Care Assessor

Responsibilities:

Assessment and Evaluation: Conduct thorough assessments of clients' care needs, taking into account their physical, emotional, and social requirements.

Care Planning: Collaborate with the care team and clients' families to develop individualized care plans that cater to specific needs and preferences.
•
Client-Centered Approach: Ensure that the care plans are designed to promote independence, dignity, and overall well-being of the clients.
•
Care Recommendations: Make recommendations for appropriate services and support to meet the clients' identified needs effectively.
•
Regular Monitoring: Regularly review and update care plans based on changes in clients' needs and circumstances.
•
Communication: Maintain clear and open communication with clients, families, and the care team to ensure everyone is informed and involved in the care process.
•
Quality Assurance: Ensure compliance with all relevant regulations and standards while maintaining the highest quality of care delivery.
•
Client Advocacy: Act as an advocate for clients, ensuring their voices and preferences are heard and respected.
•
On Call : You will be part of a rolling rota offering support outside office hours to our clients and staff.
Requirements:
• Previous experience as a Care Assessor, Care Coordinator, or a similar role in the care sector is preferred.
• A deep understanding of care assessment processes and care planning.
• Excellent interpersonal and communication skills to interact with clients, families, and the care team effectively.
• Compassionate, patient-oriented, and empathetic approach to caregiving.
• Knowledge of relevant regulations and compliance requirements related to domiciliary care services.
• NVQ Level 3 in Health and Social Care (or equivalent) is an advantage.
• Strong organizational skills and the ability to manage time efficiently.
• Driving licence and access to a car is essential.
