This role is part of the Wiltshire Service which provides individual and family support to domestic abuse victims and their children.

The Early Interventions Team act as the gateway for all domestic abuse and referrals to the Wiltshire Service. Working within an experienced and established team, the EI Triage Advisor will:

- Triage referrals, with a particular focus on domestic abuse referrals.
- Conduct risk assessments and offer immediate safety planning advice.
- Respond to safeguarding concerns.
- Signpost victims and professionals to other support agencies where appropriate.
- Offer specialist domestic abuse advice and support to professionals.

In addition, the role requires the building of effective relationships both internally within the Wiltshire Team and externally, supporting a multi-agency approach.

Main Duties and Responsibilities:
Working within the agreed policies, principles and codes of practice of FearFree, the post holder will:

Working with vulnerable people and risk management:

• Provide a victim focused service, with an understanding of the impact of trauma on vulnerable people.
• Demonstrate specialist knowledge and understanding of domestic abuse and how this may impact on the wider family unit.
• Provide advice, guidance and support to people who are victims of domestic abuse.
• Assess, manage and review risks to service users, colleagues and self, including the use of the dash risk assessment tool and SOAG (Severity of Abuse Grid).
• Make safeguarding referrals where appropriate: including to Children’s Social Care, Adult Social Care, Mental Health crisis teams and MARAC (multi-agency risk assessment conference).
• Work directly with individuals or groups as agreed with the Team Manager.
• Safeguard the health and welfare of service users and their families.
• Respond to emergencies and crisis situations including supporting a service user to access a place of safety if appropriate.
• Manage and support service users by telephone with low to medium support needs in line with the agreed values and aims of the service.
• Act as duty worker as part of a rota system. Duty shifts take place in the Trowbridge Office on weekdays between 9 and 5pm.

Team working

• Offer professional advice and support to multi-agency partners.
• Feedback to the Team Manager, Wiltshire Service Manager and/or other Support Workers any information related to the service users support needs or risk issues, and progress on support tasks.
• Work effectively in partnership with multiple statutory and voluntary agencies to enhance service delivery, safety and safeguarding.
• Contribute to effective team working with a flexible and pro-active approach, including cover for other team members’ holiday and sickness.
• Work closely with other agencies, to network with, make referrals to and maintain good relationships with, and be able to disseminate information appropriately.
• Communicate with other staff, as well as the Team Lead and Wiltshire Service Manager, issues related to activities and work undertaken.

Administrative duties

• Help provide a responsive support service with a high quality of customer care.
• Answer the telephone and take messages and referrals, take action on messages and referrals and provide a triage service for all referrals.
• Assist in the referral process and EI follow up, contacting other agencies and helping to arrange appointments.
• Assist in the assessment and allocation of service users.
• Update written and computerised records with accurate clear information to deadline, including maintaining details of any special needs required by service users.
• Assist with monitoring and evaluation policies and procedures and producing reports.

• Ensure personal safety for self, other staff and service users at all times.

The role includes conducting assurance checks known as 1st Line of Defence (1LoD) to ensure our sites are complying with all our legal and MOD HS&EP responsibilities

This is a high-profile role and your duties will encompass:-

Leading and monitoring HS&EP working procedures to ensure compliance, and producing site HS&EP policy and environmental management system. Conducting assurance checks (1LoD), developing post-audit action plans and monitoring progress of achieving goals.
Providing HS&EP advice to staff and visitors, conducting site HS&EP briefings for new staff and contractors. Overseeing maintenance of site first aid facilities, fire equipment, and evacuation equipment. Providing guidance in relation to Disability Discrimination Act-compliant facilities, including ‘access to work’ and personal emergency evacuation plans. Promoting a HS&EP culture across our sites, and arranging HS&EP awareness campaigns, and safety days. Facilitate first aid and fire warden training.
Coordinating site Fire, Bomb and Emergency procedures including tests and exercises, and contributing to the business continuity plans. Conducting ‘lessons learned’ reviews post-exercise to improve future performance.
Inspecting site physical security provisions and processes, promoting security awareness among all site staff, and acting as the lead facilitator during security exercises.
Undertaking site-wide hazard surveys and resulting risk assessments (RA), for example Fire, Spills and environmental. Develop and implement action plans to mitigate risks. Ensuring the annual reviews are completed and RA updated.
Monitoring accident, incident and near miss data to identify trends, and report as appropriate. Collating and analysing HS&EP data, using this to produce site performance reports and statistics to show progress against targets and objectives from the over-arching action plans. Reporting findings to the Accountable Person (AP) for the sites, and leading the production of the annual site HS&EP reports.
Promoting, monitoring and measure performance against site initiatives, objectives and targets for reducing waste, recycling, energy and water saving. (Per site DBS Environmental Action Plan).
Reviewing and updating agreements between the AP and non-DBS lodgers on our sites. Monitoring and facilitating site HS&EP arrangements, providing advice and guidance


Some of the locations in our Southern region where you will provide guidance, carry out risk assessments and conduct assurance activities are remote and inaccessible by public transport means, therefore a full driving licence is essential.

