The post holder must be available to deploy the A-EQUIP model as required by the Trust. To support nurses to identify their own personal action for quality improvement if required. Use their understanding of personal and professional resilience and support others to develop this attribute. Ensure that there is a strong interface between the PNA and Clinical Governance at the NNUH. Use their expert knowledge in nursing practice to support the implementation and monitoring of practice, ensuring that there are robust clinical governance assurance systems in place. Provide support to nurses in sharing of good practice learnt through the risk management processes with the aim of reducing the incidence of harm and litigation cases. Be involved in continuous monitoring and maintaining quality standards of clinical practice through a variety of ways for example audit, working clinically with nurses for those who require additional support and development. Act as a role model at all times and promote a high standard of safe and effective care which is based upon best evidence. Meet with nurses in their designated area and/or caseload on either an appointment or ad hoc basis. Use effective communication strategies and influencing skills to achieve desired outcomes and make appropriate referrals for advocacy, mediation or arbitration. Support midwives to work in partnership with patients to develop plans of care which meet their individual needs, to listen to them and to advocate for them as required. You will have a detailed induction to our PCN and any practices that you work in. This will help you develop an understanding of how each practice functions, along with training in SystmOne which all the practices use. While working, you will have access to a GP clinical supervisor/mentor at all times including enhanced access sessions. As well as undertaking normal GP patient care duties, if wished, you will have the opportunity to develop a special interest or spend time on PCN development projects, for example, our PCN currently has a project to help improve the mental wellbeing of our children and young people via a Youth Worker and all age Social Prescriber. We are also looking at using coproduction to develop improved accessibility to healthcare for our patients with ASD, vision or hearing difficulties, or other reasonable adjustment needs. We also have a large Clinical Pharmacist team who are developing additional skills such as managing heart failure and hypertension. We hope that, once you have been with us for a few months, you will: Be developing into a more skilled and confident clinician Feel supported by the team around you and feel able to support your colleagues too Have an understanding of how different practices work Feel engaged and enthused by the variety of your work Be developing supportive relationships both within practices, and within partner organisations.
