• The post holder will provide a specialist consultation liaison service to referring clinicians and clinical teams, assisting them in decision making in difficult and complex cases in order to help reduce the number of patients awaiting mental health assessment and to provide short term input including assessing and treating inpatients working intensively to support early discharge.
• The post holder will promote choice and hopefulness and ensure a recovery orientated approach.
• The post holder will promote and initiate any actions to safeguard vulnerable adults.
• The post holder will be responsible for the full assessment of care needs of patients presenting with a wide variety of clinical conditions without direct supervision. Following this, the post-holder will be responsible for planning and referring on for appropriate follow up where necessary.
• The post holder will provide a comprehensive consultation liaison service to the ED and individual inpatient areas/clinicians, particularly in assisting them develop emergency/longer term plans of care and make difficult decisions in cases where patients are exhibiting complex psychosocial needs and/or challenging behaviours.

The role involves working autonomously carrying out mental health assessments and treatments while working closely with the team members. You will be expected to work as a mental health Nurse specialist within the hospital. The post involves fostering staff relationships and supporting and working closely with our physical health colleagues.

You must be comfortable working flexibly, both autonomously and in a team. You will also be expected to provide regular teaching to hospital staff. Continuous Professional Development and regular professional supervision are a priority for the Trust and you will receive regular supervision working within this role.

KEY RESULT AREAS:

Clinical:

The post-holder will:
1. Under supervision, be required to make clear decisions, based on an analysis of complex presenting problems and judgement about available options. This will include those related to the admission and discharge of patients.
2. Utilise a wide variety of clinical skills appropriate to the needs of individual patients and the clinical setting in which they are required to be seen.
3. Under supervision, carry out robust assessments (when working with out of hours service providers such as CHTT and/or on call SpR and AMPH’s can be utilised) ensuring discharge from the ED is clinically sound and timely. Have an ability to engage challenging and /or acutely distressed patients in potentially hostile and/or antagonistic situations.
4. Provide a broad range of specialist psychosocial treatments for individual patients and a specialist consultation-liaison service to referring clinicians and clinical teams, assisting them in decision making in difficult and complex cases.
5. Demonstrate a high standard of record keeping and documentation with adherence to trust policy and an understanding of professional guidelines and application of confidentiality.
6. Work with colleagues to develop and promote the service’s philosophy, framework of care delivery and strategic objectives.
7. Work with colleagues to develop clinical expertise and high standards of nursing practice, as defined by the Nursing and Midwifery code of professional conduct (2002).
8. Deliver care that is evidence based, reflecting current best practice and research.
9. Participate in structured clinical supervision in accordance with Trust policies and procedures.

The post-holder will:
1. Demonstrate the ability to receive and give complex clinical details clearly, with patients, carers and colleagues.
2. Demonstrate the ability to communicate clearly and sensitively in situations that are contentious and/or hostile.
3. Demonstrate a high level of interpersonal skills, self awareness and empathy in all communications.
4. Demonstrate an ability to negotiate and problem solve in contentious and/or antagonistic situations where agreement is required or communicate clear decisions and rationale when agreement cannot be reached e.g. in situations where the Mental Health Act or Mental Capacity Act may be needed to be used.

The post-holder will:

The Trust is seeking a full time consultant psychiatrist for St Helier Hospital Liaison Psychiatry. This is an established post. The vacancy has arisen as a result of the retirement of the current consultant.

This post is for 10 Programmed Activities (PAs) per week.

This is an exciting opportunity for an energetic and dynamic consultant psychiatrist to join this effective, friendly and rewarding service.

The Consultant Psychiatrist will work with the other Liaison Consultant, Dr Sentil Soubramanian, to provide senior cover to the hospital wards and the Accident and Emergency Department, whilst working within and jointly clinically supervising the Liaison Team. During daytime hours, the team sees patients above 18 years in all wards and departments, including the Emergency Department and Clinical Decisions Unit.

The St Helier Hospital Liaison Psychiatry service has been well established over a number of years, having successfully expanded since 2015 to a 24/7 service, recruiting a number of substantive new clinicians over the last two years, including a second consultant and a pharmacist. The Older Adult Psychiatry specialists are integrated with the clinicians providing services to working age adults. This model ensures that colleagues in the general hospital have a single point of referral, and that expertise in old age psychiatry and general adult psychiatry are shared within the team. All liaison staff are expected to work with patients across the whole age range.

St Helier Hospital has approximately 530 beds and is situated close to the Merton and Sutton borough boundary, in Greater London. Approximately 1/3 of the acute admissions are Merton residents, and 2/3 are Sutton residents.
