We are looking to appoint a highly motivated, innovative and enthusiastic Senior Pharmacy Technician (Clinical Services).

Your role will involve working with the Chief Pharmacy Technician (Clinical Services) to manage and develop our technical ward services team.

This role requires excellent organisational and interpersonal skills, alongside sound knowledge of clinical ward services. Relevant experience and good written and verbal communication skills are also essential.

You will alsosupport the Chief Pharmacy Technician (Clinical Services) to manage and co-ordinate the technical staff on wards, implement omnicell systems at ward level and manage the satellite dispensaries on wards.

In addition, you willbe required to provide medicines management cover to clinical ward services; completing medicines reconciliation, discharge planning and final accuracy checking of prescriptions.

The department operates a 7 day service to the Medical Care Group. Participation in the late night working, weekend and Bank Holiday rotas will also be required.

KEY DUTIES
• To work under the supervision of the Chief Pharmacy Technician Clinical Services to manage the operational provision of pharmacy services to wards and provide cover in their absence.

• To work with the Chief Pharmacy Technician Clinical Services to performance manage band 5 pharmacy technicians and other staff allocated to ward services within the department including:

- The recruitment and selection process of pharmacy technicians and assistants

- Ensure all direct line reports have an annual appraisal and appropriate personal development plan which is in line with the strategic objectives of the department, ensure this is reviewed and updated in regular 1:1’s

CLINICAL & PROFESSIONAL RESPONSIBILITIES
• To perform final accuracy checking on prescriptions and inpatient requisitions which have been clinically verified by a pharmacist in accordance with Trust procedures and ensure dispensing errors are corrected by the appropriate member of staff.
• Labelling and dispensing of medication as required including the accurate input of data into the pharmacy JAC computer system, extemporaneous dispensing, and blister packs.
• To provide a specialist clinical technical service to a designated ward, in accordance with professional, departmental and Trust policies. This will include:

- the taking of a comprehensive medication history for new patients as soon as possible after admission using multiple sources of information

- Close liaison with external organisations e.g. nursing homes/community pharmacies, other hospitals, and GP practices

- Assessment of patients own drugs for reuse.

- Accurately transcribe medicines from EPMA to ICE discharge identifying any discrepancies to the ward doctor and ward pharmacist

- Processing of TTO’s at ward level in a timely manner to maintain patient flows in the Trust.

- Ordering medication in a timely manner to avoid missed doses, particularly of critical medicines.

- Monitoring for adverse drug reactions and submit ‘yellow cards’ to the CSM where appropriate.

- Counselling and educating patients on how to correctly take their medicines including advice on side effects and appropriate storage.

- Identify unintentional nonadherence to medicines and intervene where appropriate.

- Follow up patients to ensure interventions put in place have been successful.

- Monitoring for adverse drug reactions and submit ‘yellow cards’ to the CSM where appropriate.

TEACHING & TRAINING RESPONSIBILITIES

- Transcribing of discharge prescriptions

Ensure that equipment is used in accordance with departmental policies and suppliers instructions. Undertake routine and more complex maintenance procedures on the analysers within the Department. To carry out appropriate internal maintenance procedures and maintain maintenance records Undertake the repair of complex laboratory analysers and installing replacement components as requested by the supplier. Ensure prompt technical validation and authorisation of multiple test sets from laboratory investigations ensuring accuracy and precision as specified by laboratory protocols. Also to assess the validity of the results as to whether conflicting information is due to the pathology of the patient, contamination, or compromised integrity of the sample prior to release of the results for clinical validation. Ensure the accuracy and precision of laboratory investigations within the section using appropriate quality control procedures. Carry out corrective action and record when the quality control procedures indicate loss of performance. Reporting to Operational Manager as necessary. TEACHING & TRAINING RESPONSIBILITIES Section Leads have responsibility for the day-to-day supervision, education, and training of Biomedical Scientists (BMS), trainee BMS and Medical Laboratory Assistants (MLA) Section Leads must ensure the delivery of training, development and competence and proficiency assessment for qualified, trainee and probationary Biomedical Scientists, MLA, and locum staff. Assist as required by the Departmental Manager in the organisation of, and delivery of formal training programmes for BMS, Trainee BMS and MLA staff. Undertake tutorial sessions to all department staff and to help complete staff competency portfolios. Act as mentors for Biomedical Scientists, Trainee Biomedical Scientists and Medical Laboratory Assistants. LINE MANAGEMENT/SUPERVISORY RESPONSIBILITIES Supervise the day to day running of a section of the department, organising and prioritising the workload, allocating workload and resources to ensure the section runs as efficiently as possible with the available resources. Supervise staff and maintain the skill mix and staffing levels appropriate to the needs of the section on a day-to-day basis. Applications are sought from Biomedical Scientists who have current HCPC registration and meet the person specification. It is desirable that applicants have either an MSc with a Transfusion specialism, an IBMS Higher Specialist Diploma in Transfusion Science or a Fellowship of the Institute of Biomedical Science (FIBMS) in blood transfusion or transfusion science.

