We are looking for an enthusiastic discharge care coordinator who will support and coordinate discharge in different specialties and locations to improve patient care and the patient experience within the Trust. The post will play a pivotal part in ensuring there is a smooth transfer of care from UCLH to the community services. This will also offer an exciting opportunity for someone with previous healthcare experience to build upon their knowledge and experience in this innovative role.

You will work under the supervision of the registered nursing team and/or clinical leads, but will work independently in facilitating a smooth and timely discharge process for patients. This will include supporting the patient discharges from wards to create new capacity but ensuring that the continuity of care is maintained. The role also involves undertaking both routine and specially identified tasks for which the post holder has been trained and assessed as competent.

To be successful in the role, the applicants must have a keen interest in improving the patient experience of discharge planning and patient flow in the wards, and care about the continuity of the service between the acute and community setting.

An exciting opportunity is open for a motivated and enthusiastic Occupational Therapist to join our team as the Band 7 Team Lead Occupational Therapist for our Medicine team. This is a full-time, permanent role.

The team are responsible for the assessment and treatment of patients admitted under Care of the Elderly, Internal Medicine, Infectious Disease and Respiratory Medicine. The post holder will carry a varied caseload of patients whilst also supporting and developing other members of the therapy team. We are looking for an enthusiastic, compassionate, and highly motivated Band 6 Senior Staff Nurse to join T13 Oncology at University College London Hospitals. At UCLH we provide highly specialised cancer care for oncology patients with a wide range of solid tumours, we are one of two cancer centres in London offering specialised care to sarcoma patients. T13 Oncology is an acute 44-bed ward. As a Band 6 Senior Staff Nurse, you would be working alongside a supportive and committed multi-disciplinary team.

Excellent communication skills, a friendly approachable nature and an ability to manage your own workload are required. Previous Oncology experience including chemotherapy is essential, additional training and support will also be provided.
