Cheshire and Merseyside Adult Gender Identity Collaborative (CMAGIC) is a new NHS England pilot, establishing how Gender Incongruence can be best treated within a Primary Care setting. We are seeking a specialist nurse to support the delivery of this care and they will work closely with our MDT, local GP's, relevant partner agencies and of course directly with service users.

This is an exciting opportunity to be involved in developing not only a new service, but a new way of delivering care for a disadvantaged service user group. The role will likely involve travel, so a car user is essential.

To provide direct, clinical nursing care to the service users of the Cheshire and Merseyside Adult Gender Identity Collaborative (CMAGIC). The Trust is seeking to recruit a full-time Consultant Forensic Psychiatrist based at Rathbone Low Secure Unit in Liverpool. The service has32 male beds across two wards and provides assessment and management for patients from North and South Liverpool, South Sefton (up to Southport) and part of Kirkby. The population covered is approximately 1/3 million. The service provides low secure forensic care for detained patients with a mental disorder who present a significant risk of harm to others and cannot be managed in a non-secure environment. The service forms part of the secure services pathway within Mersey Care.

The vacancy is an established permanent post which has arisen due to retirement. The post holder will join a comprehensive MDT with good trainee medical support including a full-time substantive staff grade doctor and core trainee. There is also a full-time substantive Physician Associate. They will manage a caseload of 16 male inpatients and will maintain responsibility for the patient throughout his journey through the unit. There is a second substantive consultant who has responsibility for the other inpatients.

The post currently attracts a 10% R&R payment which will be reviewed in 2025.

The post holder will act as Responsible Clinician within a multidisciplinary team. They will lead on the management of a caseload of 16 male inpatients. This will include preadmission planning, MDT meetings, CPA reviews and discharge planning. The post will require liaison with other agencies, including the Ministry of Justice, police, probation, social care and other mental health services. They will exercise statutory responsibilities for patients detained under Restriction Orders, including completing statutory reports to the Ministry of Justice. For both non-restricted and restricted patients under their care they will be responsible for providing reports regarding Consent to Treatment and providing reports to, and attending, Tribunals and Managers’ Reviews.



The post holder will lead on the assessment of individuals referred for admission from a variety of sources including other secure services, general adult services, courts or prisons. . These are discussed at the weekly referral meeting. The postholder will participate in the panel meetings which are also held weekly and review referrals for admission to low secure and forensic step-down beds across the Merseyside and Cheshire region.



The post holder will be expected to participate in the management of the service and attend clinical team meetings and other service-related meetings as appropriate. They will also provide supervision to the junior doctors and / or Physician Associate working in the service.

The post holder will exercise statutory responsibilities as a Responsible Clinician within the meaning of the Mental Health Act 1983 (Amended 2007) for patients detained under Restriction Orders, including completing statutory reports to the Ministry of Justice. This is an exciting opportunity for a Band 5-6 developmental Caseload Holder Childrens Bladder & Bowel Team.

The Development Service Delivery Manager will work with a range of stake holders (both internal and external) to ensure that developments are delivered to support our partners and customers objectives. Responsible for the provision of appropriate, high quality care to a defined caseload.

Responsible for workload planning and delegation of work to other team members, taking into account competence levels.

Undertake holistic assessments of need for the patient and family, which may be highly complex, using the appropriate model of care and evidence based practice.

Work in partnership with other professionals to enable patients/clients to be maintained in an appropriate environment, thus avoiding hospital admission. Provide advice to patients, clients and carers on a wide range of health issues to include health promotion, life style advice and health care.

Ensure that the team provides a high quality clinical service to its clients by participation in caseload and clinical supervision.

In conjunction with the team leader, develop new ways of working to provide a responsive service to patients with both planned and unplanned care needs.

Develop partnerships and joint working within other agencies as appropriate. Maintain registration in line with professional bodies.

Clinically lead, direct, mentor and support the team. Support the team leader in the development of caseload management across the local health economy.

Acts as an advocate and champion for patients and/or clients. Act as a role model to team members and students so that patients/clients receive the most effective care possible.

Contribute to the development of policy and services to reflect the needs of the local neighbourhood population.

Participate in the development, implementation, and monitoring of the clinical team contract/objectives.

Provide information to the team leader on staff and patient/client activity as requested, to enable the effective management of the team.

Support the team leader in the implementation of practice development, improvement and service modernisation initiatives.

Support the team leader by monitoring and recording sickness and absence, capability and other issues within the team, reporting them as soon as possible. In conjunction with other caseload holders, provide day-to-day supervision and support for the team.

Support the team leader in providing induction to the local working environment, clinical services and policies for new team members and students.

Undertake personal development plans with other members of the team, as delegated by the team leader.

Act as an authorised signatory for mileage and time cards for submission to the team leader.

Communicate with the multi-disciplinary team to provide best outcomes for patients/clients; this may include organising case conferences related to highly complex cases.

Participate in individual and group supervision.

Monitor record keeping within the team and inform the team leader of any issues.

Support the training and development of team members who need to improve on the level of patient care delivery. This includes record keeping and improving clinical competency.

Support the implementation of any initiatives to improve quality within the clinical governance framework.

Participate in patient satisfaction reporting to improve patient care. Work with the team leader to ensure achievement of your own personal development plan.

Maintain up to date knowledge and competence in line with professional and service requirements and demonstrate critical thinking, decision-making and reflective skills to ensure own professional development.

Support the Team Leader in creating an environment in which learning and practice development is fostered, evaluated and disseminated. Facilitate effective learning within the area of practice for all students and practitioners.

Facilitate pre and post registration student placements within the team.

Mentor and assess preregistration students.

Participation in clinical audits and research, as required.

The post holder shall as necessary provide cover for and undertake duties of absent colleagues.

The post holder will provide expertise and advice in the treatment of highly complex conditions, undertaking treatments as required. In addition the post holder will use their expertise to be a source of training and mentorship to those within the immediate team/area and as necessary, more widely.

Deliver clinical training and supported experience, identified via Personal Development Plan (PDP) and Knowledge and Skills Framework (KSF).

Work within guidelines to identify and manage risk, reporting identified risks to the service lead.

Report any incidents as per Trust policies and support or undertake any investigations as delegated by the service lead.

Monitor and ensure that the quality of the patient care delivered by those to whom training is provided is evidenced based and supported by best practice, through the use of audit, caseload and clinical supervision.
