The PACT Physician (MD/DO) position is responsible for direct supervision of outpatient care at assigned location. The PACT Physician (MD/DO) Primary Care position reports to the regional section chief who oversees his/her assigned location.

Duties:
- Functions as the Physician Team leader of their Patient Aligned Care Team (PACT), Ambulatory Care Services, Community Based Outpatient Clinic (CBOC) setting at (MTVAHCS). May perform additional duties (temporary or permanent) as a float provider (face-to-face or via telehealth) as those duties are assigned.
- Responsible for providing medical care in an outpatient setting following PACT principles. Will routinely provide medical care using Face-to-Face (F2F), VVC, and/or Telephone appointments. When performing duties as a float provider (F2F or telehealth) for remote patients, will provide care to outreach clinics as needed. Float provider duties (temporary or permanent) may require travel to the supported CBOC(s) periodically to cover a provider vacancy.
- Assures that Veterans receive compassionate, efficient and safe patient care in their respective care line.
- Assures that clinical care provided meets all timelines and required documentation as required by VA. Providers must review alerts daily and have no more than 200 alerts at any given time. They must encounter their notes within 72 hours. Encounter errors must be completed within 7 days.

Major duties include, but are not limited to the following:

• Psychosocial Assessment - The social worker the develop an assessment of the veteran, family, economic, community, and psychosocial support systems when clinically indicated. The assessments must reflect a high level of skill in gathering, analyzing, interpreting, and documenting data that is pertinent and directly related to patient health care planning, treatment, education, and case management. The veteran, family members, significant others and the interdisciplinary treatment team are involved in this process, whenever possible. The goal of the communication is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status and safely maintain the veteran in his/her home. The social worker assists veterans and families with assessing for supportive services such as Home maker Home Health Aide services, nursing home or assisted living placement and other community resources. The social worker will also complete follow up assessments as needed to review veteran needs.
• Treatment planning/Goal Setting - Incumbent demonstrates basic knowledge of psychosocial treatment modalities and demonstrates the ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. The Advanced Practice Nurse (APN) III Certified Nurse Practitioner (NP) practices in Surgery Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management.
The NP practices to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position. The NP has prescriptive authority for a panel of patients or program as indicated by their position.

ASSIGNED DUTIES:
The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients.

Duties unique to Wound Care include but are not limited to:
-Possess Wound Care Certification
-Performing thorough evaluations of wounds related to diabetes, vascular disease and poor circulation including size, depth, location, tissue viability, and presence of infection.
-Creating individualized care plans based on the wound assessment, including appropriate dressings, medications, and therapies.
-Performing procedures like debridement, wound irrigation, etc.
-Providing detailed instructions to patients and caregivers on wound care techniques, hygiene practices, and preventative measures to promote healing.
-Consulting with physicians, other nurses, and specialists to coordinate comprehensive patient care.
-Regularly assessing wound healing and making necessary adjustments to the treatment plan as needed.
-Identifying patients at high risk for developing pressure ulcers and implementing preventative strategies.
-Performs History and physical for pre-op patients.

Major duties and responsibilities of the position:
-Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
-Develops and implements an individualized plan to achieve optimal population health and disease management.
Demonstrates leadership and works collaboratively with the interdisciplinary team.
-Orders and interprets labs, tests, and diagnostic studies.
-Prescribes pharmacologic and non-pharmacologic interventions.
-Initiates referrals and consults to specialty services.
-Provides education to patients and their families.
-Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.

Duties include but not limited to:

Performing the following tasks during the clinic visit:- Reviews information from outside providers- Conducts history and physical exam- Assesses patient's clinical condition- Formulates treatment plans- Completes provider-specific clinical reminders- Reviews test and procedure results- Completes patient referrals in CPRS- Manages patient's medication (including medicine reconciliation)- Gives medical and lifestyle advice and counseling

Performing the following direct patient care responsibilities when patient is not present:- Completes view alerts- Follows-up on patient-related information (test and procedure results, consults and medications)- Calls patients and family on the telephone- Sends and receives secure messages from patients- Discusses cases with consultants and with PACT team- Reviews and acts on non-VA medical records- Completes forms/paperwork- The provider participates in all PACT team meetings and other team activities- Collaborates with NP and PA providers and directs care for the panel.


Work Schedule: Monday - Friday 8:00 AM - 4:30 PM (Will take after hours critical lab call on a rotating basis (approx. 4 weeks yearly))
