About the opportunity:

We have a fantastic new opportunity for a temperature controlled Driver (van provided) to join our existing team based in our Plymouth Distribution Centre.

Reporting into the Transport Manager, you will be responsible for checking, loading and delivering orders to a range of customers on a predetermined route.

Your key responsibilities will also include ensuring the products are carried in the correct conditions with temperature, cleanliness and security all being paramount. As a very visible face to the public and our customers we are looking for the best, and our rate of pay certainly reflects this.

In this role you will be required to be able to lift (products up to 15kg), bend, twist, kneel, and crouch.

Salary/Working Pattern

£33,114.90 per annum, working an average of 52.5 hours per week. Working a rotating pattern of:

Week 1: Monday to Friday for 10.5 hours per day
Week 2: Monday to Saturday, 9.7 M-F + 4 Sat

Set up activities for sports users. Plan small fitness challenges and activities for gym members. Assist in the setting up of furniture for activities and functions. Always maintain a visible presence in the fitness suite to coach and advise users where necessary, Take prospective members on a tour of the facility as required. Due to further expansion of our services we have an exciting opportunity for an experienced, passionate and dedicated Senior Physiotherapist to join our established and highly regarded team here at Nuffield Health.

This is a great opportunity to be considered for a full- time post with us working cross any of our 3 sites within Plymouth: the Nuffield Health Plymouth Hospital, Devonshire FWB Gym and Plymouth FWB Gym. Although full time, consideration can be given to part time or job share for the right applicant. There will be a requirement to work on our rota for weekend inpatient orthopaedic work so willingness and an experience in inpatients is also desirable on application.

The successful candidate must have strong experience in the field of musculoskeletal Physiotherapy and experience working within the inpatient sector.

