Utilise highly developed communication skills that are consistent with peoples level of understanding, culture, background and preferred ways of communicating to effectively impart information to patients and their carers/ families and in situations where there may be barriers to understanding and / or acceptance. Communicate with patients, formal and informal care providers and partner professionals and agencies in circumstances that may be highly sensitive and contentious including the safeguarding of vulnerable adults, impaired mental capacity and degenerative long term conditions. Demonstrate sensitivity and empathy when communicating with patients and their relatives/ carers regarding their highly complex and changing needs. Remain professional and calm and act as a role model in situations of conflict, seeking support from others in keeping with organisational systems and policies. KEY PERFORMANCE AREAS Develop a detailed understanding of East Lancashire Hospice Community Model, knowing the different options and levels of provision available to provide supportive palliative care in our community including that delivered by other organisations in order to maximise available support for patients and their families Utilise outstanding organisational skills to develop, implement and maintain robust systems and processes necessary for the effective coordination of supportive palliative care. Respond creatively and pro-actively to requests for supportive palliative care following assessment by qualified members of the clinical team, recognising the individuals preferences and choices, including their preferred place of care Demonstrate excellent communication skills to engage with patients and their families, both face to face and by telephone, to plan a suitable package of supportive palliative care in line with the initial clinical assessment and the persons changing needs and circumstances Contribute to patient review meetings feeding back on the supportive palliative care provided and its reported impact Maintain the provision of supportive palliative care to a caseload of patients and those important to them, liaising with named work to coordinate support visits, conduct scheduled reviews and provide crisis intervention Produce rosters for supportive care staff and volunteers and organise allocation of support visits taking into account the needs of patients and their families, location, continuity of care and skill mix Establish and maintain effective communication with hospice supportive care staff and volunteers to highlight any changes in needs, circumstances or risks and adjust the provision of support accordingly Provide timely feedback on any changes noted or communicated in a persons condition or circumstances taking account of the potential need for adjustments in service provision Utilise electronic clinical records systems including EMIS and PAS, to record support provided, progress and produce reports on contacts, impact and outcomes to demonstrate the effectiveness of supportive palliative care provision Conduct dynamic environmental risk assessment where there is an urgent need for unplanned responsive intervention for example to support early discharge Answer calls via the ELH response line, recognising when escalation of the call to a qualified member of the team is required Contribute to the recruitment, induction, training and support of volunteers providing supportive palliative care in the community Contribute to audit and monitoring of the provision of supportive care obtaining feedback from patients and their families, staff and volunteers identifying good practice, areas for improvement and potential solutions to problems in order to improve service provision Promote the work of the community team and wider hospice services throughout our community

Set up/upgrade of user accounts after training delivery, conforming to correct levels of role based access. Undergo annual appraisal and be responsible for evaluating the currency and sufficiency of own knowledge and practice against KSF outlines. The post holder is an experienced specialist Health Care Professions Council Registered Biomedical Scientist who provides a continuous and comprehensive blood science service as an aid to effective diagnosis and treatment. The post entails confirmation of the analytical and technical validity of investigations, liaising with clinical and blood science staff to ensure diagnostically abnormal results are urgently acted upon.Where undertaken, staff committing to working unsocial hours will have significantly different working patterns and responsibilities to routine core hours. This is because these staff operate and communicate as a lone front line self-governing service provider and assume first line responsibility. Detailed job description and main responsibilities This post will be based within the Acute Conditions Research Team, supporting research studies within various disciplines including but not exclusively, Anaesthetics, ICU, Surgery, ED/Trauma, MSK and Stroke.You will be working as part of our small, friendly team, responsible for identifying, recruiting and supporting patients throughout their research journey. The post is primarily based at Blackburn Royal Teaching Hospital. However, the post holder may also be expected to support trials at Burnley General Teaching Hospital and other Trust sites, to meet the needs of our patients and studies. Therefore flexibility is an essential attribute. Most of the research activity is undertaken Monday to Friday within normal office hours. However, research activity times may also vary and early starts (7:30 onwards) or late finishes (after 5pm) may be required to accommodate studies involving morning theatre lists & late running clinics. Transcription and production, from digital dictation and audio-tape, of typewritten reports/correspondence, using word processing packages, for the medical teams, which may include outpatient reports, detailed discharge summaries, operation notes and other relevant clinical and non clinical correspondence. This may include correspondence that is time-sensitive (such as complaints) ensuring information is dictated and typed within any designated time limits. Type medico-legal summaries, maintain medico-legal files and liaise with legal department in respect of medico-legal cases Manage patient investigation results effectively including maintaining systems for monitoring and retrieving outstanding investigations and responsibility for chasing up missing/delayed results. Retrieve patient results from ICE/CRIS systems, ensuring that results are available as soon as possible Check all incoming laboratory/radiology results, ensuring that abnormal results are communicated promptly to appropriate Clinician. Will be required to instigate appropriate referrals to other disciplines in respect of specific abnormalities in line with Departmental protocols. Ensure all reports are seen and signed by Clinician prior to filing in case-notes. Ensure that pathways are closed where appropriate following discharge or paperwork or coded appropriate if not discharged. Process health records of patients discharged from hospital; where standard letters have been generated at ward level, ascertain from case-notes whether any supplementary letter/action is required and deal with appropriately. Ensure that follow up appointments or referrals to other specialties are arranged where necessary. Ensure that relevant investigation results received after patients discharge are communicated to GP where appropriate, particularly where treatment may be required.
