The Senior HUD-VASH Social Worker may be assigned administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns. Assignments include clinical settings where they have limited access to onsite supervision such as CBOCs or satellite outpatient clinics. The Local Recovery Coordinator (LRC) functions as a champion and advocate for the recovery model. LRC also serves as a recovery ombudsman to the Associate Chief of Staff of Mental Health (ACOS-MH), program managers, and staff on recovery and implementation of recovery-oriented services. In conjunction with the ACOS-MH, the LRC develops a 3-5 year plan for the facility that identifies staged implementation of recovery-oriented services (e.g., use of recovery plans). LRC is responsible for coordinating the evaluation of recovery implementation at the facility and reporting results to the National Recovery Coordinator (NRC). With guidance from the NRC, the LRC will implement measures to assess recovery policy and procedure adherence, as well as assess the extent that the recovery model is embodied in the Mental Health programs. Specialized treatment services are provided in various community settings, including the Veterans' home, residential care homes, or other supportive housing, contract nursing homes, and various other community locations. Work is done in the context of an interdisciplinary treatment team, and extensive liaison activities with other VA and community partners are required to provide continuity of care for assigned Veterans in outpatient settings

Veterans treated are within a geographical area of approximately 50-mile radius, with an approximate case load of between 10-15 Veterans. Their psychiatric and psychosocial needs are complex and require a high degree of clinical oversight and high level of problem-solving capabilities. The position is separate from the medical center, is community based and approximately 75% of treatment occurs in community or home based settings. The Senior Social Worker practices independently, using professional judgment and expertise to make decisions about services and treatment provided to Veterans and their families.

• Provide intensive case management and treatment services to assigned veterans and significant others, consistent with the goals and methods of MHICM and the VA Mental Health Initiative and with all relevant Medical Center policies and procedures. The incumbent is primarily responsible for providing assessment, discharge planning, suicide prevention, crisis intervention, and discharge planning services to Veterans and their families in the emergency department. Over days, nights and weekend hours, the incumbent will cover the Gainesville Emergency Department as well as the Lake City VAMC Emergency Department. The Lake City location is covered by virtual care technology. The incumbent is part of a team of licensed clinical social workers who cover the Emergency Department on site.
Based on staffing and coverage needs, incumbent will be cross trained to cover Acute Psychiatry and Acute Medicine/Surgery. Work entails working with patients presenting with psychosocial, mental health and medical needs. Work requires a high level of skill in assessing, developing treatment/discharge plans and use of crisis intervention skills for highly complex psychosocial, medical, and mental health needs of Veterans presenting to the Emergency Department and admitted to the acute care settings.

The social worker will exercise mature professional judgment and use a wide range of clinical skills in independently providing clinical psychosocial, case management, and mental health services to Veterans and their family members/caregivers. The social worker requires knowledge of a broad range of psychosocial, medical, and mental health issues. The incumbent must ensure appropriate protocol is followed for VA suicide risk assessment interventions. The incumbent must be familiar with the latest version of DSM and understand how to appropriately diagnosis. The social worker may provide psychosocial education, group and individual counseling,/psychotherapy appropriate to level of licensure. This work may occur in the Emergency Department or on other units, to include Acute Psychiatry and Acute Medicine/Surgery within the medical center.

The incumbent works with an interdisciplinary team. It is important the incumbent is a team player with excellent communication and documentation skills. It is important the incumbent is highly flexible as the work can vary day to day due to the nature of the Emergency Department and Acute Care Units. Office space is shared among multiple staff members. The incumbent will provide coverage as needed for co-workers, which may include providing social work coverage through virtual care options to other locations of the health system. The social worker may also participate in the student training program as approved by supervisor, as well as, serve on facility, VISN, and national committees and workgroups. Tours of Duty include evenings and weekends, but generally include 4- 10 hour days.

