The Role

We are looking for enthusiastic and motivated support worker to join our Integrated Urgent Care Team within our community team. The Integrated Urgent Care Team provides short term interventions to maximise independence and responds to urgent/crises situations to enable people to continue living in their own home for as long as possible. The team responds to patients discharged from hospital wards, A and E and MAU (Medical Assessment Unit), assists in preventing inappropriate admissions to hospital, as well as supports those in crisis in the community.

The team is made up of staff from health and social care, working together to provide a holistic assessment and support plan to ensure outcomes are achieved and independence is maintained wherever possible. The team provides support to Younger and Older Adults with varying degrees of disability, social complexities and vulnerability living in the community.

About You

As a key member of this busy but supportive team you will be required to work in a practical and positive way to enable Service Users to regain confidence and to motivate them in regaining daily living skills.

This post is subject to a rota pattern working over 7 days a week and includes some weekend and bank holiday working.

It is essential that you hold a current valid driving licence and have access to a vehicle for work use for which a casual car allowance will be paid.

The young people attending the Springboard Project experience a personalised approach to their learning. They are taught by fully qualified specialist teachers, industry experts and support staff sensitive to their needs, providing a nurturing and stimulating environment. It is a unique place where pupils are nurtured and developed to encourage independence and skills for life, and their social, emotional and mental health needs are at the forefront of our work.

Due to the complex nature of our students, high levels of emotional and physical resilience are needed in dealing with challenging behaviour. However, the knowledge that our work is making a positive impact in helping the young people to grow and work towards independence brings great reward.

We are now looking to recruit an experienced and highly motivated Admissions and Education, Health & Care Plan Co-ordinator to support the admissions and annual review process across the school.

The primary purpose of the role is to ensure that all processes relating to individual Education, Health and Care Plans are carried out in accordance with regulatory requirements. This will involve facilitating the annual review process for individual students. The role also involves active participation in the admissions process; liaising with professionals in order to share information or to gather further evidence for the Admissions Panel and working flexibly with other schools to promote a speedy and smooth admission process for the students into the school. Applicants should refer to the Job description and Person Specification as a more detailed guide to the responsibilities attached to the post.

Previous experience in this area of work is preferable, although full training will be given. Key requirements for any candidate are:

• Communication skills to influence, persuade, motivate and engage with a wide range of children, young people and their families including presenting to a wide range of audiences.
• Listening skills to support children, young people and their families through understanding their point of view in a non-judgemental approach.
• Experience of using computer packages for word processing, spreadsheets, databases, e-mails and researching information.

The Social Care Assessor will be responsible for undertaking holistic Care Act 2014 assessments both within the community and hospital/intermediate care facilities.

The role holder will undertake home visits and carry out holistic strengths-based observations and assessments of citizens to support a determination of social care needs.

Responsibilities include arranging timely support for reablement, packages of care, respite, carer assessments, equipment and minor adaptations to support independent living, assistive technology and support carers and family members.

Advising citizens and referring them to appropriate services based on the observation and assessment completed, ensuring that the provision of services supports citizens maximise their independence.

Developing and implementing citizen support plans to meet identified social needs of individuals, enabling the citizen to achieve maximum health and well-being.

Establishing and maintaining relationships with citizens and working proactively with appropriate stakeholders to assess the individual’s needs and support positive outcomes for citizens.

Complete strengths based holistic assessments and observations, identifying and arranging appropriate Adults services to meet the needs of the citizen.

Providing and facilitating the provision and fitting of equipment, adaptations and/or intervention from other agencies or departments.

Identifying safeguarding and mental capacity issues and referrals as appropriate; assisting in investigations of allegations of any form of abuse to vulnerable adults

Actively participating in new initiatives and future changes intended to improve service delivery.

The Role

We are looking to recruit enthusiastic and supportive Community Care officers within our Neighbourhood teams. This is a great opportunity to join Tameside MBC whilst we are undergoing a vast period of change.

Role Overview:

The Role
To do everything possible to ensure the team provides an efficient and effective service to the residents of Bolton. This involves detailed assessment of need (Care Act) and effective asset-based support planning, taking into account available resources, universal and voluntary services.
