The RN Care Coordinator provides care coordination for veterans who are assigned to a Geriatric Patient Aligned Care Team (PACT). Coordination of care and services for veterans that includes but is not limited to veterans with chronic disease, veterans with limited functional status, complex medical, psycho-social and/or mental health needs, severely-injured service members and veterans, and veterans with intensive continuing care needs that will require coordination of and referrals to various levels of care. Policy Program Development and Management:

• Directs and assists staff in administrative research, management analyses, and development of reports.
• Collects and analyzes data on acquisitions, patient population, economic demography, patient care, work performance, etc.
• Conducts studies on manpower, space, and equipment utilization.
• Adjusts allocations as needed and prepares related reports.
• Develops and maintains awareness of available resources and makes recommendations regarding needs, distribution, or re-distribution of resources.
• Prepares and updates performance standards.
• Participates in quality assurance activities.
• Plans and implements construction plans.
• Serves as a team lead planning and assigning work, preparing schedules, providing training, etc.
• Serves as budget and financial advisor by developing and maintaining budgets and ensuring funds are available.
• Performs other related duties as assigned.

 Work Schedule: ???????Monday-Saturday; ROTATING schedule; 7:30am-4:00pm and/or 10am-6:30pm. Flexibility to work both tours as needed by Service is required. Recruitment & Relocation Incentives: Not authorized Critical Skills Incentive (CSI): Not approved
