To provide support, give time to an allocated group of carers and thus promote carer support and recovery and promote their place in the broader community. Responsible for providing support to the care co-ordinator, through providing support to the carer and linking any shared objectives into ongoing care planning. The Children Looked After (CLA) & Care Leavers Health Servicework in partnership with Hertfordshire County Council (HCC) Childrens Services to identify the health needs of CLA and Care Leavers.
