Salary: Main Band 9, £36,647 to £40,220 per annum, or Senior Band 10, £40,220 to £44,428 per annum
Contract: Permanent
Hours: Full time (37 hours per week)

The Service:
There have been exciting changes within Portsmouth Adults Intermediate Care Team, which went live on 1 April 2024 and has brought together a number of teams to provide a single approach to supporting people who are moving through admission avoidance, the acute hospital, inpatient settings, and rehabilitation and reablement services. This is an exciting time to join us and help us on our journey to improving outcomes for people following a crisis or decline in health.
The post is based within the Portsmouth Adults Intermediate Care Team supporting our integrated services including, Community nursing, Urgent Community Response, Portsmouth Rehabilitation and Reablement team, The Transfer of Care Hub, The acute hospital and Inpatient units.

Your New Role:
You will join a team of qualified Social Workers and Independence Support Assistants based within the Portsmouth Integrated services. Your role will involve assessing the needs of adults and their Carers, to identify creative ways to meet their assessed outcomes under the Care Act 2014.
The role includes outcome led interventions, using a strength-based approach to support people to live the lives they chose. You will work collaboratively with others, maintaining a strength based and aspirational approach to achieve the outcomes the person wants.
Duties include assessing clients’ needs and agreeing a person-centred support plan, completing reviews of assessments and support plans, supporting people with their health, mental health, and social care needs, and assessing mental capacity when required. You will work independently, manage your own caseload, and have opportunities to support unqualified staff and pursue your own personal learning and development.

What you'll need to succeed:
You have a qualification in social work and are registered with Social Work England. You have experience of working with and safeguarding adults, and a thorough understanding of adult social care legislation. You can communicate with a diverse range of people, in sometimes challenging circumstances. You manage risk, and maintain records effectively and confidentially. You can collaborate effectively, and advocate for individuals and carers. You are professionally curious and have a positive approach to motivating others.