KEY DUTIES

Technical
• Undertake manual, semi-automated and fully automated laboratory investigations on a rotational basis play a lead role in Blood Transfusion.
• Ensure that equipment is used in accordance with departmental policies and supplier’s instructions.
• Undertake routine and more complex maintenance procedures on the analysers within the Department. To carry out appropriate internal maintenance procedures and maintain maintenance records
• Undertake the repair of complex laboratory analysers and installing replacement components as requested by the supplier.
• Ensure prompt technical validation and authorisation of multiple test sets from laboratory investigations ensuring accuracy and precision as specified by laboratory protocols. Also to assess the validity of the results as to whether conflicting information is due to the pathology of the patient, contamination, or compromised integrity of the sample prior to release of the results for clinical validation.
• Ensure the accuracy and precision of laboratory investigations within the section using appropriate quality control procedures.
• Carry out corrective action and record when the quality control procedures indicate loss of performance. Reporting to Operational Manager as necessary.
• Prepare reagents required for laboratory investigations.
• Inform the Operational/Service managers of any event which may cause a service delivery failure.

Diagnostics
• Use interpretive skills to determine the clinical significance of results of laboratory tests, for example, by:

- Preparing, examining and reporting on the analysis of specimens.

- Performing diagnostic tests by various methods
• Interpret diagnostic laboratory results and take appropriate actions by:

- Authorisation and reporting of results.

- Adding technical comments.

- Referring results to medical staff for a second opinion as necessary.

- Informing the requestor/medical staff of clinically significant and critical results.

Policies & Procedures
• Ensure the standard operating procedures (SOPs) are in place and adhered to.
• Participate in the regular review, production, and update/amendment of SOPs.
• Ensure the implementation of and compliance with all established Trust and Departmental policies.

Clinical Governance & Quality
• Lead on governance/quality issues for the section working with the operational leads and service lead to ensure all aspects of governance are monitored and maintained in required order.
• Undertake method and laboratory instrument evaluation and produce statistical reports.
• Take appropriate action should the quality assurance systems be outside acceptable limits.
• Perform External Quality Control and be aware of the External Quality Control performance for the section.
• Present internal and external quality control performance and recalibration data at monthly departmental meetings.

Communication
• Communicate results verbally to persons authorised to receive them in a clear, concise and empathic manner, following appropriate policies and procedures.
• Provide advice and information to Laboratory, Ward nurses and clinicians, GP staff and patients regarding samples & sample handling including information on appropriate tests and collection conditions for in house, referred & infrequently requested tests.
• Deal with result queries imparting information to clinical users where information may be complex and difficult to understand.
• Communicate any relevant information to laboratory colleagues within the section and to inform effectively at hand over.
• Actively participate in team meetings and as required by more formal means such as presentations.
• Communicate Team Brief to Section and other departmental staff.
• Represent the Department on Directorate and/or Pathology related Trust groups as directed by the Departmental Manager.

TEACHING & TRAINING RESPONSIBILITIES
• Section Leads have responsibility for the day-to-day supervision, education, and training of Biomedical Scientists (BMS), trainee BMS and Medical Laboratory Assistants (MLA)
• Section Leads must ensure the delivery of training, development and competenceand proficiency assessment for qualified, trainee and probationary Biomedical Scientists, MLA, and locum staff.
• Assist as required by the Departmental Manager in the organisation of, and delivery of formal training programmes for BMS, Trainee BMS and MLA staff.
• Undertake tutorial sessions to all department staff and to help complete staff competency portfolios.

This post is for a Senior Clinical Fellow in Urology. Offering an exciting opportunity for a highly motivated Medic to join a dynamic, rapidly expanding team delivering multidisciplinary acute care in St Helens & Knowsley Teaching Hospitals.

The Senior Clinical Fellow will undertake elective and non-elective work in Urology. The successful candidate will have ample opportunity in elective, non-elective, and diagnostic activity. The clinical Fellow will work with more than one consultant.

