Medicines Information Provide evaluated medicines information and advice from a variety of sources including clinical trials and published literature to pharmacy, medical and nursing staff, allied healthcare professionals and members of the public (via the medicines helpline) at all levels to promote the safe, effective, evidence-based and economic use of medicines thus optimising individual patient therapy. Job Summary The post holder will: Be responsible for supporting the Chief Pharmacist and Associate Chief Pharmacists with delivering safe, high quality, person- centred medicines optimisation work programs and activities. Be responsible for supporting the Chief Pharmacist and Associate Chief Pharmacists with strategic medicines optimisation (MO) and clinical professional advice across NHS Nottingham and Nottinghamshire ICB and Place Based Partnerships ( PBPs ) and have delegated responsibility for implementation in lead areas. Be integral to the transformation of safe and effective medicines functions and services linked to the development of the Integrated Care System (ICS). In this responsibility, the post holder will work in conjunction with the Associate Chief Pharmacists, Chief Pharmacist and Senior Medicines Optimisation team in providing strategic oversight to raise standards for patients by improving the safety, quality and cost effectiveness of prescribing in primary care, and management of clinical governance and risk in relation to medicines. Be responsible for the direct management of the Medicines Optimisation teams and the co-ordination, development, and implementation of medicines optimisation strategies across the PBPs and support of implementation of such strategies within Primary Care Networks (PCNs). Be responsible for provision of expert advice to NHS Nottingham and Nottinghamshire ICB, PBPs, senior managers, Primary Care Networks (PCNs), prescribers, members of the public and other key stakeholders on medication related issues. To take ownership of receiving and recording incidents and service requests, presented through a variety of mediums (face to face, phone, self-service, email) for a wide range of customers who use Nottingham University Hospital systems. To provide first line support via analysing and diagnosing as they are reported to the Service Desk, ensuring all relevant information is captured and recorded whilst resolving the majority of incidents at first point of contact using remote support tools where applicable to achieve this. Any recurring problems should be flagged with senior management and the necessary actions completed on the call logging system. Job Purpose The post holder will manage an Urgent Care Caseload in a clinician-led environment, dealing with a wide range of presenting undifferentiated conditions.
