Identify patients requiring medication review and conduct appropriate medication review clinics or home visits if required by the organisation/practices and liaise with carers, Community Health teams or social care staff in order to optimise drug therapy, reduce polypharmacy and minimise unnecessary waste, communicating directly with patients as necessary. Provide medicines use advice and support to GP practice staff, practice registered patients, and the wider practice team such as community staff (district nurses, midwives, and physiotherapists) where appropriate. Help support the safe and effective operation of the prescribing system in the GP practice, including updating repeat medication regimes, issuing of appropriate acute prescriptions, flagging relevant laboratory testing and identifying patients due chronic disease reviews (e.g., annual asthma reviews) Support dispensary and pharmacy teams with clinical patient queries, electronic tasks and dispensing medication (when required) Deal with appropriate prescription queries presentations, such as medications not available due to stock shortages, synchronisation of repeat medications telephone queries from patients regarding their prescribed or over the counter medicines. Provide make every contact count public health education interventions to patients. Provide pharmaceutical support to PCN GP practices or care settings to enable them to develop, implement and monitor an agreed prescribing process plan which meets the objectives of the medicines management plan. Assist practices/ care settings in developing formularies, guidelines and policies to ensure the implementation of NICE guidelines, National Service Frameworks and the requirements of the medicines-related aspects of the GMS contract. Advise GPs, care staff and primary health care teams on risk management processes relating to the prescribing and monitoring of specialist drugs e.g., controlled drugs and those subject to shared care arrangements. Support practices to review specific areas of prescribing, specified by the line management or identified with the GP prescribing lead, including the interpretation of e-PACT data and the use of practice information to review prescribing and identify areas for action. Assist with the development and ongoing review of practice policy with regard to repeat prescribing systems and repeat dispensing to encourage the implementation of prescribing guidance and monitoring guidelines where appropriate. Liaise with Community Pharmacists to encourage them to reinforce changes made, encouraging them to identify savings which can be achieved with regard to generics, dose optimisations etc. and to assist in building their relationship with the practice. Answer medicine related queries from GPs, care settings, practice staff, PALS, social care staff and the general public in a timely and appropriate manner, escalating to and involving other relevant health care professionals within the practice in the absence of national guidance or agreed protocols where needed Provide advice and support to improve the disease registers within each practice to ensure all suitable patients are identified and offered suitable treatment and monitoring for their condition. Participate in multi-disciplinary meetings related to prescribing topics and to engage in the training of other healthcare professionals and practice staff on matters relating to medicines usage. Undertake clinical audits of prescribing in areas identified by yourself and agreed by the PCN or as directed by the PCN, feedback the results and implement changes in conjunction with the relevant practice team. Work innovatively with new technology to facilitate novel ways of working, which may include as part of a virtual hub within the network which provides support remotely to a number of practices, working alongside clinical pharmacists. Respond to MHRA and other patient safety alerts by searching for relevant populations included and implementing agreed patient safety measures in line with the practice prescribing lead. Undertake any other duties appropriate to the grade as allocated by line management.

Responsible to the Lead Pharmacist for HCE, Frailty, and Medicine and Associate Chief Pharmacist Clinical Services for strategic planning and delivery of clinical pharmacy services to patients within Healthcare of the Elderly and Frailty. Support the Chief Pharmacist / Deputy Chief Pharmacist / Associate Chief Pharmacist and Lead Pharmacist in developing, monitoring and reporting on drug expenditure and medicines budget and the delivery of financial savings as part of the pharmacy CIP. Development, implementation and update of clinical pharmacy policy in conjunction with the Lead pharmacist within Healthcare of the Elderly and Frailty. Work as a member of the Clinical Pharmacy Team, Healthcare of the Elderly, and Frailty Service Lines and wider UHP pharmacy team. Monitor, review and report on medicines related incidents within Healthcare of the Elderly and Frailty. Provide highly specialist clinical pharmacy care to patients in the Healthcare of the Elderly/Frailty areas and related units. To manage, lead and co-ordinate the Healthcare of the Elderly and frailty pharmacy teams including junior pharmacists and pharmacy technicians in the provision of a safe, effective and efficient clinical pharmacy service in accordance with local and national standards and strategy. To also ensure all members of the pharmacy team are trained and competent for their roles. To act as a role model as a clinical pharmacist and team leader, to pharmacists, pharmacy technicians, pharmacy support assistants and all other members of the multidisciplinary team. To be a lead and role model in the field of Healthcare of the Elderly and Frailty locally, regionally, and nationally. To practice as an independent prescriber. To work towards advanced clinical practice. To engage with audit, quality improvement, practice and other research. To write and review policies and procedures relevant to pharmacy practice in Healthcare of the Elderly and Frailty. Monitor and evaluate medicines related protocols and guidelines in Healthcare of the Elderly and Frailty. Ensure an appropriate level of clinical pharmacy service is provided to the Healthcare of the Elderly and Frailty. Propose policy or service changes which improve services and may have multidisciplinary and resource implications. Configure and implement inductions for pharmacists in Healthcare of the Elderly/Frailty and the pharmacy department with support from the Associate Chief Pharmacist Clinical Services and Lead Pharmacists. Assist in the development and implementation of PGDs and non-medical prescribing initiatives, when appropriate. Ensure that expertise within your area is seen as a resource within and outside the Trust and form working partnerships with other health and social care service providers. Recommendation of appropriate therapy for individual patients where evidence is not available, controversial evidence exists or other professionals may challenge advice Contribute to the organisations wider antimicrobial surveillance programme, implementing local programmes ensuring appropriate antimicrobial prescribing Attendance at Consultant/GP white board/MDT meetings/ward rounds to actively facilitate and provide advice on treatment, including highly complex patients. Promote compliance by prescribers and other healthcare staff with local/national policies and the South and West Devon Formulary. Application of highly complex knowledge of the evidence base behind drug therapies used. Manage day to day medicines queries, including face to face consultations with patients identified to have compliance or other problems in medicine administration. Refer patients to other community services where appropriate. Liaise with all levels of healthcare staff, patients and carers in all areas of work. Analysis, interpretation and monitoring of clinical data including but not limited to: drug levels, U&Es, blood counts and microbiological sensitivities to guide diagnosis and drug therapy. Assess clinical requirements of patients as a clinical pharmacist and an independent prescriber through interpretation of clinical data and examination. If required request appropriate tests and examinations for patients directly under your care. Contribute to pharmaceutical initiatives that help with the seamless transfer of patients between different healthcare settings. Provide education and training to all healthcare professionals including medical students. Pharmacists are expected to work largely unsupervised and to act on their own professional judgement. Clinical supervision from another health professional can be arranged and is to be encouraged. Clinical roles are supported by policies and guidelines within which the pharmacist is allowed to exercise professional discretion / interpretation. Participate in any wider departmental rotas where needed.

