About The Opportunity

As a Patient Care Ambassador, you will be a key member of our Clinic and Medical Centre, being the face of the Medical Centre, as well as handling administrative tasks and contributing to the smooth operation of our facility.

Role and Responsibilities:

The incumbent is a Social Worker in the PCMHI program for the VA Roseburg Health Care System, located in Eugene, Oregon.The incumbent conducts brief mental health and substance abuse assessments, provides consultation to primary care, Behavioral Health, and UCC providers to assess eligibility and appropriate level of care for Veterans in need of services. Incumbent provides brief therapy to individuals, families, and couples with mental health issues. They work closely with the Substance Abuse Treatment Program, PTSD program, Transition Care Management and Behavioral Health Interdisciplinary Program teams to ensure a seamless transition to appropriate services for Veterans.

FUNCTIONS OR SCOPE OF ASSIGNED DUTIES:
A. Clinical Functions (100-90% of time): Incumbent is a professional social worker assigned to the PCMHI Team. The incumbent must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans referred to the team. The incumbent completes thorough psychosocial and risk assessments when the Veteran is initially referred to Behavioral Health. Other social work services include psychotherapy with individuals, couples, and families, crisis intervention, supportive counseling, and referral for resources and benefits. The incumbent:
(1) Must have a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans and/or their families. The incumbent is able to independently work with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems.
(2) Must independently complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. The incumbent must be able to utilize this assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable.
(3) Must actively participate as a member of an interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of a treatment plan.

· Identify and develop potential business opportunities;