This job role may be suitable for hybrid working, which is an informal, non-contractual and voluntary arrangement, blending a balance of attendance in the workplace (your permanent duty station) and working from home as a personal choice, with management approval and to meet business needs. MOD has an expectation that on average, a minimum of 60% of your working time will be spent on a MOD site. To ensure communication is clear and carried out in a way that consistently displays professionalism at all times to ensure effective working relationships with colleagues, clients/patients and other professionals/agencies. The post holder will communicate in person, via telephone calls, emails and MS Teams and ensure information is provided and received in line with GDPR and Information Governance. This will require high levels of interpersonal skills and a level of tact and empathy. This will also require advising and signposting to appropriate services. Provide a presence and support to designated wards/departments to facilitate safe and timely discharges from hospital including attendance at board rounds to partake in setting PDD and identify and resolve any barriers to achieving PDD. To report daily into the IDH management of updates on patients discharges. Attend meetings/huddles for allocation of patients to commence discharge planning. Provide Information and support to patients, families and staff in relation to complex patients discharge from initial contact to 72 hours post discharge Lead and/or participate in MDT meetings, discharge planning/professional and best interest meetings to support patient discharge Work alongside the other members of the IDH to promote the IDHs role in discharge planning. This involves educating and empowering other professionals to be involved in discharge planning. Work with the MDT to implement discharge requirements for each patient on admission to the ward and ensure early referrals. Recording all information regarding the patient in the electronic patients records. To ensure that there is discharge practitioner representation during the operational hours of the IDH To manage complex and sensitive information on a daily basis, using empathy and reassurance skills at all times with all people. Ensure that information is recorded and shared in line with information governance. To contribute to the rota management. Ensure relevant databases used in discharge planning are updated to enable accurate oversight of patients length of stay and any barriers to discharge. To contribute in any system events related to patients discharge and length of stay (e.g. escalation). Ensure that information relating to a patients discharge is accurately recorded and kept secure on the appropriate database/system and the electronic care record. Ensure that the referral to IDH is completed in full as soon as possible after admission, for patients needing or requiring support with discharge by the appropriate professional. To collaboratively work with the patient and/or representative along with MDT professionals to set goals for discharge and beyond. Ensure that at board rounds the MDT sets the PDD as per national golden standard (within 48 hours of admission) and identify and resolve any barriers to achieving PDD To lead on complex discharge planning working with the MDT to ensure discharge is safe and seamless. To resolve barriers in relation to a complex discharge and escalate further as and when necessary in order to facilitate safe and efficient discharge. To work collaboratively with and delegate or share duties as appropriate to other members of the IDH to coordinate complex discharges.

To be professional and legally accountable for all aspects of your work including the management of patients in your care. To use specialist professional and clinical knowledge across a range of procedures based on a sound knowledge of evidence-based practice and treatment options, using clinical assessment, reasoning skills and knowledge of treatment skills. To undertake assessment of patients, using clinical reasoning skills and manual assessment techniques in order to diagnose clinical need. To assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment. To provide a triage role for referrals admitted to the service, allocating assessment of patients need according to the appropriate prioritisation of response time, to reduce risk to their health and safety. Ensure the participation of patients families and carers in decision making and planning mutually agreed objectives Use a range of verbal or non-verbal communication tools to communicate effectively with patients, relatives carers and other health and social care professionals to progress rehabilitation and treatment programmes. This will include patients who may have difficulties in understanding or communicating. To manage clinical risk within the Home 1st therapy caseload. To be responsible for the safe and competent use of all appropriate equipment. Work collaboratively with the MDT with discharge planning. To comply and uphold standards of the HCPC and the Royal College of Occupational Therapists. To be responsible for maintaining own competency to practice through CPD activities and to maintain a portfolio which reflects personal development. To be responsible for attending and participating in formal supervision, including clinical supervision, in accordance with Trust policies and procedures and the standards of the RCOT. To participate in the staff appraisal scheme as an appraisee and be responsible for complying with your agreed personal development programmes to meet knowledge and competency requirements. To undertake the measurement and evaluation of your work and current practices through the use of evidence-based practice projects, audit and outcome measures in conjunction with the Home 1st Lead Occupational Therapist. To be an active member of the in-service training programme by attendance at, and participation in, in-service training programmes, tutorials and individual training sessions. To be responsible for planning own caseload, prioritising workload as situations arise. Acquire an understanding of CNWL-MK clinical and HR policy and procedures. To supervise designated team members and complete appraisals, maintaining accurate documentation.
