• Alternative or collateral duties may include management of a subsection of primary care patients. Typically, these duties are designated at the time of joining but occasionally during the course of employment, participation in special projects pertinent to the assigned section may be necessary.

The VISN Clinical Resource Hub (CRH) Primary Care Physician position is a VISN based position located in VISN 20. This Primary Care Physician position is under the general supervision of the VISN CRH Associate Director of Primary Care. A Clinical Resource Hub (CRH) is a clinical and administrative staff repository that serves Veterans Affairs facilities within a Veterans Integrated System Network (VISN). Staff within the CRH are expected to provide interprofessional services to VISN facilities underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Services provided by CRH staff in this position are conducted in-person.

Staff within these CRHs are expected to provide interprofessional services to VISN facilities underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Services provided by CRH staff via a mobile deployment are sent to the location for 3-6 months at a time (gap provider).

The CRH Mobile Deployment Team (MDT) provides care through on-site care and is a Mission Act 402 requirement. In-person deployments of clinical staff for short-term assignments (typically 3-6 months) will be at the VISN CRH Associate Director of Primary Care discretion. They will depend upon clinical needs and available resources.

• Serves as a liaison for PACT Pain related issues, patient advocacy, Facility-related congressional inquiries, items requiring PACT Pain engagement, and/or information around national initiatives and tools (SCOUTT, CARA mandates, opioid safety, Facility pain teams). This role would support the execution of PMOP initiatives within the PC/PACT purview.
• Provides PC/PACT subject matter expertise and work closely with Primary Care, Pain Specialty, and Facility leadership to facilitate implementation of the Stepped Care Model for Pain Management (SCM-PM) and the PACT Roadmap for Managing Pain, proper pain management and optimization of Whole Health oriented pain care, Opioid Safety Initiative (OSI) and data-based risk reviews for opioid-exposed patients (including Stratification Tool for Opioid Risk Mitigation (STORM)), other interdisciplinary pain care forums, and transition of pain management-related patient care from PC/PACT to other collaborating service(s) assisting with the step up in care when needed for complex pain patients.

The post holder should be able- -To manage own specialist wound care caseload. - Work autonomously within Trust policies and professional practice, maintaining accurate and comprehensive up to date records. -To carry out specialist assessment and treatment of podiatry patients, facilitating safe and timely discharges and transfer of care as appropriate. -Determine a clinical diagnosis, develop, deliver, implement, evaluate and modify individualised treatment programmes and/or provide specialist advice, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required. -Take part in the education and training of podiatrists and qualified and non-qualified healthcare / social services / education staff including medical staff. - To support the Team Leader in the day to day running of the Podiatry Service. -To support and advise within the wider MDT on the delivery of the service -To develop and deliver effective clinical interventions within the speciality of wound care, contributing to clinical practice in the Podiatry Service in line with current evidence base. -To manage autonomously a clinical workload of patients / clients at a specialist level, assessing and treating patients and maintaining clinical records. -To have specialist assessment and treatment skills in the areas listed but not limited to: - the assessment, diagnosis, development and implementation of individualised care programmes for patients with a wide range of acute or chronic health problems; such as diabetes, rheumatoid arthritis neuropathy, vascular insufficiency and wound management. - Receiving and processing complex medical information relating to high-risk patients, thus, facilitating diagnosis and implementation of an appropriate treatment plan, providing advice and education as an integral part of the plan. - reviewing treatment plans regularly and when appropriate. - Discharging the patient with a care package of education and/or orthotics with the option to self-refer back to care. -To apply evidence based knowledge within the specialist clinical area acquired through experience and academic study to a specialist post graduate level. - To act, within Podiatry and the Trust, as a resource and provide advice for podiatrists, other professionals and colleagues within the MDT about the assessment, treatment, management and care of patients / clients who have a multi-pathological illness and/or complex psychosocial need. -To work collaboratively with health and social care community and acute colleagues. To work with other members of the MDT to ensure effective wound care pathways and transfer of care / discharges. -To proactively and appropriately facilitate a seamless transfer of care and continuing therapeutic management of podiatry patients between care settings. - To work collaboratively in an interdisciplinary manner within the MDT, being able to debate the blurring of boundaries across professions and where competent take on aspects of blurred boundary working, where appropriate. -To develop specialist programmes of assessment, treatment and intervention for podiatry patients - To manage and prioritise own clinical workload as appropriate, working efficiently to manage professional time and delegate according to the training and competency levels of other team members to ensure the effective delivery of the service. - To participate in the supervision process with staff on a daily basis and for students as required. -To ensure a high standard of patient care is provided including implementing and reviewing clinical standards relevant to the work undertaken.
