• Undertake basic physical monitoring e.g., vital signs, blood sugar monitoring and venepuncture for service-users.

An opportunity has arisen for a fixed term contract Band 6 Occupational Therapist to join the North Kensington and Chelsea Community Mental Health Hub. The post is from the starting date until November 2025. The team is located in W9 close to Westbourne Park station, with great transport links and serving the diverse community of North Kensington. We are looking for an Occupational Therapist with motivation and enthusiasm for working in a community mental health role.

The post holder will be a valued member of the team and will offer Occupational Therapy assessments and intervention for clients as well as supporting the overall work of the team by providing a named worker role for some clients and supporting the overall work of the team.

Rough Sleepers Mental Health Nurse - Brent CMHT

FIXED TERM - 12 MONTHS

The Communiy Mental Health Team are seeking to recruit an enthusiastic and dynamic mental health nurse who can be clinically creative in engaging an often marginalized client group.

Your skills and abilities will enable you to work closely with other statutory mental health services, housing provider services and social care teams to deliver an assertive and responsive service throughout the rough sleeper pathway.

You will also gain experience of working closely with third sector, charitable and voluntary homeless services. This role involves working in many, varied settings and on any given day you may find yourself engaging clients at their sleep sites, hostels or day-centres in addition to the more usual community practice areas.

Key Duties



Clients/Users



· To contact rough sleepers, homeless people in Day Centres and other voluntary organisations providing services for homeless people.

· To undertake individual comprehensive assessments of mental health housing and social care needs.

· To offer short-term intervention to link clients into mainstream services where appropriate.

· To undertake care management assessments and responsibilities for individual clients in relation to care planning.

· To carry an allocated caseload of clients with ongoing Care Management responsibilities for assessment, care, treatment and review for individual clients.

· To assist where necessary in rehabilitation programmes for individuals and groups.

· To advocate for clients in relation to access to services, including welfare benefits and housing and social care, and complete applications as required.

· To participate as a full member of the multidisciplinary team.

· To work assertively, where necessary, with clients who may actively wish to avoid contact with mental health services, identifying and assessing the potential risks involved in work activities on the street and how best to manage these risks.

· To liaise closely with other outreach teams and assist in the assessment of clients referred by them.

· To undertake follow up work with individuals contacted on outreach and to engage and assess clinical and social care needs.

· To undertake regular sessions as required in hostels and day centres as a means of maintaining contact with service users who are street homeless, and contributing to the team’s work generally.

- To deliver a highly specialist evidence-based mental health assessment, formulation of diagnosis ,care planning, treatment and review for women in the perinatal period .

- To provide specialist evidence based interventions such as CBT ,VIPP , VIG, to women in the perinatal period , to the parent infant dyad or the family system.

- To provide specialist consultation, advice and support to both CNWL, local Maternity Hospital in-patient and community services on best practice when working with women with mental health needs; at preconception, pregnant women and women with infants up to one year old.

- To work jointly with care coordinators from CMHTs with a small complex and challenging caseload of pregnant women or those with an infant up to one year old.

- To work in consultation with other professionals making known concerns regarding safeguarding children and/or adults to the appropriate and responsible service. This includes attending Child Protection and Review Conferences as appropriate where the child is at risk because of the mother’s mental illness.

- To work with colleagues to establish a clear process and criteria for receiving referrals to undertake direct work or information for advice and liaison work.

- To assist in setting up structures that allow the dissemination of knowledge and skills regarding working with perinatal women in both in-patient and community services.

- To liaise and work with specialist tertiary services such as Mother & Baby Units, personality disorder services, eating disorder services etc.

- To work with fathers, partners and significant others to provide appropriate education training and support.

- To identify carers and refer/sign post for assessment as appropriate.

- To work as a member of the multi-disciplinary team exercising autonomous professional responsibility for managing a clinical caseload independently, providing highly specialist assessments and follow-up for women referred with complex problems, which includes assessment of risk in terms of self-harm, self-neglect, vulnerability, harm to others, domestic violence and safeguarding children.

- To seek consultation where necessary with clinical supervisor and/or line manager on clinical work and participate in regular supervision.

- To be mindful of the needs of women and their families from a wide range of racial, cultural, sexual, religious and social backgrounds and to develop, with colleagues, sensitive and innovative ways of meeting these needs.

