Professional/Clinical Under the direction of the Clinical Director of Pharmacy and supported by the Advanced Clinical Pharmacist you will: Deliver optimum clinical pharmacy services to patients within our care Attend MDTs and consultant ward rounds. The post holder will assist registered staff within the Complex Needs team at harbour Drug and alcohol services, working under supervision, in providing support in the community for people experincing mental health and co-existing substance misues issues. The post holder will work alongside the other services and staff working with people that live complex lives in the community, where they will manage a caseload under supervision from a registered member of staff. The aim is to promote recovery from addiction and improve the health and wellbeing outcomes for individuals, linking them in with Community activities and wider supports which include improving access to activities, meaningful employment, and training. The role will require the post holder to work across the complex lives system, developing in consultation with service users flexible and realistic support packages and person-centred plans within agreed guidelines and service models. This post is seen as crucial in working alongside Registered staff to develop and implement person centred care plans in order that the person using the service can optimise their level of functioning despite any barrier their complex problems may present. JOB PURPOSE The postholder will work within the Plymouth Community Dental Service PCDS to develop, implement and evaluate defined Oral Health Improvement activities. Working closely with clinicians to ensure effective and efficient delivery of oral health programmes in line with national oral health strategies such as Delivering Better Oral Health, an evidence-based toolkit. This role will undertake oral health appointments with patients of the service, including adults/children and those with additional needs or who are medically compromised and to undertake fluoride varnish application under the prescription of a clinician. SCOPE AND RANGE This post will provide a comprehensive oral health service for patients with complex needs, as well as evidence-based oral health promotion to key population groups, in line with national and local strategies for reducing inequalities in oral health. The provision of Oral Health Promotion is multifaceted, requiring a range of inter-personal, presentational, and organisational skills, as well as an understanding of the impact of social and environmental determinants of health. The ability to work independently and flexibly is an essential part of this post. Responsibility for People Management Facilitate and support with on-going staff training in oral health promotion to the wider dental team and other healthcare professionals where appropriate. Ensure oral health improvement programmes are patient focussed. Work together with peers to ensure service promoting oral health; identifying new ways of working regarding oral health promotion where necessary. Collaborate and work in partnership, developing and strengthening professional working relationships to improve oral health outcomes with other health care professionals. Under take appraisals on junior staff where necessary Responsibility for financial and/or physical resources Ensure effective marketing and communication systems are used within the service for oral health promotion, including the use of digital approaches. Act as a point of contact and source of knowledge and skills for both internal and external enquiries to the service regarding oral health. Develop and deliver interventions to improve oral health in conjunction with key national oral health guidelines such as mouth care matters and delivering better oral health toolkit. Comply with standards and evidence-based guidelines developed in the Community Dental Services through its Clinical Governance audit and training programmes Other Responsibilities You may be required to support the oral health nurses with the fluoride varnish project with primary schools and oral health messaging. You will also be required at times to undertake dental nurse duties clinically in surgery. You may be required to represent the service on the Plymouth Oral Health Improvement Group, in conjunction with Plymouth City Council, Public Health and other oral health providers. Ability to be able to visit different locations within the city to support with oral health promotion for the dental service. COMMUNICATIONS AND RELATIONSHIPS This postholder will work with patients of the service, other dental colleagues to inform of up-to-date oral health promotion toolkits. Oral health promotion will also be undertaken with wider Livewell colleagues (such as in-patient settings), local residential homes and local schools. PHYSICAL DEMANDS OF THE JOB This post will undertake oral health activity in a dental surgery setting as well as talks in local schools and residential homes. Driving between different locations will be necessary. Creation of and maintaining of oral health resources as well as collation of oral health promotion evidence to be collated using various administrative systems, such as excel and good keyboard skills required.

