The veteran, family members, significant others and the interdisciplinary treatment team are involved in this process, whenever possible. The goal of the communication is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status and safely maintain the veteran in his/her home.
Clinical diagnosing using DSM-5 and ICD-10 behavior codes - Incumbent demonstrates knowledge in DSM-5 diagnosing and documents diagnostic impressions in psychosocial assessments when appropriate. Clinician may function independently or within a team to determine an appropriate diagnosis and formulate a treatment plan based on that diagnosis.
Treatment Planning/Goal Setting - Incumbent demonstrates basic knowledge of psychosocial treatment modalities and demonstrates the ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. The social worker is responsible for developing a treatment plan, and setting achievable treatment goals with the veteran/family in collaboration with the primary care interdisciplinary treatment team members when indicated. Referral to service providers - Throughout the course of treatment the social worker is the subject matter expert on VA and/or community resources. The social worker will collaborate with other service providers in reassessing the veteran's needs for VA and community resources and entitlements. The social worker is responsible for educating the veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his/her surrogate decision-maker.
Resource Development - The social worker is responsible for developing a resource file of VA and community social service programs and will refer the veteran to needed services.

The Columbia VA Health Care System is currently recruiting for the position of a Rotational Pharmacy Technician. Pharmacy Technicians at this grade level can work autonomously, using independent judgement to perform a full range of duties. The technician will assist in both outpatient and inpatient Pharmacy areas. Inpatient functions include packaging unit dose medications utilizing unit dose packaging machine, replenishing stock in automated dispensing machines, conducting routine ward inspections, preparing intravenous admixtures, and delivering medications to inpatient wards and outpatient clinics. The Memory Disorder physician will be part of an interdisciplinary team of health care providers, caring for Veterans with dementia. The physician will be assigned to the Neurology Service and will see patients at the North Charleston and Charleston Neurology outpatient clinic. This involves outpatient neurologic consultations, review of screening results completed by the Neuropsychologists and development of treatment plans and future care for veterans. The Should the Memory Disorder physician will collaborate with other cognitive specialists from neurology, geriatrics, geriatric psychiatry, and social work services in the interdisciplinary management of dementia patients. Anti-amyloid infusion therapy and Amyloid Pet Scans are available to the physician at this facility. Suitable candidates will be offered up to 50% of unassigned clinical time for research purposes. This research will be conducted under the umbrella of the state-of-the-art VA Brain Health Center and National Coordinating Center for Research on Alzheimer's Disease and Related Dementias. Should the Memory Disorders physician have a need to refer patients to a subspecialist, these patients can be sent to the Charleston VA neurology clinic for consultations in the fields of epilepsy, headaches, movement disorders, neuromuscular medicine and , MS, and dementia. Charleston VA Neurology Service also has EEG/EMG testing, neurology inpatient service and an epilepsy monitoring unit and is partnering with the academic affiliate, Medical University of South Carolina, on educating medical students, residents and fellows. available. The interaction between the North Charleston Memory Disorder neurologist and the Charleston VA subspecialists will be of collaborative and interactive nature outpatient care and services.

• Documents patient's response to medication and/or prescribed treatment regimens and the reporting of any unexpected change in patient's condition to RN or provider.
• Recognizes urgent or emergent patient care situations, seek assistance of the RN and/or provider, and initiate appropriate emergency interventions as directed.

Major duties include, but are not limited to:

• Initiates, performs and completes assigned duties in providing care to variable patient populations.
• Serves as a preceptor in collaboration with the RN and/or provider to appropriately orientate/train less experienced LPN/LVN and/or Nursing Assistants (NAs)/Health Technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery.
• Carries out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant] patient information. Completed work should need only a general review by a registered nurse (RN) or provider for appropriateness and conformity with established policies/procedures.
• Observes, identifies, and responds to the patient's needs for care, including medication, equipment-assisted care and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors.
• Interacts in an appropriate manner with patients, family, members, professional and other support personnel involved in the delivery of patient care.

Duties Include, but are not limited to:

• Provides patient management, care coordination, and discharge/disposition planning for the outpatient setting.
• Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs within the surgical population.
• Coordinates care or the scheduling with various labs, tests, scans, studies, clinics, multiple disciplines and providers across all levels and sites of care.
• Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice.
• Evaluates care and outcomes to ensure timely and appropriate provision of services. The nurse brings principles and theory with limited application of critical thinking skills.
• Demonstrates leadership on developing and improving holistic care through collaborative strategies with others.
• Participates in activities and strategies to sustain an evidence-based practice culture.
• Evaluates practice in an ongoing process, based on best evidence.
