As an Oncology Pharmacist at our Oxford, The Manor Hospital, you'll be registered with the General Pharmaceutical Council and you'll have proven post-qualification experience. It's also important that you have excellent communication and people skills, as you'll be working closely with our patients and a variety of clinical staff.

As an Oncology Pharmacist, you will be responsible for the following:

Patient Care:

• Clinically screen all SACT prescription and process the planning and ordering of treatments for patients.

• Collaborate with healthcare professionals to optimize pharmaceutical care for oncology patients.

• Conduct medication reviews and provide personalized pharmaceutical care plans for oncology patients.

Medication Management:

• Manage and review chemotherapy and other oncology-related drug therapies.

• Ensure the safe and effective use of oncology medications, including dose adjustments and monitoring for potential adverse effects.

• Advice patients on the use of SACT medications, side effects counselling and supplementary medication.

Drug Information and Education:

• Serve as a resource for healthcare professionals, providing up-to-date information on oncology medications and treatment protocols.

• Educate patients and caregivers on the proper administration and management of oncology medications.

Quality Assurance:

• Contribute to the development and maintenance of oncology pharmacy policies, procedures and protocols.

• Monitor and evaluate the quality and safety of oncology pharmacy services.

Collaboration:

• Collaborate with multidisciplinary teams, including oncologists, nurses, and other healthcare professionals, to enhance patient outcomes.

• Participate in interdisciplinary rounds and meetings.

Patient-facing long-term condition clinics Consult with patients who have one or more medical conditions that require medication optimization e.g., COPD, asthma, hypertension. Review the ongoing necessity of each medication, assess monitoring needs, and provide support to patients in managing their medications, ensuring they achieve the best outcomes from their treatments i.e., medication optimization. Provide GPs with recommendations for medication improvements. Conduct clinical medication reviews with patients and offer recommendations to the senior clinical pharmacist, nurses, and/or GP regarding prescribing and monitoring. Respond to medicine-related inquiries from GPs, practice staff, other healthcare teams e.g., community pharmacy, and patients. Follow up with patients to monitor the effects of any changes. Review the use of medications commonly linked to unplanned hospital admissions and readmissions through audits and individual patient reviews. Implement changes to medications in response to MHRA alerts, product withdrawals, and other local and national guidelines. Monitor practice prescribing in accordance with the local formulary and recommend to GPs which medications should be prescribed by hospital doctors (red drugs) or be subject to shared care (amber drugs). Audit the practices compliance with NICE technology assessment guidance. Comply with safeguarding policies and support the commitment to promoting welfare. To actively provide locally commissioned services, both private and NHS. Ensure maximum reimbursement through accurate dispensing, purchasing and endorsing procedures, including any specific instruction from branch manager or superintendent pharmacist Identify new opportunities to grow the business and seek branch manager or superintendent pharmacist approval before proceeding. Monitor and control operating expenses in order to maximize the profitability of any of the pharmacies worked at Ensure that the range and level of stock is maintained in all pharmacies to a level that meets the needs of the business. Maintain high retail and dispensary housekeeping standards. Ensure high standards of security practice within the pharmacy premises. Attend meetings or training events called by the directors or superintendent pharmacist. Monitor local competitor activity and respond accordingly, ensuring branch manager or managing director is kept informed. Key Responsibilities

• Manage expectations and work closely with clinical pharmacists &/or physicians on alternate sources & drugs when usual sources have supply issues.
• Ensure proper and comprehensive evaluation of all drugs and gather inputs from key stakeholders (physicians, nurses and/or clinical pharmacists). Represents institution as evaluation committee member in evaluation of drugs and pharmaceutical products.
• To keep optimal inventory to achieve optimal stock availability vs overstocking and avoid wastages.
• Work closely with pharmacy inventory team on inventory management of all pharmacy sections, support stock take activities and reporting.
• Participate in key activities and related operational workgroups, quality improvement/productivity projects, management of critical supplies & stocks, attend management meetings at the supporting site.
• Timely sourcing and processing of purchase requests from users – Purchase requests must be fulfilled on time in order not to affect patient's treatment plan.
• Verify and submit requirements of national contracts taking into considerations past usage and future trends of disease treatment protocols that aligns with MOH directives and international guidelines.
• Achieve cost-savings from purchases (Group/cluster contracts and institution quotations/negotiations)
• Manage controlled drugs, distribute effectively and in accordance to requirements of Misuse of Drug Acts.
• Ensure continuity of critical supplies or alternate products during exceptional periods.
• Minimize total value of goods written-off (Pharmacy Store and all pharmacy sections/substores).
• Material /item code creation and updates. Timely and accurate maintenance of PHI pharmacy systems (SAP & dispensing systems) on cost price, selling price, drug class and drug subsidy status/type.
• Handle audit queries in all related areas, stakeholder management at supporting site, and lead and motivate the team at site.

Requirements

• Degree in Pharmacy with minimum 5 years' of relevant professional practice experience
• A registered pharmacist with the Singapore Pharmacy Council
• Strong interpersonal skills and experience in engaging stakeholders
• Excellent written and verbal communication skills, proficient in Microsoft Office applications, and knowledge in SAP-MM
• Relevant experience in Procurement and inventory management of drugs and pharmaceutical products.
• Good negotiation, interpersonal and communication skills
• Self-motivated, independent worker with team spirit and good problem solver

To attend and pro-actively take part in ward rounds (e.g. TPN) and clinical meetings as appropriate. To work with the Gastroenterology Lead Pharmacist in the provision and delivery of a TPN service. Duties you will be performing in this position include but are not limited to:

• Initiating, modifying, or discontinuing medication therapy.
• Serving as a consultant for intensive medication therapy management.
• Designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing the most effective, least toxic, and most economical medication treatments.
• Helping achieve positive patient-centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams.
• Performing physical assessments
• Ordering, assessing, and responding to laboratory and other tests to help determine the efficacy and toxicity of medication therapy.
