Main Duties Networking- Develop the local Health Champions network, support them in achieving the PCNs reducing health inequalities aims and objectives. Inequalities in health Understand local health inequalities and help to develop policies and strategies for addressing these. Effective communication- Support health messages and encourage local Health Champions to reach their community to make positive changes to their health & wellbeing. Organise Health Champion training, meetings and health promotion events in the community. Specific Duties The duties and responsibilities may include any or all of the items in the following list. 1. Utilise population health intelligence to proactively identify areas of inequality and work with a cohort of patients with long term health conditions to deliver personalised care. 8. Support patients to utilise decision aids in preparation for a shared decision making conversation. 9. Support people to take up training and employment, and to access appropriate benefits where eligible. 10. Support people to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing. 11. Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level. 12. To be aware of the PCN philosophy and to treat people with dignity and respect, offer constructive support to colleagues and the general public. 13. To be responsible with other members of the team for maintaining and monitoring the PNC & practice systems 14. Primary Duties & Areas of Responsibility Assess, plan, develop, implement and evaluate treatment programmes for patients that promote health and well-being. Help the practice develop and deliver specialist primary nurse led services, including chronic disease management. Manages programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions. Maintains disease registers in liaison with administrative staff. Undertakes annual reviews of patients understanding and ability to self manage. Make professionally autonomous decisions for which he/she is accountable and provide safe, evidence based, cost effective, individualised patient care. Where necessary evaluate patient test results in conjunction with the patient and their GP and ensure the appropriate course of treatment is initiated. Make appropriate referrals to other members of the primary, community and secondary care teams. Undertake regular medication reviews for patients for groups of patients deemed suitable by the clinical team. Administer child and adult immunisations and vaccinations in accordance with national and local programmes. Competent in anaphylaxis and resuscitation techniques. Ensuring safe storage, rotation and disposals of vaccines and drugs and that adequate stock levels are maintained Undertake procedures including: venepuncture, BP management, urinalysis, peak flow, spirometry, stitch/clip removal, cytology and wound management Promote health and well being, giving general education advice on diet, contraception, smoking chronic disease management, exercise etc... Initiate and carry out programmes of health screening as agreed with the practice clinical team. Enables, supports and encourage individuals, families and groups to address issues which affect their health and social well being. Ensure all data protection requirements are met when gathering, recording and storing patient data. Liaise and maintain good working relationships with all members of the practice team.
