PRINCIPLE RESPONSIBILITIES To provide technical support to the Ophthalmology service. To independently assess and interpret visual acuity measurements. To independently perform accurate imaging of the eye using OCT scanning and Optos photography on a range of patients referred from the ophthalmology service. To independently perform accurate visual field assessment using the Humphrey Visual Field Analyser on a range of patients referred from the ophthalmology service. To select appropriate visual field tests taking into account patient ability i.e. age, visual acuity, mobility and diagnosis. To undertake intraocular pressure measurement using Reichert ORA. To undertake corneal mapping using Pentacam. To adjust variables to ensure optimum performance for each patient. To be able to measure patients glasses accurately on a lens meter (focimeter). To use knowledge and experience to identify when a patient is not complying with an assessment, and inform medical staff of the appropriateness of the test for these patients. To use knowledge of the anatomy and physiology of the eye and visual pathway to help identify pathology and adjust testing programmes as appropriate. Use knowledge of ophthalmic conditions to prioritise follow up appointments. To ensure that results are fed back to the appropriate clinician. To perform and demonstrate tests to other members of staff and aid with teaching them when necessary. To work independently within departmental guidelines and identify when input from other medical staff is required. To communicate well with all members of the team and demonstrate a courteous and helpful approach to patients and visitors. To be adaptable and manage the workload which may be unpredictable and require changes and prioritisation at short notice to meet demand. To ensure equipment is used safely and is correctly calibrated. To keep line manager informed of any equipment faults. COMMUNICATION To be able to exchange information with patients using persuasion and reassurance to ensure best test results while making sure the patient is comfortable many patients will be elderly and require moderate physical help. Empathy is needed when dealing with patients with sight threatening and life changing sight problems. To liaise with other staff within the service to ensure a good patient experience, keep visits to a minimum and maximise resources. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Anticipate barriers to communication and take action to improve communication. Adapt the information given to the level of understanding of patients of all ages and abilities. This may be of a sensitive nature and will need to be phrased so there is full understanding of the condition. Ensure respect and dignity for patients at all times. Ensure awareness of sources of support and guidance (e.g. PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate. Develop, organise and maintain the Pharmacy Clinical Trials Services in the dispensaries, in conjunction with the Clinical Trials Pharmacist, ensuring compliance with Good Clinical Practice (GCP) guidelines, the Research Governance Framework, the E.U Clinical Trials Directive, the Medicines for Human Use (Clinical Trial) Regulations 2004 and other relevant guidelines, regulations and policies. Manage the process of purchasing, ordering, receipt, storage, dispensing, return and disposal of Clinical Trial drugs within the dispensaries and maintain stock levels. Manage and maintain the database to monitor patients activity in non-commercially Sponsored Clinical Trials within the dispensaries. Manage and maintain the database of the training of the dispensary staff undertaking Clinical trial work. Obtain Good Clinical Practice (GCP) Training. Work within the Medicines for Human Use (Clinical Trial) Regulations 2004 and the EU Directives relating to Clinical Trials and the Research Governance Framework. Review all Clinical Trial protocols which will involve the dispensary prior to Trust Research & Innovation(R&I) Approval. Participate in confidential communication with the Clinical Trials Pharmacist, Investigators, Research Nurses, Trust R&I, Sponsors, Pharmaceutical Company Clinical Research Associates, Pharmacy Aseptic Services Unit (PASU), dispensary staff and the Clinical Trials PASU Technician during the Pharmacy Approval process, the setting up of new studies and during the running of the study. Develop, implement and maintain clear and accurate written SOPs for dispensing for each clinical trial in the dispensaries in accordance with Good Clinical Practice (GCP) and each studys protocol. Ensure Clinical Trial protocols are followed during dispensing of Clinical Trial Investigational Medical Products and other drugs included in the protocol. Maintain and setup systems to reclaim any reimbursement of free or discounted stock of Investigational Medical Product used in Clinical Trials within the dispensaries. Maintain Clinical Trial filing systems. Design drug accountability documentation and prescriptions for Clinical Trials, if required. Work in line with R&I objectives/strategy

PHARMACY TECHNICIAN JOB DESCRIPTION Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-cantered, coordinated care in our Practices. The Clinical Pharmacy in General Practice model is supported by the direction of national policy including the Five Year Forward View and GP Forward View where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice. Pharmacy technicians play an important role, complementing clinical pharmacists, community pharmacists and other members of the PCN multi-disciplinary team. Pharmacy technicians are different to clinical pharmacists as they are not able to prescribe or make clinical decisions, instead working under supervision to ensure effective and efficient use of medicines. The purpose of the role is to lead improvements to maximise safe, cost-effective best practice in prescribing to improve the quality of patient care. The post holder will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines. In addition, the post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments. SPECIFIC JOB RESPONSIBILITIES CLINICAL Work with the Clinical Pharmacists and Clinicians in accordance with agreed practice prescribing standard operating procedures. Process repeat medication requests, including the conversion of acute requests to repeat medicines, changing quantities of medicines. Processing prescriptions in line with agreed clinical protocols and within your competency support medication reviews/monitoring and medicines reconciliation post discharge (or other transfer of care) and synchronizing medicines for patient especially if transferring between care settings, linking with local community pharmacies. Undertake patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients. Support the Clinicians and Clinical Pharmacists in Structure Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR. Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients . To develop and maintain medicines related communication to all Practice staff To assist with the development and review of medicine audits Provide a contact for patients and colleagues with medication queries. Support with project work/prescribing initiatives/clinical audits under the support of the Pharmacist Liaise with Clinicians and staff to resolve prescribing queries. TECHNICAL AND ADMINISTRATIVE To update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken. To assist the Pharmacist with data analysis of prescribing behaviour in line with NICE guidance and local clinical commissioning directives. To adhere to strict confidentiality policies at all times. Attend local meetings seeking to improve access to medicines or repeat prescribing processes. Support promotion and maintenance of Electronic Repeat Dispensing (eRD) and online ordering Support the development of practice reception teams in streaming general prescription requests, so as to allow GPs and clinical pharmacists to review the more clinically complex patients. Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
