The Registered Nurse - Polytrauma Transitional Rehabilitation Programs Admissions Coordinator will work with the interdisciplinary team for the coordination of all admissions to Polytrauma Transitional Program (PTRP), The Servicemember Transitional Advanced Rehabilitation (STAR), and the Intensive Evaluation Treatment Program (IETP) programs in Building 514 and serves as a liaison between referral sources.

This staff works closely with our Active-duty service members, veterans, our stake holders as well as our community referral sources. The Registered Nurse - PTRP/STAR/IETP Admissions Coordinator will work with the Medical Director of the Programs, Nursing Staff, Social Work, and bed flow to coordinate all admissions to the programs.

Duties include, but are not limited to:

-Triage and screening of requests for admissions to the PTRP/STAR/IETP programs.

-Informing the interdisciplinary team of pending admissions as soon as information becomes available.

-Pre-admission education of patients and families regarding the program.

-Obtaining pre-admission authorization to the programs.

-Orientation of patients and families, and staff to the program.

-Responsible for completion of documentation of assessments required by the RN to include medical review in CPRS and JLV.
