Band 6 Mental Health Practitioner Link Worker, Bury CAMHS.

As part of the Child and Adolescent Mental Health partnership

in Bury, the post holder will undertake a wide range of duties to develop support and make strong links with partner agencies, including local schools. The Link Worker will provide a key facilitation role in developing collaborative working between CAMHS Minds and local schools/colleges. The main aim of this role is to provide levels of support and advice that will lead to a reduction in the need to refer to CAMHS, but ensuring those children and young people with the most serious needs have easy access to mental health services. This role will be based within our Single Point of Access.

The post holder will also be responsible supporting the management of a new Single Point of Access enabling parents, teachers, education psychology services, school support staff and other professionals across all children’s and young people’s services to get easy access to advice, support and urgent care, where necessary

The successful applicant will be expected to demonstrate excellent understanding of the delivery of the child and adolescent mental health services link role and have the ability to engage with children and young people and families and to develop pathways into third sector and partner agencies.

The post holder will also have experience in screening referrals and decision making in terms of appropriate pathways, along with excellent risk management skills. They will also have duty responsibilities as part of the role.

The successful candidate will join a busy multi-disciplinary CAMHS team which includes nurses, psychologists, social workers and psychiatrists. Our vision is for our local community to be well equipped to support the young people of Bury to grow up with good emotional health and wellbeing.

We work closely with our partners in education, children’s services and the 3rdsector to achieve the recently developed ambitious Local Transformation Plan for CAMHS services. This plan provides us with the guide and resources to begin delivering our vision of having CAMHS workers available in new ways, and working closely with children’s universal and targeted services in each of our locality areas.

Primary Duties & Areas of Responsibility Assess, plan, develop, implement and evaluate treatment programmes for patients that promote health and well-being. Help the practice develop and deliver specialist primary nurse led services, including chronic disease management. Manages programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions. Maintains disease registers in liaison with administrative staff. Undertakes annual reviews of patients understanding and ability to self manage. Make professionally autonomous decisions for which he/she is accountable and provide safe, evidence based, cost effective, individualised patient care. Where necessary evaluate patient test results in conjunction with the patient and their GP and ensure the appropriate course of treatment is initiated. Make appropriate referrals to other members of the primary, community and secondary care teams. Undertake regular medication reviews for patients for groups of patients deemed suitable by the clinical team. Administer child and adult immunisations and vaccinations in accordance with national and local programmes. Competent in anaphylaxis and resuscitation techniques. Ensuring safe storage, rotation and disposals of vaccines and drugs and that adequate stock levels are maintained Undertake procedures including: venepuncture, BP management, urinalysis, peak flow, spirometry, stitch/clip removal, cytology and wound management Promote health and well being, giving general education advice on diet, contraception, smoking chronic disease management, exercise etc... Initiate and carry out programmes of health screening as agreed with the practice clinical team. Enables, supports and encourage individuals, families and groups to address issues which affect their health and social well being. Contribute to the practice achieving its GMS contract obligations including quality, organisational and access targets. Maintain accurate records according to NMC rules. Ensure all data protection requirements are met when gathering, recording and storing patient data. Liaise and maintain good working relationships with all members of the practice team. Work within the multi-disciplinary team within the practice and across the wider health system. Promote a learning environment for patients, nurses and other health professionals. Assist with the planning and implementation and teaching for practice staff including medical students and health care assistants. Attend and contribute to relevant internal and external meetings as required including practice clinical meetings, nurse team meetings and gold standard framework meetings. Maintain a personal progress and training plan to keep up-to-date with current thinking and developments as well as identifying own training deficiencies and attending suitable courses and appropriate. Promote evidence based practice through use of the latest research based guidelines. Monitor the effectiveness or their own clinical practice through quality assurance strategies such as the use of peer audit and review. Maintain professional registration. Work within the latest NMC Code of Professional Conduct. Record accurate consultation data in patients records in accordance with NMC guidance and other standards. Keep up to date with pertinent health policy and work with the practice team to consider the impact and strategies for implementation. Work collaboratively with colleagues both internally and externally. Participate in protocol development. Identify opportunities to delegate tasks to junior members of the nursing team. Help the practice develop in a cost effective manner, monitoring and using resources appropriately e.g. stock control of dressings, vaccinations. Participate in practice meetings as required. Participate in audits and inspections as required. As part of the nursing team ensure all appropriate practice policies are fully implemented. Promotes effective communication, relationships and team working within the practice. Provides clinical educational advice to support develop of the team and range of services provided. We have an exciting opportunity to appoint a Band 7 CBT pathway lead for Bury CAMHS.

The post holder will be qualified in CBT and CBT supervision or have an interest in pursuing accredited CBT supervision training. The postholder will have experiencing line managing mental health professionals, and will have the following responsibilities:

· To manage and develop the stepped care, high intensity CAMHS service for this age group (age 5 up to 18 years)

· To manage CAMHS clinicians and provide supervision for team members undertaking assessment and a range of interventions / managing caseloads.

· To develop and implement therapeutic plans of care for children and young people with mental health needs.

· Supporting the service to deliver training and consultation to other children’s agencies.

· To liaise with other agencies regarding the implementation of care plans and the management of risk for children and young people.

· To promote and support mental health awareness in agencies supporting children and young people.



The successful candidate will be able to work effectively in senior clinical leadership position, have excellent communication skills, welcome a challenge and have a real desire to make a difference to the care we provide to the child and adolescent age group inclusive of evolving models of care and modernisation of mental health services.

You should be able to demonstrate strong leadership skills and have a flexible approach.

You must have experience of working and delivering CBT to a wide range of clinical presentations including working in the field and child and adolescent mental health.
