The Transformation Director will be instrumental in supporting the delivery of large-scale transformation, particularly with the clinical reset programme, with and through the Provider Collaborative team and broader partners across Lancashire and South Cumbria. Working with the Managing Director for PCB, the Transformation Director is responsible for leading the Provider Collaborative’s Clinical Portfolios to deliver the following:

Change Management and Delivery
• Develop and drive the delivery of key programmes of change and improvement, in partnership with clinical teams to deliver patient benefit and financial sustainability.
• Work with the System Planning Lead on oversight of the operational planning cycle across the Provider Collaborative, ensuring key linkages between changes to leadership arrangements to financial ,workforce and activity are in place; closely aligning to the NHS Long Term Plan.
• Responsible for ensuring the plans of the Clinical Portfolios align to NHSE strategic aims, and have actions in place to deliver the clinical, operational, and financial outcomes sought; and that complexity / uncertainty is understood and mitigated.
• Proactively support the working relationships between the Trusts at CEO, Executive and Professional Lead level; providing fair and transparent brokerage focused on achieving the outcomes agreed.

Strategy and Planning

The post holder will hold a case load of patients suffering with severe and ensuring mental illness, working towards the recovery model to support patients to achieve positive outcomes. The post holder will provide a good role model, develop and maintain good systems of communication, be able to communicate with a wide range of people in a range of situations. Reporting verbally and in writing with regards to clients progress, assessments and care planning to the wider multi-disciplinary team. Maintain accurate and complete records. Discussing with client and their families / carers regarding care planning, assessments and treatment. Be able to acknowledge the barriers to effective communication and adapt communication style in response. Maintain professional standards in line with NMC code of conduct and Trust Nursing Strategy. The post holder will monitor and maintain the health and security of self and others. Managing identified risks. Ensuring that health and safety legislation is adhered to. Manage emerging situations, taking action as appropriate. Reporting actual and potential incidents. Attend Trust mandatory training. The post holder will contribute to the continuous improvement of services. Identifying the implications of direction, policy and strategy on current practice in a constructive and timely manner. Actively involve service users and the public in service improvements. Timely investigations and response made when dealing with PALs issues in line with Trust policy.

Duties include but not limited to:

Performing the following tasks during the clinic visit:- Reviews information from outside providers- Conducts history and physical exam- Assesses patient's clinical condition- Formulates treatment plans- Completes provider-specific clinical reminders- Reviews test and procedure results- Completes patient referrals in CPRS- Manages patient's medication (including medicine reconciliation)- Gives medical and lifestyle advice and counseling

Performing the following direct patient care responsibilities when patient is not present:- Completes view alerts- Follows-up on patient-related information (test and procedure results, consults and medications)- Calls patients and family on the telephone- Sends and receives secure messages from patients- Discusses cases with consultants and with PACT team- Reviews and acts on non-VA medical records- Completes forms/paperwork- The provider participates in all PACT team meetings and other team activities- Collaborates with NP and PA providers and directs care for the panel.


Work Schedule: Monday - Friday 8:00 AM - 4:30 PM (Will take after hours critical lab call on a rotating basis (approx. 4 weeks yearly))
