Multi-Disciplinary Teams Overall responsibility for arranging the daily and weekly PCN and Community team led MDT meetings to ensure smooth running of integrated care within the team setting. Key roles include searches and discharge data analysis to identify patients, managing the meeting agenda items; ensuring that all new referrals are identified, and information circulated to team members of the meeting as required. Coordinate and manage the administrative functions of MDT meetings. Liaise with all clinical and non-clinical members in the MDT to ensure effective MDT function. Take minutes of MDT meetings and disseminate; chase progress against actions identified in these meetings and ensure follow up where necessary. Manage reporting required and associated within the DES specifications for required services. Patient Identification Receive and collate information from transfers of care (including hospital admissions and discharges) plus out of hours calls and present this information to the MDT as required. Liaise with service providers and clinicians to identify frequent flyers, and new service users utilising risk stratification tools provided and present this information to the weekly MDT meetings. Support the completion of new referrals by checking criteria, and where criteria have been met, direct referral to the MDT. Signpost team members, service users and carers to relevant services Maintenance of IT based information systems and responsibility for key performance data: To ensure the IT requirements for recording activity are adhered to in collaboration with other team members Accurate update and maintenance of GP systems within the MDT. To provide agreed performance/activity data within the MDT and PCN. Communication and collaborative working relationships Demonstrates ability to work as a member of a team. Is able to recognise personal limitations and refer to more appropriate colleague(s) when necessary. Actively work toward developing and maintaining effective working relationships both within and outside the PCN or group of PCNs. Liaises with other stakeholders as needed for the collective benefit of patients including but not limited to Patients GP, Nurses, other practice staff and other healthcare professionals including pharmacists and pharmacy technicians from provider and commissioning organisations. Work with service users, PCN practices and partners Develop excellent working relationships with all partners, wider service networks including the voluntary sector, GP practices, adult social care, hospitals, community pharmacists and other members of the MDT Acting as a point of contact for residents, families and professionals who visit the care home, such as MDT members and in-reach specialists. Meet regularly with the clinical lead and review case load and MDT function. Keep the MDT and OHP organisation abreast of good news stories. You will provide general secretarial/administrative support to the Doctors, Health Professionals and Practice Manager. This will involve and general administrative tasks including word processing, electronic filing and communication administration. You will be a first point of contact for the surgery and liaise with many agencies and people inside and outside the service. You will represent the surgery at all times in a calm, courteous and efficient manner and demonstrate good customer skills. You will organise the practice electronic, paper and telephonic communications reliably and efficiently and prioritise effectively. You will log and will actively manage incoming requests for information reports and other work and referrals to hospital, communications and other workflows to ensure the practice functions as efficiently as possible. You will log and track payments for work carried out and ensure timely receipt and follow up of defaulters. You will work cooperatively within a mixed team of receptionists, administrators and clinicians and will develop and facilitative good team-working. Job Responsibilities: Secretarial • Provide an efficient, accurate and high quality word processing service for GPs and Health Professionals as required. This includes the typing of letters, reports, patient referrals, minutes, invoices, memorandums. We electronic tools which require editing and formatting before saving, printing and sending. • Ensure that all word-processing of correspondence and reports is dealt with promptly and efficiently in accordance with agreed target times • Manage the patient referral process safely and efficiently including the urgent 2 Week Wait cancer referrals, both to the agreed targets times and to confirm receipt. • Expedite appointments/results/investigations as required by the Doctors • Act as first point of contact for the surgery administrative team. You will liaise with patients, hospitals, Social Services, Health authority, insurance companies, solicitors’ offices and other outside agencies • Take messages for GPs and PM and any other staff members from patients and other agencies ensure the recipient has received them in a timely manner • Handle incoming and outgoing mail passing through the Secretary’s office efficiently • Maintain workflow logs and manage your workflow efficiently, alerting the appropriate person when their work is outstanding and alerting the PM if a backlog of your work is likely • Manage the Safety Netting process for the practice. Key Responsibilities

• Dispense prescription medications accurately and efficiently, ensuring compliance with all legal and ethical guidelines
• Provide medication counselling and support to patients, answering their questions and addressing concerns
• Collaborate with healthcare professionals, including doctors and nurses, to optimize patient care
• Conduct medication reviews and ensure safe and effective medication management
• Maintain accurate patient records and manage pharmacy inventory
• Provide immunization services (where applicable)

Medication Review and Optimisation: To respond to reception queries to amend prescriptions, reauthorise repeats, clarify doses, clarify products, give appropriate alternatives when availability issues occur To discuss specific patient and prescribing issues (e.g. complicated regimes/polypharmacy, compliance difficulties, multiple adverse effects, medication reduction regimes) with other clinicians as necessary Face-to-face clinics: to identify patients in need of medication review and develop he system of patient invitation with the admin team, and see patients face-to-face for medication reviews and to discuss specific issues, such as those described in the previous point To discuss medication issues with patients as needed and appropriate by telephone, e.g. adverse effects/interactions, overdose/inadvertent ingestion, OTC remedies, queries from reception, queries from care homes Prescribing Pharmacists will also complete DTA forms for District Nurses Medicines Reconciliation: Working alongside the Pharmacy Technician: To review secondary care requests for new medication (as communicated by discharge summaries, outpatient letters, etc.) and raise any queries with the relevant GP, and discuss how to respond when inappropriate prescribing requests arise. To review and continue repeat medications for new patients who have just registered with the practice, and suggest GP medication review where appropriate. Medicines Information/Education: To monitor and inform colleagues as relevant about ongoing prescribing issues, e.g. new guidelines (national and local), new products being requested by secondary care, manufacturing and supply problems, new prescribing restrictions or contradictions, and individual and systematic errors made by colleagues To advise on cost effective prescribing and prescribing budget issues Prescribing Systems and Policies: To liaise with reception, admin and clinical staff in discussing, developing, and implementing medicines management systems in the practice To work with the GPs (especially the Clinical Lead) and Practice Management to review, develop and implement prescribing policies and strategies for the whole practice To suggest and design audits in relation to prescribing targets, implementation of locality policies and the Quality Outcomes Framework (QOF) To work with the GPs, Practice Management, and Data Team on choosing and delivering targets for the local Prescribing Quality Scheme (PQS) To monitor the practices prescribing performance (via the CCG prescribing dashboard, progress on the PQS, ePACT, etc.), and suggest changes to practice prescribing as appropriate Chronic Disease Medicines Management: According to experience and training, this may include: Annual Asthma reviews with patients Hypertension (lifestyle advice and medication initiation and optimisation) Diabetes, with referral to GP/Community Diabetes Nurse CHD (medicines optimisation, with referral to GP where necessary) Depression reviews Heart failure Liaison with community and hospital pharmacies: To support and advise practice teams when needed Communicate updates and changes in practice policy Liaise when medicine availability issues occur Prescribing: Having achieved the Independent Prescriber qualification: To undertake where appropriate and within clinical competency To keep clinical competency up to date and inform others of changes (the reception team and other clinical colleagues) To sign Patient Specific Directions and GR as appropriate To pass prescribing tasks to an alternative clinician if without area of competency Teaching/Training: We are a training practice, and often have learners (GP trainees, medical students and Parenteral Nutrition trainees) sat in on consultations and occasionally having tutorials one-to-one. Responsibilities of a Parts Supervisor
·Supervision of parts department, staff and workload
