We are looking to recruit a B5 Dialysis Nurse who takes patient centred care to the heart of their role. If you are experienced , dedicated and proactive you will enjoy the post at our Aintree Satellite Dialysis Unit , based at Aintree Hospital. We are a LUFT dialysis centre and have 9 units across Merseyside and strive to provide excellent care and standards to our dialysis population. The unit currently has 12 stations with 3 shifts of patients per day .The unit is open Mon - Sat 0700-0000hrs with shifts that include a variation of 0700-15.30/1500-0000 and 0700-18.30hrs.

It is a Nurse Led Unit with 2 Consultant Nephrologists , and a dietician as a part of our MDT. We follow the Named Nurse Concept and all Dialysis staff have a cohort of patients to monitor and provide an Holistic approach to care.

This post will support the activity and productivity of outreach staff delivering targeted work with vulnerable groups at risk of sexual ill-health. The principal target groups that Sexual Health Outreach provides services to are;

The post holder will also be required to work as an accountable practitioner working on their own in community settings, making decisions according to policy and protocol. Hours of work will be flexible to include evening and weekends across the axess regional footprint as required.

As the pharmacist in charge you will be responsible for the provision and delivery of an integrated, safe, legal, high quality prison pharmaceutical service. The Norris Green Community Mental Health Team supports individuals who have severe and enduring mental health difficulties. The team is a busy and skilled multi-disciplinary group which actively promotes close partnership working within the wider community.

The role will include providing high quality evidenced based care, formulating risk management plans, supervising risk assessments, monitoring medication, administration of depots, providing expertise and advice, care co-ordination and supervising junior staff. In addition, we require a strong commitment to working with service users with complex needs, in order to assist them plan and take steps towards their recovery as well as promoting signposting to additional services such as Psychology, Life rooms and wider resources available in the community.

As well as managing a caseload you would be expected to be a part of the duty rota responding to service users in crisis during the teams working hours.

The post holder will be based within the Community Mental Health Team and will :

Provide assessments and develop treatment plans for service users on caseload

Act as named professional/care coordinator and coordinate care between services

Hold a caseload of people with severe and enduring mental health problems Participate in team meetings, MDT meetings and training opportunities

Provide advice and support for service users and carers

Participate in delivery of group interventions

Supervision of Junior colleagues in the team

Liaise with colleagues and coordinate care for service users on caseload

Support and monitor the service user whilst commencing medication

Work collaboratively with other members of the MDT

Attend regular supervision and team meetings

Participate in training opportunities

Support people with complex needs and provide advice and guidance as people’s needs change

Be part of a duty rota

Monitor medication and administer depot medication

Assist the Divisional and Lead Clinical Pharmacists in the development of pharmacy services and medicines management within the directorate. Support the NHS in its efforts to use cost-effective treatments within your speciality. To provide a safe and effective advanced clinical pharmacy service to the directorates within the Trust. Assist in providing clinical training and supervision, relevant to the directorate, for other departmental staff. Provide non-medical prescribing services to our patients Identify the clinical and economic impact of medicines use, develop and implement clinical guidelines, monitor drug expenditure and support the area to meet their clinical and financial targets. Support the national agenda in helping create value within the organisation supporting biosimilar switches.