Applicants must be eligible for full Registration with the GMC at time of appointment.

The Urology Directorate has a Clinical Director and a Directorate Manager. Along with other surgical specialties, the Urology directorate forms part of the Surgical Care Group.

Whiston Hospital is the base for the Urology Directorate. Non-elective and inpatient elective activity is concentrated at Whiston Hospital. Day case treatment and diagnostic urological activity is concentrated at St. Helens hospital. Some urological clinics take place at nearby Newton Community Hospital.

This Urology department is in collaboration with its counterpart at the Southport and Ormskirk NHS Trust. The 2 departments have a joint out of hours urology on call arrangement. Two of the current consultant urological surgeons are joint appointment between the 2 Trusts. All of the consultants Urological Surgeons have complimentary honorary contracts with the 2 Trusts.

The Trust delivers acute hospital care, intermediate care, community, and primary care services to a population of over 600,000 people with a combined workforce of around 9000 dedicated and skilled staff from 17 locations including Whiston, Southport & Formby, St Helens, Ormskirk and Newton hospitals.

The Clinical Fellow in Urology will be expected to:

Contribute to the care of both out-patients and in-patients.

Participate in the urology middle grade on call rota.

Help to supervise the work of the FY2 and FY1.

Help with emergency work.

Assist in the compilation and organization audit sessions at Whiston Hospital

Deputize during the day time for absent colleagues.

Assist with duties outside those normally roistered but which are judged to be essential for patient care, under exceptional circumstances.

CLINICAL To hold a caseload of both specialist and generic work dependant on clinical competence and experience. To undertake assessments, treatment and consultation work based upon a conceptual framework and employing research based evidence. To provide direct therapeutic interventions in line with the grading of the post, including assessments, formulation and therapeutic input into children, young people and families referred into the service. Undertake risk assessments and care plans accordingly, in line with the THRIVE framework To work in ways that are sensitive to the needs of children and young people families from a wide range of racial, cultural and religious backgrounds. To be responsible for the screening of referrals to children and Young Peoples services. To participate directly in the teams duty rotas. To exercise responsibility and autonomy, within supervisory, managerial and accountability structures, for the assessment, treatment and discharge of clients To deliver care co-ordination, ensuring effective assessment, care planning and risk management processes are maintained. Involve the views of children, their families, referring agents and other professionals involved in the network of care including organising multi-agency meetings. To produce a clinical formulation and intervention plan founded on evidence based practice in co-operation with the child young person and carers To hold responsibility for standards of clinical practice within area of specialist knowledge To work jointly and collaboratively with other team members in order to enhance and develop the provision of services to young people and their families. To have an effective knowledge base of other modalities so as to engage and work effectively with colleagues in other disciplines. To provide consultation, advice and guidance to junior colleague. To promote effective multi-agency work. To contribute theoretical knowledge to the process of managing risk within the multi-agency network Undertake risk management for all children including the protective and risk factors present in the network of significant relationships. Identify and respond appropriately to child protection issues, as described with the guidelines of Local Safeguarding Childrens Board and Northwest Boroughs Healthcare NHS Foundation Trust safeguarding policies Maintain clear and comprehensive electronic clinical records, to ensure information is accurate and complies with Data Protection Act and professional body and Trust standards. To engage with using routine outcome measures as clinically appropriate including their use in clinical practice and inputting data into clinical records. To work collaboratively with children, young people and their families and contribute to the trusts service user participation agenda. COMMUNICATION To communicate in a skilled and sensitive manner information concerning the assessment, formulation and treatment plans of clients and to monitor and evaluate progress during the course of both multi and uni disciplinary care. To effectively communicate professional opinions, verbally and in writing to other relevant professionals. Attend Child Protection Case conferences as a representative from children and young peoples mental health services. Provide and receive highly complex, sensitive or contentious information to children, young people and families and to other professionals with in young peoples services. Communicate effectively with children, young people and their carers, when in crisis and in challenging family situations Organise and facilitate multi-agency meetings and reviews Elicit highly sensitive information using engagement/ communication skills which are appropriate to the young persons stage of development Work, as part of an integrated multi professional team, ensuring good communication and working relationships with other professionals Provide written reports detailing the progress of the mental health intervention. TEACHING, TRAINING AND SUPERVISION. Participate in regular supervision, engage in reflective practice and committed to continued development. To provide training to other professionals to improve mental health knowledge and skills in line the transformation of children and young peoples mental health services To provide specific training and /or presentations to other agencies, statutory and voluntary, and promote and deliver specialist training programmes To contribute to training placements for a range of professional disciplines from different background so that trainees acquire the necessary skills, competencies and experience to contribute effectively to positive change. To assess trainees competencies and write placement reports for trainees in line with Trust, professional body regulations Work collaboratively with partner agencies within the principles of THRIVE. This will include joint working; consultation; and training aiming to develop joint working across agencies. Maintaining registration / accreditation requirements in accordance with the appropriate professional body in line with their professional qualification. Be accountable for own clinical practice and professional behaviour MANAGEMENT, POLICY AND SERVICE DEVELOPMENT Identify priorities within and initiatives within the service and advice managers on the level of resources for service development. Contribute to the services evidence based practice by engaging with audits and evaluations as appropriate To contribute to recruitment processes by assisting with shortlisting, interviewing and the induction of recruited staff. To support the workload of colleagues within the service through co-working practices. Contribute to service developments. RESEARCH AND SERVICE EVALUATION To maintain an up to date knowledge of the evidence base in relation to mental health knowledge and other interventions. To contribute to planning and implementing systems for evaluation and monitoring of therapeutic practice and outcomes of clinical work within the service. Participate in research, evaluation and audit in response to the clinical and professional interest of the post, and to monitor standards, quality and consumer reaction to the provision of services in accordance with NICE guidance Participate in appropriate audit. Quality assurance programmes and research as directed. CONTINUING PROFESSIONAL DEVELOPMENT To receive and engage in regular clinical supervision in accordance with Trust Clinical Governance Standards and the Code of Practice and ethical guidelines of professional body To contribute to the development and maintenance of the highest professional standards of practice through active participation in internal and external CPD training in consultation with the post holders service manager to meet professional body requirements for CPD and registration. To contribute to the development of best practice in the service by taking part in regular supervision and appraisal and maintaining awareness of current developments in the field. To maintain professional body registration.