- To ensure that all Trust Perinatal service users have a perinatal care plan, which is implemented. This requires attending and contributing to pre and post-birth meetings.

- To work with other professionals in supporting them to implement an agreed parent –infant care plan.

- To attend the CNWL Safeguarding Children multidisciplinary meetings and any other relevant multidisciplinary meetings within Maternity services, as appropriate.

- To be responsible for monitoring, recording and reporting on clinical work and communicating highly complex clinical information to a variety of recipients (e.g. patient’s, families and carers, other professionals) orally and in writing.

A vacancy has arisen within the South Westminster Community Mental Health Hub for a Band 6 Primary Care Mental Health Practitioner. Based in Victoria, we are a small and friendly team with a reputation for providing excellent care for our service users, professional growth and strong collaborative partnerships with our primary care and third sector colleagues.

Our Hub comprises the Victoria and West End Primary Care Networks, which work as distinct MDTs with the Triage Team functioning semi-autonomously as a central point for new referrals. The North West London Occupational Health Shared Service (NWLOHSS) is hosted by Central and North West London Foundation Trust (CNWL) and is made up of a team of experienced occupational health doctors and nurses, supported by administrators. Job Description & Main Responsibilities

The successful candidate will be expected to:

Conduct project work regarding the monitoring and audit of occupational health activities to ensure these are undertaken in the most efficient and effective manner, in accordance with the clinical governance strategy for the service. This will include, computer based and document review processes, customer and client surveys including MoHaWK surveys and maintaining SEQOHS accreditation.

Act in a calm and professional manner at all times, affording courtesy to all with whom he/she may come in contact through the course of his/her work.

To plan and organise activities or programmes for their own area, of work; and work with the OH Clinical Team Leader on service changes impacting outside their own direct area of activity. He/she must comply with all CNWL and OHS policy and inform the Service Manager immediately of any difficulties in undertaking the requirements of the post.

Will have specific responsibilities in respect of mentoring and training.

In accordance with the CNWL policy you will be provided with appropriate opportunities and support for their professional development and training.

You will assess fitness to work of an individual in relation to the specific post for which they are to be employed, in accordance with Occupational Health Service standards and procedures. This will include the discussing and processing of highly complex and sensitive information, in situations where clients may exhibit hostile/ highly emotive, behaviour. High levels of interpersonal and communication skills are required especially where judgements on fitness for work, affects future employment.

You will protectemployee health at work including; the organisation and undertaking (both internal and external) of vaccination clinics; vision screening programmes, workplace risk assessments; providing advice to management and employees on the measuresnecessary toprotect against any physical, chemical, psychological, or microbiological hazards, arising from their work or from the conditions in which it is carried out.

You will represent the Occupational Health Service at committees, working groups, and site visits; plan organise and undertake training. Co-ordinate occupational health activities with other health, safety and welfare services utilised by the employer and employee.

You will promote employee health at work including the planning and undertaking of specific screening programmes; health and well-being project work and ad hoc occupational health training activity as directed by the OH service manager and / or requested by the contract managers.

You will supportof individuals with disabilities in both gaining and sustaining employment through the provision of specialised advice concerning fitness for work; phased return to work involving modification, adjustment, restriction of duties either on a temporary or permanent basis; liaison with other health, safety and welfare services. Such advice may be supplied either within a one to one referral process, or in a range of group setting such as, case conferences or employer / manager groups.

You will maintain client's clinical records, in accordance with professional standards, ensuring confidentiality of information held, and meeting legislative professional and clinical standards of record keeping.

You will take supervisory responsibility for:

You will oversee day-to-day operational activities related to assigned contract(s), ensuring the efficient and effective delivery of service. This will include ad hoc delegation of duties for clinical and administrative staff where circumstances require such and to report to the service manager of any operational or resource difficulties.

For any other activity as commensurate with the post.

You will establish strong working relations with the authorised officers of the customer is an essential element of this post

You will interview and support client through the various assessment processes and communicate complex and at time contentious information in a sensitive manner

You will be required to establish and maintain good working relationships with all occupational health staff

Maintain professional relationships with staff from other NHS voluntary and local authority agencies

You will plan, prepare and deliver specialist advice regarding fitness to work and protection, promotion, support of disability at work, via oral, written & electronic use and deal with highly complex, sensitive or contentious issues. The above may be on a 1:1 basis or to groups / small customers, as directed by the service manager.
