We are looking for a driven Social Worker to join Adult Services. If you are passionate about supporting adults to live well at home, we would love to hear from you.
About You
You will have excellent communication skills and have the ability to build effective professional relationships with other disciplines and with other local health and social care services to enable a coordinated response to individuals and families.
Experience of working with adults and previous employment in a similar position would be desirable.
It is essential that you hold a current valid driving licence and have access to a vehicle for work use.

We currently have a position in our Integrated Urgent Care Team. It’s an exciting time to come and work for us as you will be joining the team at a time of transformation and great investment into services for adults and social work practice.
You will take the lead on supporting adults to live well at home. You will be part of a friendly and supportive team and your work will include managing a caseload, Care Act Assessments, Support Plans, Mental Capacity Assessments and Best Interest Decisions and signposting to internal and external services where necessary to enhance wellbeing and independence.

This post is a designated customer facing role under the fluency duty and requires a specified level of spoken English in the person specification.
All appointments will be to the bottom salary point in the grade with annual incremental progression each April until the grade maximum is reached. Where an individual can provide evidence that their existing salary in a relevant role surpasses the grade starting salary, a higher entry salary will be considered, subject to the professional grade development criteria also being met if that point is higher than the bar point.