Responsible to the Lead Associate Chief Pharmacist Clinical Services for strategic planning and delivery of clinical pharmacy services to patients within the Surgery Care Group. Deputises for the Associate Chief Pharmacist Clinical Services along with the other Lead Pharmacists, in their absence as required. Support the Chief Pharmacist / Deputy Chief Pharmacist / Associate Chief Pharmacist in developing, monitoring, and reporting on drug expenditure and medicines budget and the delivery of financial savings as part of the pharmacy CIP. The post holder will be required to represent the clinical pharmacy team and service line at both internal and external committees and forums including the Drug and Therapeutics committee, Medicines Safety Committee, Clinical Governance and Care Group operational meetings as appropriate. Work as a member of the surgery services lines and wider UHP pharmacy team, highly providing specialist clinical pharmacy care to patients across the care group in both the inpatient and outpatient setting. To lead on the implementation of NHSE commissioning decisions relating to the treatment of new technologies across Surgical Care Group and Surgical Care areas, ensuring safe prescribing by the production of protocols and guidelines. Participate in ward rounds, outpatient clinics and relevant MDTs, review prescriptions, challenge treatment choices and communicate highly complex drug and therapeutic information to senior clinicians, senior manager, other professionals, patients, and their carers in order to influence treatment options. To utilise non-medical prescribing and advance practice skills across the care group following collaboration with the MDT. Lead on the monitoring, review, and report on medicines related incidents within Surgery Care Group. To manage, lead and co-ordinate the advanced specialist pharmacists within the surgery care group. In conjunction with the Lead pharmacist for Critical Care, Anaesthetics, Pain and Theatres to support the wider Surgery pharmacy team including advanced specialist (surgery, trauma, orthopaedics, specialist surgery, vascular, neurosurgery, ophthalmology, anticoagulation, dermatology etc) pharmacists, rotational band 6/7 pharmacists and pharmacy technicians in the provision of a safe, effective and efficient clinical pharmacy service in accordance with local and national standards and strategy and to ensure all members of the pharmacy team are trained and competent for their roles. Ensure an appropriate level of clinical pharmacy service is provided to the Surgery Care Group. Configure and implement inductions for pharmacists across Surgery Care areas. Provide education and training to all healthcare professionals including medical students. To act as a role model as a clinical pharmacist and team leader, to pharmacists, technicians, ATOs and all other members of the multidisciplinary team. Practice as a Pharmacist Prescriber / advanced practitioner within a service line within the Surgery Care group working within professional competence and an agreed scope of practice at all times. To engage with Quality Improvement and practice research. Development, implementation, and update of clinical pharmacy policy, relevant to pharmacy practice and medicines in Surgery Care group and surgical care areas. Monitor and evaluate medicines related protocols and guidelines across the Surgery Care group and surgical care areas. Propose policy or service changes which improve services and may have multidisciplinary and resource implications. Assist in the development and implementation of PGDs and non-medical prescribing initiatives, when appropriate. To use your expertise as a pharmacist working within the Surgery Care group and surgical care areas both within the Trust and externally to form working partnerships with other health and social care service providers from across the wider healthcare system. As and advanced specialist pharmacist in your chosen area of practice: - Recommendation of appropriate therapy for individual patients where evidence is not available, controversial evidence exists or other professionals may challenge advice.- Contribute to the organisations wider antimicrobial surveillance programme, implementing local programmes ensuring appropriate antimicrobial prescribing. - Attendance at Consultant/GP, speciality team MDMs. white board/MDT meetings/ward rounds to actively facilitate and provide advice on treatment, including highly complex patients. - Promote compliance by prescribers and other healthcare staff with local/national policies and the South and West Devon Formulary. - Application of highly complex knowledge of the evidence base behind drug therapies used. - Assess clinical requirements of patients as a clinical pharmacist and an independent prescriber through interpretation of clinical data and examination. If required request appropriate tests and examinations for patients directly under your care. - Contribute to pharmaceutical initiatives that help with the seamless transfer of patients between different healthcare settings. Identifies and manages child protection issues and raises awareness with medical and nursing staff. Work in areas subjected to frequent highly unpleasant conditions such as odours/body fluids and patients showing aggression. Liaises with appropriate agencies and members of the multidisciplinary team to ensure care in managed for at risk patients on leaving the emergency department and/or hospital. To undertake the use of moderate physical effort in controlled restraint of patients undergoing procedures with learning disabilities, the manoeuvring of patients in confined spaces and patients presenting with decreased level of consciousness due to trauma, drugs, alcohol. To work closely with members of the multi-disciplinary team both within the department and beyond to ensure care is delivered to the patient both effectively and efficiently. To independently refer patients to other professionals such as clinical specialities, physiotherapy and occupational therapy. To update other staff appropriately on clinical conditions of patients and maintain accurate documentation and computer records for patients in the department and on discharge. Provide written, as well as verbal specialist advice in relation to the care of patients on discharge home from the emergency department. To provide health promotion advice to patients and their families/carers to support the maintenance of their health and well being. To act as a mentor and resource for more junior colleagues, supporting and assisting them to provide care of the highest quality. To supervise the maintenance of accurate, legally acceptable records, both in written format and on the departmental computer system To ensure patient confidentiality is maintained in accordance with the Data Protection Act. Leadership and Management Assist in the orientation, training and development of trainee emergency nurse practitioners to include clinical supervision, ensuring that identified training needs are conveyed to the lead nurse and Matron. . CLINICAL Key Results: To provide an Occupational Therapy service at Plym Bridge House in accordance with the overall aims of the unit and multidisciplinary team. This will include leading of a group programme on the unit; specialist individual and group assessment and treatment of young people; contributing to the clinical understanding of patients within the multidisciplinary team. Key Activities: To provide clinical leadership within the OT team (the band 5 OT and OTA) To plan appropriate assessments and interventions, evaluating the progress of adolescents in accordance with the aims of the multidisciplinary team. To liaise with the multidisciplinary team by attending necessary clinical meetings and providing verbal and written reports as required. To demonstrate knowledge and understanding of relevant protocols and procedures, including risk assessment and clinical governance procedures. To provide specialist Occupational Therapy ADL and CPA reports relevant to the practice setting. ORGANISATIONAL Key Results: To be responsible for planning and delivery of Occupational Therapy within the adolescent unit, prioritizing clinical needs in order to ensure delivery of an efficient and effective service. Post holder fully engaged with activities relating to Policy, Service Development and Clinical Governance. Key Activities: To demonstrate autonomy and initiative in managing the Occupational Therapy Team workload and liaising with other members of the multidisciplinary team in relation to delivery of group work and individual sessions. The development of innovative occupational therapy assessment and intervention procedures and work related to the better care of children, adolescents and families and is supported by appropriate research. PHYSICAL DEMANDS OF THE JOB This section should describe the nature, level, frequency and duration of the physical effort (sustained effort at a similar level or sudden explosive effort) required for the job. It takes account of any circumstances that may affect the degree of effort required, such as working in an awkward position or confined space. The physical skills required in the job should also be described e.g. requirements for speed and accuracy, keyboard, driving skills etc. There is likely to be frequent sitting and standing. Frequent effort to lift and manual handing of equipment. Keyboard work will be used throughout the working day The post holder will be required to maintain competencies relevant to the role. Some training may inflame emotions e.g., Suicide Prevention and Mental Health First Aid. The post holder may need to seek support from colleagues or line manager on such occasions. The post holder will use VDU for high proportion of time. However, all staff will be entitled to a workstation assessment and VDU check. The post holder will be required to work across a range of different community and health care settings. The service requires cover between 0800-1800 therefore some flexibility of working hours will be required to ensure the service is always covered. There will be some evening and weekend requirement. 7. ADDITIONAL INFORMATION FOR ALL POSTS The post holder is required to comply with all relevant policies and procedures pertinent to their post. Bring knowledge of the relevant homelessness legislation, local services and welfare entitlements to the team. Conduct detailed individualised assessments of housing and support needs to patients. Escort patients to appointments. Facilitate reconnection of patients to areas of local connection as appropriate. Advocate and resolve disputes with local authorities and providers of other services. Be familiar with the principles of safeguarding of vulnerable adults. Be committed to involving patients as fully as possible in the process of planning for their care and discharge from hospital. All team members are expected to work collaboratively and supportively with each other across Primary, Secondary Care and Community care boundaries to improve outcomes for people who are homeless. They should also work to the Pathway model (with flexibility as required), to attend team handover and MDT meetings, and to be present to take referrals, and undertake assessments and ward rounds where needed Carry out rapid, detailed assessments of new homeless patients referred to the team, identifying support needs, risks, and any possible rapid routes into housing for the most vulnerable homeless patients. Produce individualised, clear and patient led care plans around any issues affecting the homeless patients health and housing circumstances, including identifying and addressing risks to the patient, staff, and other patients. Provide direct assistance with housing, financial and wellbeing needs including access to primary and specialist health services on discharge. Coordinate and support reconnection of patients to housing and other services in areas of local connection as appropriate. Advise, signpost and refer patients to a range of external services relevant to homeless and multiply excluded patients in their local area. Proactively identify and act on safeguarding issues, ensuring Safeguarding of Vulnerable Adults and Children legislation and relevant service protocols are adhered to. Maintain regular contact with and support a caseload of patients. Accompany patients to housing offices to provide face to face advocacy and support when needed.