To act as first point of contact in US department and receive, coordinate and communicate information for users within the US Department and Derriford Hospital. This will include staff reporting absences/sickness. The absences will be checked against the current ultrasound rota and gaps identified to be covered. Liaise with the Ultrasound Rota Coordinator to cover gaps in service. It will also include the reporting of equipment faults and faults within the Department. This will be reported either to Estates or the equipment service provider. To keep track of stock levels and order items either via Materials Management or the Eproc system. Crosscheck when items are received and store appropriately. To cross cover the Ultrasound Rota Coordinator in times of absence and update any changes to the rotas/healthroster and link in with the Superintendent Sonographer. To provide cross cover for the existing IP US Coordinator during times of absence, this will include alternate weekend working and bank holiday cover. To create/modify the CRIS diaries to allow the Bookings team to effectively book the outpatient lists. To provide administrative support to the sonographers secondary duties and update shared spreadsheets accordingly, ie ultrasound equipment, non-medical equipment etc. To monitor staff mandatory and appraisal requirements and ensure they are completely in a timely manner. To cross check the ultrasound rotas with healthroster to ensure accuracy. To minute the monthly sonographer staff meeting minutes, as well as ad-hoc meetings that take place. To monitor and administer any Datix received. To monitor the protocols and procedure documents for ultrasound, both paper copies in the rooms and electronic versions on Q-pulse and keep them updated. To maintain spreadsheets and databases relating to the Ultrasound service. To work effectively with US Team including US Operators, including Consultant Radiologists, Radiology Registrars and Sonographers, US bookings teams, and US Rota Coordinator. To work effectively with other staff groups such as porters, ICAs, HCAs, Nurses and Imaging Management Team in order to meet service needs. To attend the Imaging Production Board daily, including weekends and bank holidays where appropriate. To establish good communication links with staff across Imaging and in Derriford Hospital. To be able to identify risks, raise concerns and report incidents according to Trust policy in an appropriate manner to the US and Imaging Management Team where appropriate. To co-ordinate the appointments and patient flow through the inpatient ultrasound scan rooms. To ensure the most effective use is made of US resources and inpatient capacity is optimised. To have an understanding of US operator skills matrix in order to organise appropriate type of examination or procedure for the operator. To engage with ward readiness arrangements. To be familiar with patient preparation for US examinations and procedures. To be able to liaise with Nurses effectively in order to optimise any inpatient capacity for US interventions. To monitor closely inpatient flow and utilisation of US capacity throughout the day and highlight to US Team and Imaging Management team any delays. To have regular contact with ICAs/HCAs working in US and be able to coordinate this staff group for portering where appropriate. To prioritise scans dependent on clinical urgency, patient waiting times and US inpatient capacity. To be able to use multiple computer programmes such as CRIS, Healthroster, Q-Pulse, SALUS, Synbiotix and other platforms as appropriate. To communicate constantly changing situations to relevant parties in a clear and concise manner, via the Microsoft Teams, telephone, email and face to face. The Ultrasound Administrator is a 37 hour per week post and the working hours could be 0830-1700h Mondays, Tuesdays, Thursdays and Fridays.Biweekly Wednesday 0830-1330h with a biweekly Saturday, Sunday and some Bank Holidays. Momentum Instore have an exciting opportunity for you to join us as a Beauty Merchandiser on a flexible basis, working 8-hour days Monday to Friday as and when we have projects (2025).

We have a busy 6 weeks from the start of January 2025 (schedules of work to be given in December) and are looking for key people in your location. Paid training days will take place in December and paid before Christmas.

Work throughout 2025 on a project-by-project basis.

Role and Responsibilities


Visiting retailers and using detailed planograms to update the stand (attention to detail)
Moving and installing new parts around and resequencing stock according to a new planogram
Changing over the visuals/graphics
Maintaining the stand, keeping it looking fresh for customers that visit the store
Travelling to your depot* for kit collection, which is paid time.

The Exeter Nuffield hospital is based next to the RD&E hospital in Wonford, Exeter. Exeter is a vibrant cathedral city in the heart of the beautiful county of Devon, situated on the river exe and only 10 miles from the coast. The hospital has 2 theatres, as well as a hybrid cardiac catheter laboratory/theatre and an Endoscopy suite. There are two ward areas: an inpatient area with 21 beds and a day-case ward with 12 beds. We also have an on-site Pathology department as well as a busy outpatient department, health assessment service, pharmacy, physiotherapy, and radiology department including an MRI scanner.

Successful candidates will be expected to attend a training camp, which is scheduled for February 2025.
