A great opportunity to join the integrated mental health team as a senior support worker at HMP Forest Bank .
The successful candidate would provide support and time to an allocated group of service users with secondary service mental health needs who are in custody at HMP Forest Bank. To provide both mental and physical health support to our service users that are under the secondary mental health team. The post requires competencies in physical health such as ECG/taking bloods.
The service users under the team are those with severe and enduring mental health needs and neurodevelopmental conditions within a custodial setting. The successful candidate would also assist Case Managers in their assessment, planning, implementation, and evaluation of individual care plans.
The post holder would also work effectively in providing multidisciplinary care, working with internal and external agencies. The post holder will also provide support to the running of weekly psychiatry clinics with the team visiting psychiatrist.
To provide support and time to an allocated group of service users with secondary service mental health needs who are in custody.
The post holder would have responsibility for providing the link into the care planning process for several individual service users. This will include arranging psychiatric reviews and possibly liaising with other HMP establishments if a service user is transferred.
To ensure that service users engage effectively with the agreed Care Plan and access appropriate services provided on a regular and consistent basis. Maintain an awareness of risk enhancing factors and past risk behavior, ensuring that appropriate consideration has been given to the nature of previous offences where appropriate prior to engagement in 1:1 work. To be alert to any changes in the service users mental state, functioning or social situation and report them to the case manager/line manager. Promote social inclusion through developing opportunities in areas such as employment, volunteering and education aimed at building a positive independent identity and confidence building for move on into the community. Provide support and advice regarding move on options, supporting the individual with regards making housing applications and referring in to appropriate services. Liaison with relevant internal agencies to achieve this. Developing a rapport based upon companionship and befriending but within appropriate and transparent boundaries. Contribute to the CPA process in terms of pre-release planning.
