent on circumstances including where which involving the patient may not be appropriate) and also be done in Best Interest meetings with other relevant healthcare professionals where a patient lacks capacity about advanced care decision making (and where an advanced directive does not exist). Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills Refer patients directly to other services/agencies as appropriate including social services and allied agencies/services. Work with nursing, medical and health care assistant colleagues to ensure that National Service Frameworks (e.g. Coronary Heart Disease/Older People/ Diabetes /Mental Health) are being delivered. A Team Leader will be fully involved in all aspects of the day to day management of their designated service, including line managing the relevant front line staff. - Lead and motivate your team to ensure the championing of and maintenance of a positive local culture within your service
- Deliver effective supervision and team meetings with staff and ensure that information, reporting and communication
- Responsible for the personal development of staff, identifying high performers and feeding into the organisations succession plans
- Ensure that Key Performance Indicators (KPI's) required as part of the Contract are met and recorded correctly
- Develop key business relationships with all appropriate relevant customers/external parties to ensure service delivery excellence and maximise new business opportunities in own patch
- Participate in and monitor the initial and continuous assessment of needs and ensure that support planning is carried out in line with assessed needs of the customer
- Carry out other administrative tasks as relevant to the service and directed by the Contract/Heads of Operations
- Deliver an out of hours on-call service for region, where required
- Ensure that all safeguarding cases are managed in line with safeguarding protocols and are reported to Contract Manager/Head of Operations, as appropriate
- Ensure Look Ahead Health and Safety policies are adhered to at all times and to uphold all health and safety responsibilities within relevant policies and local protocols
- Adhere to all the policies and procedures of Look Ahead Care, Support & Housing plus those specific to the project including any statutory requirements

We're looking for a kind, compassionate and resilient Team Leader to join our Mental Health service in Tower Hamlets

A Team Leader will be fully involved in all aspects of the day to day management of their designated service, including line managing the relevant front line staff. You will lead and develop competence of direct reports as well as carry a small caseload of customers requiring support. You will work closely with the Contract Manager to ensure that the service delivers on all targets, both financial and those around the quality of the service.

- A good understanding of the range of Mental Health diagnoses and how these can present.
