The role of pharmacy within General Practice is part of an exciting programme of transformation which is underpinned by the NHS Long Term Plan. This is an innovative role to work within the Lakeside Healthcare Pharmacy team to ensure safe and effective use of medicines for patients within Lakeside Healthcare. As a starting point the role will initially focus on medicines reconciliation from clinic and discharge letters, and will develop further as the role of the Pharmacy team expands within the organisation. It is vital that effective systems and procedures are in place to ensure that this process is effective, safe and efficient. The right candidate will be involved in day to day management of this with regular and thorough reviews of all medications. The process also ensures that patients receive the appropriate monitoring necessary for the medication they are prescribed. Job Responsibilities/Principal Duties to include: Medicines reconciliation from clinic and discharge letters Take a central role in the clinical aspects of shared care protocols Level 1 medication reviews to include: Identifying patients on repeat medication who have not had a medication review Check medical notes to assess if any monitoring is required and arrange as appropriate Update medication as appropriate following defined protocols Remove any old or unnecessary medication from repeat Update quantities to provide appropriate supply each month Review dosage instructions to ensure concise and clear instructions Ensure prescribing is in-line with the Lakeside Drug Formulary whilst taking consideration of CCG prescribing guidance Support Clinical Pharmacists with general prescription requests and medication queries to allow them to review the more clinically complex requests Liaise with the pharmacist if there are any concerns regarding patient medication regimens Promote and support the implementation of electronic repeat dispensing Conduct searches for audits and high-risk drug monitoring using clinical systems. Located in a historic Cambridgeshire city, Peterborough City Hospital is a state-of-the-art, purpose-built facility accommodating over 700 inpatient beds on modern wards. Peterborough is accessible by road or train within easy commute distance from London, Cambridge, Leicester or Birmingham.

You will be exposed to a variety of experiences by undertaking clinical duties on NICU, the Transitional Care Unit, PAU and Amazon Ward, as well as participating in General Paediatric clinics.

We deliver over 4500 babies each year and approximately 350 babies are admitted to the unit each year. You will be exposed to and learn practical procedures such as intubation and LISA (less invasive surfactant administration), umbilical vessel catheterisation, long line insertion and chest drains.

You will join an experienced team of Consultants, Registrars, Specialist Doctors and nursing staff who will guide you and ensure you receive a comprehensive training programme with weekly teaching sessions.

Your weekly duties will be covered on a rota basis on a full shift pattern currently 1:8. The weekly average is 46 hours inclusive of annual leave cover. A week of Outpatient Clinics are factored into the rota.

The Paediatric department is located in the separate Woman & Children’s wing which displays a state-of-the-art Maternity Suite with 77 beds and a Neonatal Unit with 20 cots, 6 of which are specifically for intensive care/high dependency babies. In addition, we run a separate 14 bedded Transitional Care Ward. The general Paediatric In-Patient Unit (Amazon Ward) is a 30 bedded unit with a separate 9 bedded Paediatric Assessment Unit and an adjoining Children’s Out-Patient Unit (Rainforest).

You will be encouraged to enter appropriate examinations including MRCPCH and DCH. A structured teaching programme will be offered to you to help to prepare for this.

You will attend monthly audit meeting where you will have the opportunity to present your projects. You will be required to complete at least one audit project during your placement.

All doctors will be supported to undertake the necessary training for Adult and Paediatric Safeguarding.

About the role
It is essential you have current certification with the Australian Health Practitioner Regulation Agency
Reporting to our Care Manager and our Facility Services Manager, as a Registered Nurse you will be required to:
Complete comprehensive, individual assessments and care plans for new & current residents
Provide Leadership and supervision to Nursing Assistants and Enrolled Nurses
Have sound knowledge and procedural responsibility in medication management, wound assessment
Work a range of day, evening and night shifts on a 14 day roster
You will be required to obtain an NDIS Worker Screening Check & National Police Check

**To be considered for this position you must hold an active NMC Pin and an NVQ Level 4 in Health & Social Care**

As the Peripatetic Nurse Manager your key responsibilities include:
• Supporting 2 divisions and maintain regional contact for each Care home
• Conducting routine Head Office visits across care homes as directed by Divisional Directors
• Ensuring that a stimulated and caring environment is always maintained
• Overseeing and ensuring the effective delivery of the medicine administration process in line with the company care management of medicines policy
• Ensuring that attendance to both mandatory and required training courses, including that of staff members within a home, is met in order that skills, knowledge, and competency levels are maintained to carry out the role efficiently and effectively
• Supporting the Home Manager to plan and facilitate regular staff meetings: staff supervision establishing an effective communication system, ensuring that staff members are kept up to date with all current necessary information
• Providing support and assistance to staff members and ensuring that areas of responsibility are maintained and to delegate duties to staff as appropriate, ensuring implementation

The following skills and experience would be preferred and beneficial for the role:
• Previous experience of working with a large multi-site Care Provider within a capacity of a high-Level Manager
• Excellent knowledge of the CQC standards
• Working knowledge of Legislation and the Regulatory Framework underpinning practice in relation to the care of older people
• Flexible and willing to travel and work a variety of shifts, including the occasional weekend shifts

The successful Peripatetic Nurse Manager will receive an excellent salary of £67,900 per annum. This exciting position is a permanent full time role working through days. In return for your hard work and commitment you will receive the following generous benefits:
