Obtain a set of case notes for every patient attending specified clinics. Prepare case notes to ensure they are tidy and contain complete information including a referral letter, up to date results, patient labels and blank continuation sheets. Where patients have had a test or and an X-ray prior to their clinic appointment, the results should be traced and obtained and made available at clinic. Ensure all case notes are tracked to the appropriate location using the Case Note Tracking functionality on PAS and hand written logs where necessary. Provide a professional, efficient and friendly reception service for patients and relatives attending clinics. Ensure patient information is accurately recorded on PAS including attendance, cancellations and Do Not Attends. Clinic outcomes should also be recorded in accordance with the Trusts policy and procedure. Ensure all follow up clinic appointments are either booked on PAS or added to the appropriate outpatient waiting list. Collect additional information as requested, with the aim of monitoring the quality of the service provided in accordance with the Trusts Performance Management Framework. Obtain information on each patient attendance and in a timely manner accurately record on the Patient Administration System in line with Trust Policy. Respond to the receipt of test requests and follow-up, where appropriate. Provide suitable cover for other clerical staff in cases of absences, as delegated by the Assistant Patient Services Co-ordinator. Assist the member of staff responsible for stationary stock in ensuring an appropriate level is maintained. You should be qualified to HND level or have equivalent experience in a relevant area including experience in the safe operation and basic maintenance of A/V & A/V related equipment: video cameras, DSLRs, audio recorders & microphones, tripods & stands, projectors, media players, etc. with an understanding of the creative use of this equipment.

Review and further develop processes and systems that support quality practice learning across the healthcare community. Be responsible for developing and facilitating effective and efficient partnerships/networks locally and nationally with the view to sharing best practice. Act as a conduit between the Higher Education Institutions (HEIs) and placement providers from within NHS and non-NHS organisations. Engage in the preparation and delivery of educational programmes which impact on the quality of practice learning and student progression in practice. Provide immediate and strategic views on issues relating to practice learning. PRINCIPAL DUTIES & AREAS RESPONSIBILITY: To be an expert resource for colleagues seeking advice and guidance on issues relating to practice learning. Lead on the design and implementation of quality monitoring processes in collaboration with the education and placement providers. Lead locally in support of the strategic direction of nationally and locally driven quality assurance methodology in relation to practice learning. Lead locally on the implementation of the quality assurance national requirements which impact on practice learning. Create robust communication networks for practice learning issues to be raised between NHS and non-NHS service providers, Higher and Further education and Health Education England as appropriate. Promote effective partnership working with stakeholders Identify and create opportunities for practice learning within a multi-professional environment that meets the needs of the learning outcomes of the professions involved. Further develop existing placements and create new and innovative opportunities for students. Increase the number of quality learning opportunities within the community, mental health and primary care setting. Work with HEI and service providers to ensure the number of mentors/assessors/practice supervisors across the healthcare community is sufficient and appropriately prepared in terms of mentor updates, sign-off mentors, and triennial review, to support the numbers of students in practice now and in the future. Advise the development of effective support systems for mentors/assessors/practice supervisors. Advise and support the development and implementation of new models of placement learning.

As a HCPC Registered Biomedical Scientist, the post holder will be a member of the departmental team and assist in the delivery of a quality driven, cost effective, patient focused service consistent with LTHT Pathology objectives. To provide that service in a manner commensurate with the profession The post holders main role is to carry out the examination of pathological samples using various technologies and to report or present the results as appropriate. Working in different sections of the department you will gain the knowledge and skills necessary to contribute towards the provision of an accurate and timely service for patients. You will be expected to apply professional judgements and specialist skills to technically complex and sometimes difficult situations, and how to work, often under pressure, either independently or as part of the team. You will be required to communicate information, often related to patient management and care, to colleagues, healthcare staff and other organisations. This entails contracted participation in a 24/7 rota and / or the out of hour rota. The post holder will be required to work cross site locations (LGI & SJUH) if the service requires it.
