The Transformation Director will be instrumental in supporting the delivery of large-scale transformation, particularly with the clinical reset programme, with and through the Provider Collaborative team and broader partners across Lancashire and South Cumbria. The role demands strong leadership, exceptional programme management skills, and the ability to influence and work across various partner Trusts and system partners. The successful candidate will be responsible for:
• Leading and managing the clinical reset infrastructure, governance arrangements and administrative support, including the operation of an effective programme management function.
• Working alongside the Provider Trusts and ICB colleagues to ensure the PCB workstreams and cross-partner programmes are delivering effectively.
• Intelligence gathering, analysis and formulation of materials, including joint policy development, tactical planning and strategic development.
• Working across a wide and diverse stakeholder group to build commitment to a shared vision for the Provider Collaborative and aligned areas of focus and ensuring the contribution of the Provider Collaborative within ICB plans for Lancashire and South Cumbria.

Working with the Managing Director for PCB, the Transformation Director is responsible for leading the Provider Collaborative’s Clinical Portfolios to deliver the following:

Change Management and Delivery
• Develop and drive the delivery of key programmes of change and improvement, in partnership with clinical teams to deliver patient benefit and financial sustainability.
• Work with the System Planning Lead on oversight of the operational planning cycle across the Provider Collaborative, ensuring key linkages between changes to leadership arrangements to financial ,workforce and activity are in place; closely aligning to the NHS Long Term Plan.
• Responsible for ensuring the plans of the Clinical Portfolios align to NHSE strategic aims, and have actions in place to deliver the clinical, operational, and financial outcomes sought; and that complexity / uncertainty is understood and mitigated.
• Proactively support the working relationships between the Trusts at CEO, Executive and Professional Lead level; providing fair and transparent brokerage focused on achieving the outcomes agreed.

Strategy and Planning
• Lead the development and implementation of clinical strategic plans for the clinical portfolios, aligning national, regional and ICB strategic aims with the Provider Collaborative context facilitating engagement at all levels.
• Ensure the workplans from the Portfolios are understood by all stakeholders and are delivered utilising all available resources efficiently and effectively.

Leadership and Management

The incumbent serves as Assistant Chief for Primary Care at the Wilmington VA Medical Center and provides support to the Associate Chief of Staff for Primary Care (ACOS/PC).

• Serve in a dual capacity as Assistant Chief of Primary Care to the ACOS and may also serve as a skilled ambulatory care board certified medicine physician.

The incumbent serves as Assistant Chief for Primary Care at the Wilmington VA Medical Center and provides support to the Associate Chief of Staff for Primary Care (ACOS/PC).

Duties and Responsibilities include but are not limited to:

• Serve in a dual capacity as Assistant Chief of Primary Care to the ACOS and may also serve as a skilled ambulatory care board certified medicine physician.
• Complete Women' Health specific training in order to obtain classification as a Designated Women's Health Provider (DWHP).
• The incumbent may function as a counterpart to the ACOS and has been delegated responsibility for all medical, administrative, and management activities within the Primary Care clinical services area.
• The nature of the duties and responsibilities is such that a professional degree of Doctor of Medicine is a fundamental requirement.
• Duties require exceptional ambulatory care clinical skills; detailed knowledge of the Veterans Health Administration guidelines, policies, and procedures; detailed knowledge of the missions and capabilities of Primary Care Service; as well as, a good working knowledge of the Primary Care operations of the Wilmington VAMC.
• Must work effectively under pressure in frequent emergencies and changes by rendering assistance such as learning what the difficulty is and quickly developing factual information for the ACOS' use to guide the overall management of all Primary Care Programs
• The candidate must be willing to treat opiate use disorder with buprenorphine therapy (training available) in consultation.
• The candidate must be willing to treat Geriatric Primary Care patients and is encouraged to apply for implementation of a Geriatric Primary Care panel (encouraged but not required).
• The candidate must be willing to residency training of affiliated medical Universities and their programs.

This role is to initiate and lead service redesign and deliver changes to services which maximise patient outcomes and improves operational effectiveness. The post holder will manage the budget for their service portfolio; be responsible for the achievement of all performance/cultural targets; delivery of the service portfolios annual business plan; maintains compliance with Trust policies, procedures, and regulatory standards. As well as the preparation and presentation of reports for Trust Committee, sub committees and Trust Board which demonstrate compliance, achievement against performance targets, and effective risk management.

The Supervisory Program Specialist (Assistant Chief/Business Manager), is responsible for a wide range of administrative functions including Decision Support-Systems (RES), budget management, resource allocation, Health Information Management, Veteran benefits and eligibility for Community Care (CC), workload data management, customer service and vendor relations for claims processing and the establishment of a community care network across all VASNHCS.

Major duties include but not limited to:

• Formulates and issues policies relating to the broad area of patient administration which provides a basis for sound administrative practices, define responsibilities for carrying out specific functions, provides recognized authority consistent with applicable law under which action taken to minimize the possibility of unauthorized action or unfavorable criticism, and facilitates decisions to promote consistency of interpretation and application across organizational lines.

• Applies Veterans Health Administration laws relating to the determination of eligibility for both inpatient and outpatient related services and benefits.
• Investigates and develops facts, conclusions, and recommendations in those matters requiring investigations that come within the jurisdiction of the facility.
• Review's patient administration and patient care functions to identify areas where studies should be made and initiates management improvement surveys by staff and operation personnel.
• Provides comprehensive analysis and reports on program accomplishments to include evaluation of effectiveness of station operations and work performance to assist management in planning and decision making.
• Prepares and/or reviews and evaluates financial analysis, summaries, recurring or non-recurring reports which keep management advised of the current financial situation in the Medical Center.

The Nurse V ADPCS is of an executive nature, comprised of complex leadership and administrative components associated with critical health care issues and activities that influence the organizational mission, health care, and policy. They are accountable to the Health Care System Medical Director and are an integral part of the senior leadership team.

• The ADPCS is a member of the Executive Leadership Team (ELT) and is the top executive within nursing services.

• They ensure all registered nurses, advanced practice nurses, and licensed practical/vocational nurses are licensed, certified, and/or registered upon employment, and thereafter.
