Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way. Provide general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone. Job Responsibilities: Ensure an effective and efficient reception service is provided to patients and any other visitors to the Practice Deal with all general enquiries, explain procedures and make new and follow-up appointments. Check telephone numbers, addresses, email addresses with patients Using your own judgment and communication skills ensure that patients with no prior appointment but who need urgent consultation are seen in a logical and non-disruptive manner. Explain Practice arrangements and formal requirements to new patients and those seeking temporary cover and ensure procedures are completed. Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery. Enter requests for home visits and telephone consultations onto the telephone consultation screen, ensuring careful recording of all relevant details and where necessary refer to Duty Doctor. Action repeats prescription requests following the practice guidelines and ensure that they are ready for collection by the patient within 48 hours. Advise patients of relevant charges for private (non General Medical Services) services, accept payment and issue receipts for same. Patient notes and correspondence: o Ensure that scanning and read-coding is up to date on a daily basis. Scanning must be double checked before being read-coded. Once letters are summarised on patients computerised medical records, they must be cross shredded. o Ensure that all recent correspondence is available when patients are seen. o Enter patient information onto their electronic records as required. Update telephone numbers and email addresses; liaise with secretaries for update of addresses. Update medical records as required. Premises: o Open up premises at the start of the day. De-activate alarm and make all necessary preparations to receive patients. o When last to leave at the end of the day, ensure that the building is totally secured, radiators, fans and internal lights are turned off and the alarm activated. Undertake any other additional duties appropriate to the post as requested by the Partners or the Practice Manager. Clinical Practice Assess, diagnose, triage, and manage patients, taking responsibility for the management of a complex caseload. You will progress and request investigations when trained to do so (such as x-rays and blood tests) and referrals to facilitate diagnosis and choice of treatment. Develop and make use of your full scope of clinical practice, including skills relating to independent prescribing, injection therapy and investigation when trained to do so. Manage complex interactions, including working with patients with psychosocial and mental health needs, referring to social prescribing when appropriate. Work as part of an MDT in a patient-facing role, using your expert knowledge of MSK issues, to create stronger links for wider MSK services through clinical leadership, teaching and evaluation skills. Support research and audit programmes to evaluate and improve the effectiveness of our FCP service. Streamline the pathway of care by providing a responsive service so that patients receive timely access to care. Leadership Lead, manage and contribute to service delivery. Implement all aspects of effective clinical governance for own practice, including undertaking regular audit and evaluation, supervision and training. Communicate effectively and appropriately, with patients and carers, complex and sensitive information regarding diagnosis, pathology, prognosis and treatment choices supporting personalised care. Facilitating learning Provide feedback and support to other PCN clinical team members on appropriate referral criteria. Critically reflect on your own practice, and continue to develop your skills.

You will be expected to: Work as part of multidisciplinary teams providing care to children and young people with mental health needs, across Southend-on-Sea. Provide senior medical support and guidance to the team Work directly with young people and their families and liaise as appropriate with other external CAMHS mental health providers and acute hospitals Participate in the on-call rota frequency is currently 1:7, with a 2% of basic salary, Category B on-call supplement. Support your peers and team leader and be an effective team member whilst allowing them to work with their own initiative. Work alongside 0.8wte Specialty Doctor. Provide senior medical support to the team, and direct input on clinical cases. Provide guidance and supervision to the team. Work directly with children and young people and their families and liaise as appropriate with other external CAMHS mental health providers, and acute hospitals. Work alongside the CAMHS Consultant Psychiatrist within the service and share clinical supervision requirements for the team. Develop links already established with the local acute trusts A&E departments and inpatient wards. Be responsible for patients allocated to them and will assess, manage, plan, and deliver care. Support your peers and the team manager and be an effective team member whilst working on their own initiative. Supervise junior colleagues and liaise with other health and social professionals and agencies.

Job summary: To support the running of the Primary Care Network clinic to project a positive and friendly image to our patients and other professionals who may contact us either in person or via the telephone. To act as first point of contact for anyone accessing the PCN premises. Job responsibilities: General Duties To ensure all visitors and telephone callers to the practice are greeted professionally. To ensure that all telephone calls are managed appropriately and in a timely manner. To manage the flow of patients arriving at the reception desk, for appointments, prescriptions, or queries. Take general enquiries relating to all PCN services. Liaise with patients to book/cancel appointments using the clinical system SystmOne. Complete appointment template at time of booking.