Post holder works with special care dentists treating patients with special needs mostlyhaving treatment under conscious sedation. Post holder also provides Dental Sedation Nursing support to all Dentists carrying out Dental Sedation, including Oral Surgery. Post holder has GDC registration and holds the NEBDN certificate in conscious sedationand thereby acts as the second appropriately trained person working with the dentist in line with the British Dental association patient safety guidelines Post holder requires knowledge of complex medical, physical and mental health problems,which has been gained through a minimum of 2 years experience Post holder will be required to monitor sedated patients during both treatment andrecovery, and needs to recognise and assist with medical emergencies. Post holder will discharge patients with the dentists agreement, and be responsible forgiving relevant post operative instructions and care To assist all operators in all chair side assisting. To support and care for patients duringprocedures using conscious sedation. Follows all sedation related local policies and SOPs, including the correct preparation ofequipment and attends local drawing up drugs training with sedation lead. Records clinical observations on patients sedation record Carries out performance checks and daily maintenance on specialist equipment i.e. PulseOximeter, Oxygen cylinders, Scavenger unit and Relative analgesia machine to sure the equipment is ready for use and if faulty to remove from use and report to the relevant technician. Prepares the surgery environment using knowledge and experience to select theappropriate equipment, materials and instruments, using specialist knowledge based on patients medical, physical and emotional needs. Works as part of a multi-disciplinary team in Freedom day case units assisting operator treating patients under general anaesthesia. Providing technical and clinical support for patients undergoing these procedures. Providing post operative care plans for these patients Triage of urgent care patients to assess the urgency of treatment and determine thecorrect booking and then book appointments including outlying clinics. Registered, as a Dental Nurse, with the GDC. Thorough knowledge of dental procedures gained through a minimum of 2 years post-qualification training and experience. Thorough knowledge of routine dental procedures and equipment gained through in-house training and experience including specialised areas such as general anaesthetics. Experienced in treating patients from diverse cultural backgrounds i.e. those who require interpretation services. Knowledge of Basic Life Support techniques and certificate of training and willingness to complete ILS training/certification and regular updates, provided by Livewell Southwest Trained in recognising medical emergencies and assists in dealing with them. Trained by PCDS / Livewell in all Mandatory Training topics, including Manual Handling, Breakaway, Health and Safety when required. Clinical work is carried out without direct supervision using initiative and knowledge of procedures gained through experience and training. The guidance of a dental operator or clinical manager is sought for non-routine clinical procedures. Administrative duties are carried out without direct supervision using knowledge of procedures and referring to a Manager for non-routine procedures. To arrange Homecare provider Service Review Meetings. Monthly for the first 6 months, and then quarterly for established therapies. To arrange urgent Service Review Meetings if a service is not fulfilling its obligations. To participate in regional face-to-face and MS Teams meetings and bi-monthly conference calls with the Peninsula Purchasing Supply Alliance (PPSA). To participate in contract re-tenders. Prescription Management To supervise the prescription management workflow. To support the building, authorisation and distribution of patient specific order prescriptions by the most appropriate methods. To assist the team in the introduction and use of E-Sign (e-Rx). Invoicing To support the various statements of accounts that are received from each Homecare provider in a timely manner and the updating of the statements to accurately reflect the status of debt. To manage/support the download and appropriate storage of invoices from portals and emails To manage the matching and price checking of invoices against Homecare prescriptions. To manage/support the payment of invoices and the resolution of queries. To ensure all due invoices have been passed for payment by month end unless in query. To chase any missing invoices before month end. To manage/support the prompt resolution of credits that have been issued by the Homecare suppliers. Training To attend relevant manager courses. Participate in the PWDS Screening competency when the opportunity arises. Train new members of staff.

PRIMARY DUTIES & AREAS OF RESPONSIBILITY 1. To assist the Advanced Specialist Pharmacist Clinical Trials in ensuring that the documentation, processes and procedures for the handling of IMPs within the pharmacy department meet the relevant current UK and European legislation, ICH GCP guidelines and GMP principles required by the Medicines and Healthcare Regulatory Authority (MHRA), National Patient Safety Alerting System (NPSAS), Regional Pharmaceutical Quality Assurance Services and relevant clinical trial sponsors. 2. To assist in co-ordinating the pharmaceutical input into the setting-up, design and running of clinical trials at the UHPNT. This will involve close liaison with pharmacy, nursing, and medical staff and R&D department. 3. To act as the deputy to the lead pharmacist for the provision of IMPs for all active clinical trials. 4. To support in the critical review of clinical trial protocols on their impact on pharmacy, e.g. packaging, labelling and documentation, incorporating further comments from appropriate specialist clinical and production services pharmacists where required. 5. To assist the Pharmacy Clinical Trials Team in producing and checking costings for clinical trials where necessary. 6. To assist with the raising, completion and implementation of all relevant IMP and NIMP documents, controlled procedures, feasibility assessments, prescriptions, dispensing procedures, accountability logs, and stock management records. 