This rewarding role will require flexibility across the week and your working time will involve working shifts which include evenings, weekends, waking nights and bank holidays. A high level of motivation and personal commitment is required along with demonstrable skills in engaging service users in planning their support. You will also be expected to work professionally alongside Care Managers and health professionals and to communicate positively with families, carers and friends.

Although we value relevant experience of supporting people with a learning disability or autism you do not need to have prior experience if you can demonstrate a ‘can-do approach’, person-centred values, energy and commitment. You will need good verbal and written communication skills and the ability to develop good quality support plans. You will have a strong work ethic and duty of care and be able to contribute to a positive, team approach.

If you do not already have a relevant vocational qualification in health or social care we will support you to complete the Care Certificate and an accredited Health & Social Care Diploma. Our residential support workers provide care and support for 5 individuals with learning and physical disabilities, and other needs to enjoy an excellent quality of life in this residential care home. Each service user has their own personalised bedroom and share a large communal living room complete with an aquarium and arts and crafts station, as well as a large shared kitchen where they enjoy eating meals together.

Individuals living at the service are sociable and active and enjoy lots of interaction with their staff. They enjoy spending quality time with the staff team at the service and out in the community. Examples of activities include walks around the local area, going shopping, sensory activities, shopping in the town centre, day trips out, visiting arts and crafts, and board games.

Support Worker Duties Include:

Team Leader Duties:

- You will lead the onsite care team within our Supported Living Services to provide person centred care and support to older people at the service. This includes providing respectful personal care, emotional support and practical support with activities of daily living,
- You will supervise staff and manage the service rota to deploy staff time efficiently in line with service user needs and preferences,
- You will liaise with families and professionals to plan and review care to maximise outcomes, ensuring the individual is always at the centre of their care and support.

We are looking for team leader to work alongside our existing management team, based predominantly in Poplar Tree Court. Poplar Tree is a service for up to 18 adults. They adults have mixed needs ranging from personal care, moving and handling and also residents who live very independently.

The team leader will be required to lead a team of support workers to ensure that the service provides outstanding care to our residents. They will be responsive to issues, take ownership of audit and quality assurance work, health and safety, and line manage a number of support staff. They will work alongside the registered manager to lead on all aspects of CQC Regulation and internal governance. They will ensure good communication with the wider team and ensure our service users enjoy a varied range of person centred activities, whilst living in a high quality home.
The right candidate will be person centered, organised, and have experience of line management as well as working within a CQC registered service supporting adults with a range of health needs.
