Our aim is to work together in partnership to join up health and care services, improving our populations health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a one workforce cross system approach to the onboarding, induction, education and experience of Healthcare Assistants. To support this one workforce model the successful candidate for this post will be required to attend our5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process. Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience. Further information will be provided post interview. This will be followed by an additional 5 day (Monday to Friday) EPUT corporate / mandatory training welcome. (EPUT Welcome will be varied between face to face and virtual delivery).

Job Summary As a Medical Records Administrator, you will ensure efficient patient service and support the smooth running of the department by handling patient and third-party medical record requests (SARs, GPR). You will be the primary point of contact for record access, ensuring confidentiality and administrative accuracy. The role requires proficiency in Microsoft Office 365 and the ability to work calmly in a high-volume environment are essential. The post holder will be skilled in: Data Protection/GDPR Patient Access Medical Record Viewing methods IGPR web software Digitising information Information sharing Job Responsibilities Recognise the type of request when received, i.e. Subject Access Requests (SARs), Third Party or GPR and process accordingly. Be the practices point of contact for staff and patients with regard to requests for access to their medical records, either directly or via a third party. Process all requests and deal with accordingly. Ensure that patients and other third party requestors expectations are set and not unrealistic. Be responsible for ensuring all requests for access to health records are dealt with in an efficient and timely manner within the set timescales. Process the requests for GPRs Convert to digital reporting where possible (IGPR) Redact records where applicable Liaise with the GPs for approval Raise invoices and ensure all payments are received. Be responsible for accurately recording all requests on to the Medical Records Requests spreadsheet. As well as recording on EMIS and ensuring that there is an audit trail of all contacts with patients and third parties. Process all associated documentation regarding appropriateness of requests, consents and identification checks ensuring that processes are consistent, transparent and legally compliant. Ensure all telephone enquiries are dealt with in an efficient and confidential manner. Ensure all email enquiries are dealt with in an efficient and confidential manner. Ensure all task enquiries are dealt with in an efficient and confidential manner. Become proficient in the use of digitised redacting and adding online viewing restrictions in EMIS. (Particularly with the introduction of Prospective Records Access). Liaise with scanning colleagues with regards to restricting certain documents from online viewing. Ensure that all safeguarding information requests are sent to the safeguarding team (CaST) for completion or approval. Keeping well-informed of the Information Sharing and Data Protection laws and ensuring all disclosures are legal and seeking advice from your Team Lead when required. Maintain daily contact with colleagues working within the team to ensure priorities are met and targets achieved. To continually look and identify ways to streamline processes to aid consistency and paper-light practices wherever possible. Introduce and maintain the following to streamline the process, remove the risk of accidental disclosure and remove admin burden from the GPs: SARs redaction by iGPR or Adobe PDF. GPRs to be drafted by Medical Records Team and forwarded to GPs for completion and sign-off. Job purpose The practice is a paper-light environment, and the main responsibilities of the post-holder will be to process the incoming patient clinical correspondence both by paper and electronic transfer; working in accordance with the practices protocols. As part of the workflow administration team in the practice, your role of workflow administrator will be to process clinical correspondence safely, ensuring that it is accurately coded. You will ensure you pass appropriate issues to GPs and other members of the practice clinical team in line with agreed clinical protocols. To include: Workflow of practice correspondence (both paper and electronic) and attaching to patient records. Recording diagnoses, procedures and investigations. Identifying urgency and carrying out actions or forwarding to appropriate teams. Identifying complex or higher risk areas that require input or knowledge from the clinical team. Working to agreed practice protocols. Answering queries from other members of staff. Respond to online access requests and queries Complete other admin tasks as required Principle responsibilities Sorting and date stamping all incoming patient related mail (if required) Reading and identifying documents that can be: - Actioned by a workflow administrator following the correspondence management protocol. - Or workflowed to the appropriate team or clinician.Coding patient records accurately following practice protocol. Ensuring incoming letters are viewable on the patient record within a 48/72 hour timescale. Adding the documents in accordance with the protocol to the patients notes with the usual clinical attributes and actioning as above. Responding to online access queries to include setting up accounts, updating access, and troubleshooting on patients behalf Other duties and responsibilities Take action as requested by other team members and members of staff. Carry out administrative tasks relating to patient records, including filing and document management. Although this is not a patient-facing role, there will be occasions where the post holder is required to deal with a patient query and speak to them directly.