7. To ensure appropriate use of IMPs within the pharmacy and support in the production and maintenance of dispensing procedures for all CTIMPs. 8. To provide written instructions and training for pharmacy staff on how to dispense, label, check, handle the IMPs, and perform code-breaks. 9. To assist with training and accreditation of pharmacy staff for clinical trials. 10. To liaise with the pharmacy clinical trials team to assess the impact of substantial amendments on local trial documentation. 11. To accurately screen, dispense, check prescriptions and counsel patients regarding their medication as required in accordance with the particular study protocol, chemotherapy prescriptions produced on both the electronic prescribing system and paper templates. 12. To supervise the initial check and preparation of IMPs and NIMPs for all active clinical trials. To undertake the final check for these products made under section 37 exemption of the Medicines for Human Use (Clinical Trials) Regulation 2004 taking responsibility for the final release of medicinal products to patients including Intrathecal products. 13. To support the pharmacy investigations into study issues and incidents involving IMP, ensuring appropriate documentation and completion of any actions. 14. To maintain a good working relationship with sponsors, monitors, the R&D department, auditors and regulatory authorities. 15. To ensure that the confidentiality and security of information and data about study subjects and clinical trial studies are maintained and respected. 16. To undertake and maintain a record of personal Continuing Professional Development (CPD). 17. To ensure medicines are prescribed and administered in line with local and national policies, and within the legal framework. 18. To provide day-to-day supervision and support to the pharmacy clinical trials staff and rotational pharmacists working in the Pharmacy Clinical Trials. 19. To coordinate with the Clinical Trials Specialist Assistant Technical Officer the daily workload of injectable IMPs and NIMPs to the wards and departments taking clinical priority and the capacity plan for the unit into account. 20. To be on the Trust register for the release of intrathecal chemotherapy from the pharmacy department according to relevant Department of Health guidelines and local Trust policy. 21. To ensure that safe and effective systems of work are followed within the unit in compliance with the COSHH (1988) Regulations. 22. To undertake audits and engage with Quality Improvement and practice research projects. Education and Training 23. Design and participate in training programmes for pharmacy, medical and nursing staff, where necessary. 24. Technical supervision and mentoring of other members of ward-based staff, including pharmacists, technicians, pharmacy support assistants and undergraduate students. 25. Provide competency based assessment of junior pharmacists, trainee pharmacists and pharmacy technicians and pharmacy support assistants using work-based assessment tools. 26. Train trainee pharmacists in aspects of technical and clinical pharmacy practice to meet the General Pharmaceutical Councils competency requirements, acting as an accredited educational supervisor if required.

Prioritise own workload including the typing of clinics, discharge summaries and medical reports. Ensure effective communication with all members of the multi-disciplinary team. Sort and deal with all correspondence, keeping a satisfactory record and filing system Collate results with patient casenotes, ensuring that they are seen and actioned promptly by the relevant clinician. Take appropriate actions with regard to telephone calls from patients, GPs and other agencies using initiative, within Trust guidelines. Maintain an up to date diary for the Consultants / team, inform relevant staff of cancelled and reduced theatre and outpatient sessions due to Consultant leave, junior doctors leave, on-call commitments, CME, Audit and Directorate meeting. To obtain hospital casenotes and x-rays from both within the Trust and external agencies when required by the Consultant and/or team. 8.To ensure that the casenotes and x-rays are traced accurately using PiMS and CRIS at all times and retuned to Health Records in a timely manner. To raise hospital casenotes as necessary, ensuring that accurate and full details are entered onto PiMS in line with the Trusts AGN for health records Required to cover for other colleagues during annual leave/sickness or peak workloads. Arrange patient admissions as directed by the Consultant/team, taking into account clinical priority ensuring that any relevant investigations or pre-assessment appointments are booked. Formulate Electronic Theatre Lists to the agreed Trust standard format. Ensuring that booking guidelines are followed and reminder service actioned in a timely manner. Inform the ward of patient admissions and provide hospital notes rebook cancelled patients/informing patients of cancellations. Where necessary liase with theatres, other Consultants and departments to facilitate patients admissions. Ensure that appropriate clinic follow up is actioned as appropriate, i.e. out patient appointments, investigations, add patients to waiting list, re-admissions and referrals to other Consultants/departments. Work with the junior doctors to ensure that the discharge summary/pro formas are completed. Ensure timely despatch of clinical letters to GPs. Obtain results ensuring that Consultant /or Medical team see them and action in a timely manner. Inform GPs of patient results via letter dictated by Consultant. Ensure that all results, reports and letters are accurately filed in medical notes. General Admin, including adding referrals to IPM Provide other secretarial assistance to the team as required, typing up correspondence, references for junior staff and administration and maintain filing system of this correspondence. Exposure to distressing/emotional circumstances i.e. typing letters of a distressing nature, dealing with distressed, anxious and angry patients and relatives in person or by telephone. Requirement to exert physical effort when dealing with quantities of notes and x-rays and when retrieving notes from outpatients after Consultant clinics each week and filing them in the office to await typing.
