The Nurse Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible for providing specific care to primary care patients. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The Advanced Medical Support Assistant (MSA) in Primary Care works collaboratively in an interdisciplinary coordinated care delivery model and performs receptionist duties, customer service and other duties assigned for the proper and timely treatment of patients and maintains appointment schedules for one or more outpatient clinics. Patient care is delivered in a team based model known as the teamlet. S/he works as a member of the teamlet including a Registered Nurse Care Coordinator, a Licensed Practical Nurse/Licensed Vocational Nurse and a Primary Care Provider. The teamlet provides the majority of the healthcare needs for an assigned panel of patients. Work Schedule: Monday-Friday day with varying shifts, on-Call rotation is required; Holidays, and Weekends as needed.

The Psychologist in this position serves as the Local Recovery Coordinator (LRC) for the Behavioral Health Service at the Harry S. Truman Memorial Veterans' Hospital in Columbia, MO. The Psychologist's goals are to help incorporate the concept of Recovery into the multidisciplinary services provided across all Behavioral Health programming. Mental health Recovery is a journey of healing and transformation enabling a person with mental health challenges to live a meaningful life in the community while striving to achieve one's full potential. Fundamental components of Recovery include self-direction, empowerment, and care that is individualized, person-centered, and holistic. This position will be 75% administrative duties/planning/consulting and 25% direct patient care.

Duties may include but are not limited to:

Clinical Responsibilities:

• LRC serves as a recovery ombudsman to the MH Service Chief, program managers, and staff on recovery and implementation of recovery oriented services.
• In conjunction with the Behavioral Health Service Chief, the LRC develops a plan for the facility that identifies staged implementation of recovery oriented services (e.g., use of recovery plans).
• LRC develops a facility recovery education plan for providers and Veterans and their families. Develops an ongoing education plan for providers - includes, but is not limited to; recurrent didactic presentations about elements of recovery (e.g., recovery plans) and recovery implementation by experts; recurrent presentations by recovering individuals, especially Veterans; information about community mental health support groups and other resources and how to make referrals; information about VA and national recovery materials and resources and how to access these; information about VA sponsored trainings and education on recovery (e.g., how to communicate recovery to patients and families); availability of recovery posters and brochures; training on cultural competency; and development and coordination of local recovery training as needed.
• Develops an ongoing education plan for Veterans and families which includes, but is not limited to; recurrent didactic presentations about recovery by experts; recurrent presentations by recovering individuals, especially Veterans; information about VA and community mental health support groups and other resources; and information about how to become involved in improving mental health care for Veterans through the Consumer Council.
• LRC serves as consultant to Behavioral Health programs and recovering Veterans in development of peer support groups, consumer counseling, and consumer operated services, as per local needs and resources. When available, community resources should be utilized.
• LRC is responsible for coordinating the evaluation of recovery implementation at the facility and reporting results to the NRC. With guidance from the NRC, the LRC will implement measures to assess recovery policy and procedure adherence, as well as assess the extent that the recovery model is embodied in the MH programs.7.The LRC is also expected to spend time engaged in direct clinical services providing recovery oriented care to seriously mentally ill Veterans. Direct clinical services will be focused may be in the areas of inpatient and outpatient services, including, PTSD,MHICM, Addiction Services, etc. This will be 25% of labor mapped time for direct patientcare.

• Assists with VA regional or local recovery oriented trainings, meetings, or conferences as needed.
• Provides an annual report on implementation progress to the VISN RC and the NRC; other reports will also be prepared as requested.
• Meets monthly via conference call with the VISN RC to coordinate a network response to recovery implementation and implementation of CO policies and procedures related to recovery.
• Participates in facility MH leadership meetings and policy planning groups.
• Provides coordination within the VISN and serves as a Point of Contact to the National Recovery Coordinator and National Recovery Advisory Committee as assigned.

Major duties include, but are not limited to:

• The Supervisory Medical Support Assistant (MSA) position serves as the direct line supervisor for all administrative care coordination assigned PSA and MSA staff which includes at least one Lead MSA and is responsible for the daily oversight of the area in which they are assigned, providing direct supervision, guidance, and leadership. This team may include various MSA or PSA levels ranging from GS-3 through GS-7 levels. The Supervisory Medical Support Assistant is a foundational resource in providing technical counsel to meet procedural, legal, and administrative requirements relating to the care and treatment of patients, thus achieving maximum utilization of this medical center's resources and within the Community Care Transformation Model. Care is delivered in a team-based model which includes Registered Nurses, and MSA's. Community Care arranges all care outside the St. Cloud VA Medical Center for Veterans. Community Care works collaboratively with other members of an expanded healthcare team including pharmacists, social workers, dietitians, and all Clinical Service Lines to provide a robust interdisciplinary approach to care. Important interfaces and collaborations also occur with larger health care system providers including VA and Non-VA specialists, hospitals, diagnostic and treating facilities, and community bases programs.
• The Supervisory MSA plans and completes advance level tasks based on observance of protocols and accepted clinical practices while exercising full supervisory responsibility over subordinate staff that includes at least one subordinate team leader or subordinate supervisor. The Supervisory MSA handles directly or delegates responsibilities for resolutions of problems or unusual situations and makes decisions on appropriate action to be taken. The Supervisory MSA's work is reviewed on an overall basis by the Service Chief for conformity with VHA Directive and Medical Center policies and procedures. The Supervisory (Medical Support Assistant) MSA has thorough knowledge of VHA Directives for: VHA Outpatient Scheduling Processes and Procedures, Knowledge of VA regulations, Systems Redesign Principles, local Medical Center Memorandum/policies, and standard operating procedures. This position provides supervisory medical administrative support and, is an integral member of Community Care leadership to effectively carry out day to day administrative operations of Community Care. The Supervisory MSA has knowledge of eligibility for VA medical care, priorities for care, release of information, health Information Portability and Accountability Act (HIPAA) Law. The Supervisory MSA must be conscientious regarding the confidentiality of the variety of material that will be handled. The position requires a practical knowledge of an extensive body of the organization's rules and regulations governing confidentiality, including but not limited to the Privacy Act, HIPAA, the Freedom of Information Act (FOIA).

• The Supervisory MSA is responsible for preparation of community care official authorizations and correspondence with vendor and patient associated with pre-authorized care for both inpatient and outpatient episodes of community care. Assignments includes but not limited to call center metrics, appointment scheduling management for all community care appointments in collaboration with Veteran and community care vendor (scheduling/checking in/checking out); utilizes secure messaging, telehealth/connected care modalities, scheduling challenges; consult toolbox, and applicable reports/metric timeliness. Incumbent is responsible to manage the administrative aspects of the episode of community care. The incumbent must ensure that work units/teams under his/her immediate supervision are effectively working in close collaboration with the Case Management unit to accomplish a positive Veteran experience.
