The Dialysis Registered Nurse (RN) is a professional nurse with specialized knowledge and demonstrates advanced skills when providing and directing care for patients with end-stage renal disease (ESRD), acute kidney injury (AKI), and late-stage chronic kidney disease (CKD). Collaborates with other disciplines/departments to achieve desired patient outcomes. Functions as primary nurse for ESRD patient panel and serves as key resource assisting patients navigate the healthcare system. Completes documentation for all clinical and technical recordkeeping in the electronic health record and treatment flowsheets in accordance with established policies and procedures. Monitors, tracks/trends, and reports ongoing care continuity needs and outcomes to the interdisciplinary team. The focus is on individual growth and development in practice with increased levels of self-direction. Seeks guidance on how to improve care provided to patients and families. Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient. The nurse brings principles and theory with limited application of critical thinking skills. This is the dependent stage of working closely with a preceptor with a goal of developing skills and judgment to utilize the nursing process in real situations. The focus is on individual growth and development in practice with increased levels of self-direction.
The primary purpose of the Peritoneal/Hemodialysis Nurse RN position is to demonstrate leadership in delivering and improving holistic care through collaboration strategies with others to provide comprehensive nursing care to patients assigned. The Peritoneal/Hemodialysis RN demonstrates clinical competence through providing direct-care and case management to people with chronic kidney disease, provision of peritoneal/hemodialysis treatments, continuous renal replacement therapy (CRRT), patient and family education, assessment, and management of peritoneal and/or hemodialysis catheter access, and provision of the holistic needs of the patient under his/her charge. Responsibilities include but are not limited to renal anatomy and physiology, technical and didactic knowledge of dialysis therapy (i.e. water testing, treatment modalities, dialysis machine operations, disinfection and routine maintenance of dialysis equipment), coordination of care with the inter professional team (physicians, nurses, pharmacists, intensive care unit (ICU) nursing staff, renal dialysis technicians, renal social worker, and other members of the healthcare delivery team), management of the peritoneal dialysis prescribed treatments (adequacy testing, patient/caregiver education, completion of screening, conducting home visits as necessary, collaboration with the surgical teams as necessary for catheter management, and evaluation and management of peritonitis or other emergent conditions), data collection and reporting, assisting with management of peritoneal supplies for both the facility and the patients, assisting with medical staff education, and providing training to other medical center staff on peritoneal/Hemodialysis. Depending on the case load, the Peritoneal/Hemodialysis RN will assisting/performing the duties necessary to manage hemodialysis patients on or off the dialysis unit to include ICU, emergency department, and any other area approved for dialysis within the facility. The Peritoneal/Hemodialysis RN shares responsibility to ensure dialysis services are available 24/7 for emergencies by participating equally with peers in rotating weekend, and on-call duty assignments. He/she provides care in a non-judgmental, non-discriminatory manner that is sensitive to the adult or elderly patient and family's diversity, preserving their autonomy, dignity, and rights.