To undertake comprehensive holistic assessments, devise treatment plans individualised to patient needs, reviewing and adapting during patient journey from initial assessment through to discharge. To gain consent through engaging patients / clients through their therapy journey, enabling them to experience choice and as part of their recovery. To prioritise designated Physiotherapy / Occupational therapy referrals according to need, risk and service capacity To plan and implement patient centred individual and/or group interventions, using graded activity to achieve therapeutic goals including sensory integration difficulties within virtual ward/rehab hub setting. To monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention. To apply a high level of understanding of the effect of disability and provide training and advice on lifestyle changes and adaptations to the patients social and physical environment. To provide clinical advice, expertise and leadership within your team. To monitor and offer professional advice and leadership to all team members with regards to their caseloads and working practices. To ensure that the service users care plan is developed to meet the assessed need of the individual. To monitor, via performance management, the care plans developed for service users by all sector members to ensure that they are appropriate and of a high standard. To liaise with family members, carers, GPs and other professionals asnecessary when completing the assessment and developing a care plan. To be a point of contact for service users and carers that have concerns regarding their care, making every effort to resolve their concerns sensitively and promptly, referring to and informing the Team Manager as necessary. To develop a risk management plan for service users under your care, taking note of the contributions from other members of the team. Ensure that all significant people are aware of the risk management plan and are actioning it appropriately. To monitor and work with all team members to ensure that service user risk is managed appropriately and that risk management plans are of a high quality. Understanding risk assessment, being able to assess risk and complete management plans. To lead the systematic monitoring and evaluation of service users under the care of the sector you have responsibility for. Monitor and ensure high standards relating to the planning and delivery of Physiotherapy / Occupational therapy activities and comply with The Professional bodies associated with each profession, Code of Ethics and Professional Conduct, contributing to good health and the importance of promoting these in line with organisational public health policy. To monitor the provision of a range of therapeutic activities for service users by supervision of sector members caseloads and via audit. To use skills gained through training and experience to de-escalate situations where service users become physically or verbally aggressive. Use your judgement, gained from experience, to establish if a service user requires assessment by a medic or possibly hospital admission. To use skills gained through experience to deal with service users who become anxious, hostile or distressed. To ensure all sector members get appropriate support following violent incidents and liaise with the Team Manager to ensure any identified actions arising from such incidents are implemented. To participate in and/or lead service user reviews ensuring that service user needs are met and that the team has all the relevant information on which to base their clinical decisions. To develop care plans that enable service users to reach and maintain their optimum level of health and independence to help them to remain in the community in their chosen setting or return to this on discharge.
