The Care Home Support Team in Richmond have an exciting new opportunity for you to join our dynamic team as a care planning practitioner.

Working in partnership, the postholder will support the ongoing implementation of integrated neighbourhood teams and MDT working in care homes across the Borough of Richmond.

You will support the implementation, management and delivery of enhanced MDT working in care homes across Richmond. The postholder will work in partnership with care homes, Care Home (GP) Leads, acute and community services to deliver enhanced MDT working and increase the use of the Universal Care Plan.

You will collaborate with professionals to initiate care planning discussions including advance care planning, comprehensive geriatric assessment, frailty and dementia to initiate and complete a universal care plan.

The University of Leeds has been chosen by the UK government to lead the National Institute for Health and Care Research (NIHR) Research Delivery Network Coordinating Centre (RDNCC), and will be joined by 12 new Regional Research Delivery Networks (RRDNs), hosted by NHS organisations across the country to form the NIHR Research Delivery Network (RDN). The new NIHR RDN will operate as one unified organisation across England, balancing regional context, expertise and leadership with national coordination and strategy involving government policymakers. The RDNCC will work across England’s health and care system, with staff in all settings, to support the effective and efficient initiation and delivery of research.

You will be part of a team supporting the development of the RDN strategies and developing impact reports from them. You will manage the Strategic Development Operational Group and System-Wide Review Groups. In addition, working with various external stakeholders you will be responsible for developing communications materials, running meetings and events, including agenda setting and action management, and responding to queries. You will provide line management for the coordinator and administrators within the team and work with the Project Leads to allocate work to administrators as appropriate.

You will work collaboratively with colleagues within the RDNCC and, as required, across the RDN and the University of Leeds, and with external stakeholders in a host of settings. You will support the operational delivery of the organisation-wide shift in ways of working and culture that is required to ensure the network functions as a single organisation with a shared purpose and vision across England, as well as mutually supporting colleagues and managing your own team through the period of transformation and beyond.
