Work according to the Nursing and Midwifery Council (NMC) Code of Professional Conduct and relevant professional guidelines as a named nurse or key worker taking responsibility for the assessment of care and health education needs. To ensure the delivery of high-quality specialist midwifery care and maintenance of standards in accordance with the UK Law, Nursing and Midwifery Council and Trust and EU Guidelines. To ensure robust communication pathways between the maternity service and other services involved in the care of women with mental health problems, including the local primary therapy service (IAPT) and perinatal mental health team. To be competent and confident in all areas of midwifery practice and act as an expert specialist resource in the provision of care for birth related trauma; offering highly specialist advise to other midwives and junior medical staff in birth related trauma matters. Provide midwifery informed specialist assessment and implementation of short-term evidence-based interventions for women experiencing birth related trauma. This may include facilitated signposting to other services/ support pathways? In conjunction with existing midwifes colleagues to contribute to the specialist mental health clinic. Develop, implement and evaluate multi-agency and individualised specialist mental health birth plans in conjunction with women and their significant other. Demonstrates understanding of ethics, UK law, Midwifery Rules and Standards, Nursing and Midwifery Council guidelines and those relating to childbirth, especially pertaining to advocacy, consent and sexual health. When appropriate to attend highly complex safeguarding case conference/core groups, prepare specialist reports as required for complex child protection cases. Ensure that everyone involved in the care plan receives a copy and to ensure that all relevant details are recorded on Trust IT systems. Responsible for undertaking specialist mental health screens and offering psychological interventions/approaches. Liaise with and offer specialist trauma informed assessment and advice to other agencies and professions, including when necessary participating in Multi-Agency meetings to offer specialist advice and opinions to facilitate the ongoing treatment and care. Manages effectively own time and that of junior staff and students.

Key responsibilities Provide clinical support to the surgeries within the Bridgwater Bay Primary Care Network as an independent pharmacist prescriber, consulting with patients by telephone and video consultation, Provide clinical support to patients in care and nursing homes. Carry out structured medication reviews for all patient cohorts. Support the PCN practices to improve medicines reconciliation on discharge, formulary compliance, repeat prescribing management and medication reviews. Support Bridgwater Bay PCN and Somerset ICB plans to reduce medicines waste in primary care. Interpret and provide complex medicines information to patients, carers and healthcare professionals. To provide information, advice and guidance on the safe, rational, effective and economical use of medicines. Undertake clinical audit and other outcome measures to support patient care and safety. Create and implement medicine related guidelines, policies and SOPs as appropriate to the different service areas. Provide and receive complex, sensitive information and communication relating to pharmaceutical care of patients. Provide prescribing service within sphere of competency to patients in primary care, as a pharmacist independent prescriber. To develop, advise, agree and implement action plans at a practice level, monitor the effect of changes made and ensure that any benefits are maintained in the longer term. This will include: Reviewing prescribing data for the practices to highlight areas where there is scope for improving the clinical effectiveness of prescribing. Obtaining agreement with the practices to implement medicines optimisation projects with the overall aim of following national prescribing guidance and cost effective prescribing. Advising GPs and other health professionals on cost effective prescribing and aiding rationalisation of drug choice via formulary/guideline/protocol development. Record clinical activity, interventions, KPIs and to monitor the outcome of interventions. Provide training on medicines on a regular basis to GP surgery staff, patients and carers in all relevant aspects of medicine use. Support the pharmacy technicians working within the PCN. To maintain knowledge of developments in pharmacy practice on a regular basis in line with General Pharmaceutical Council (GPhC) CPD requirements To fulfil the professional requirements of the Code of Ethics and Standards of Good professional Practice of the GPhC The post holder will be responsible to the SHS Lead Pharmacist and Primary Care Network Clinical Director but will be expected to use their own initiative to make complex decisions within defined parameters with little supervision.