The Senior Social Worker conducts psychosocial assessments, makes diagnoses and provides individual and group therapies using both face-to-face and telehealth encounters. They report to the Supervisory Coordinator of the Substance Abuse Treatment Team (SATT) and provides administrative support to the SUD Clinical Program via outreach coordination responsibilities to ensure the broadest possible population coverage.

As Senior Social Worker, they have experience that demonstrates possession of advanced practice skills and training in provision of substance use disorder evaluation and treatment and evidence-based psychotherapies. To provide this care, the Senior Social Worker, will be licensed at the advanced practice level and demonstrate specialized knowledge of Medication Assisted Treatment, PTSD, homelessness, and undertreated medical conditions that impact Veterans with substance use disorders.

Additional duties include but not limited to:

• Works closely with the SATT Supervisor on programming issues and coordinates assessments, treatment, and referral services provided to Veterans Gainesville catchment area.
• Assists with the clinical supervision of unlicensed and multi-disciplinary staff, which may include Addiction Therapists, Peer Specialists, RN's and other program and support staff.
• Perform outreach, identifying and assessing Veterans' needs related to substance abuse, mental health, and medical issues. Specific tasks may include: screening of consults, biopsychosocial assessments, treatment planning/goal setting, referral(s) to service providers and other specialty treatment clinics, resource utilization and development, advocacy, crisis intervention, education and prevention, case management and care coordination, psychotherapy and supportive counseling, and administrative responsibilities to the assigned duty areas.
• Provides individual, group, and family psychotherapy and advanced level case management interventions used in the treatment of Veterans with substance use disorders; provides services on an outpatient basis but may interact and coordinate with inpatient treatment team members across a continuum of care.
• Develops and implements methods for measuring effectiveness of social work practice and services in the SATP, utilizing outcome evaluations to improve treatment services; and demonstrates the ability to design changes based on empirical findings.
• Offers consultation to colleagues and students of all disciplines and of various clinics on the psychosocial treatment of Veterans with substance use disorders, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills.
• Demonstrates the ability to teach and mentor staff and students in the special area of practice and to provide supervision for licensure or specialty certifications.
• Willingly accepts additional and special assignments that are necessary to meet program goals or customer needs.

Duties for this position include but are not limited to:

• Receives referrals from a variety of sources to include triage, consults, walk-in visits, phone calls, and warm hand-offs from internal/external stakeholders. The Senior Social Worker is required to utilize available data sources to proactively identify patients for screening/triage, assessment, treatment planning, case management and care coordination.
• Provides crisis intervention services, seeking to address the cause as well as presenting complaint, coordinates family conferences and serves as a liaison to family members/caregivers. The Senior Social Worker has working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, mental health conditions, and common medical terminology.

• Measures outcomes of interventions to improve treatment services. The Senior Social Worker designs changes based on empirical/measurement-based care and other findings.
• Utilizes clinical knowledge in the social work profession, and writes policies, standard operating procedures, and/or practice guidelines pertaining to the service delivery area.
• Proactively addresses advance directive education and facilitates completion and proper documentation and record scanning to ensure clear and accurate information regarding the Veterans' wishes are available in the electronic health record.
• Serves as a subject matter expert for clinical social work practice/processes within the Acute Medicine-Surgery units.
• Provides specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
• Provides orientation and training for new clinical social work staff.
• Provides licensure supervision for unlicensed staff to become licensed in accordance with State laws and VA policy/standard operating procedures.
• Provides mentorship and coaching for less experienced clinical staff.
• Teaches, mentors and coaches MSW students in specialty area and provides supervision for licensure and participates in the MSW student training program.
• Uses advanced practice knowledge and experience to assist with training front line staff as well as providing clinical supervision for unlicensed Social Workers.
• Required to travel on a regular basis to other locations of care within the North Florida / South Georgia Veterans Health System to carry out job duties/assignments, as well as, required to drive and/or travel using personal or government vehicle to other sites of care between the 2 main hospitals in Gainesville and Lake City or to community partners as necessary. Travel will be for long periods of time, up to three hours each way with traffic and expected at least 50% of work week.
