An exciting opportunity has arisen for a registered nurse, other healthcare professional or social worker to work across the Bracknell Community Mental Health Team (CMHT) and substance misuse service, New Hope. This is a Band 6 role.
We are looking for a registered professional who is driven, passionate and excited to be working with and the link between two essential community services to make a real difference to our service users’ lives.
The successful candidate will support a caseload of individuals with complex mental illness with substance misuse difficulties. They will also help to support colleagues in both teams to understand the complex interplay of mental health and substance misuse by carrying out assessments and contributing to care plans and Recovery Plans.
The role will offer autonomy and variety, working across two supportive teams. The role will also offer flexibility, with the successful candidate able to benefit from a hybrid approach to working, if desired.
• To support a caseload of individuals with mental illness and substance misuse difficulties.
• To work across the CMHT and New Hope (DAAT) teams, providing support via their unique dual specialism.
• To use appropriate, evidence based, therapeutic interventions to address the co-occurring mental health, alcohol and substance use needs of service users and their families.
• Assessment of risk and care needs in line with both team’s models of work and recovery. To use these assessments to then inform the care and support that is provided to the individual.

The “must haves” for you to be considered for this role:
1. A registered nurse, HCP or social worker with a recognised degree-level qualification and professional registration.
2. Previous, significant experience of working with individuals with a dual-diagnosis.
3. Dynamic and proactive, and excited to take on a new challenge.
