This is a brand new role, created due to growth, and will be leading the Integrated Substance Misuse Service Team (ISMS) as we roll out our new One Team approach across the service. The service offers a broad and diverse range of treatments to patients suffering from a range of substance misuse issues. Hours This is a full time role, working 37.5 hours per week, Monday to Friday. Shifts are 08:00 - 16:00 / 09:00 - 17:00. Your main jobs and responsibilities will be; To manage and update central information management systems within the Integrated Discharge Service Manage a central repository of information to support the Integrated Discharge Service in a timely and confidential manner. Update of referrals onto management information systems relating to patient details, needs and resources in a timely and accurate manner. Act as a central point for the Trust where live information can be gathered on capacity across the two sites. Produce and circulate robust daily information reports to support the Integrated Discharge Service processes and make available in a timely fashion to senior management colleagues. Actively develop effective internal and external relationships and work closely with colleagues to ensure a successful service delivery. Communicate with clinical and social care colleagues within the Integrated Discharge Service involved with each patient using a variety of communication methods including, verbal, written, interpersonal and computer, and passing information in a format that is appropriate to the individual request.
